We describe a 49-year old man with hemorrhagic fever, type 1 respiratory insufficiency and acute kidney injury. Diagnostic work up showed a hyperinflammatory syndrome, hypertriglyceridemia, hemophagocytosis, very high ferritin and significantly elevated sCD25. The findings were compatible with hemophagocytic lymphohistiocytosis based on the HLH-2004 criteria. Serological testing indentified &lt;i>Puumala virus&lt;/i> as the causal pathogen. The patient was successfully treated with pulse corticosteroids, intravenous immunoglobins and supportive therapy.</pubmed_abstract><journal>Frontiers in medicine</journal><pagination>1096900</pagination><full_dataset_link>https://www.ebi.ac.uk/biostudies/studies/S-EPMC9790895</full_dataset_link><repository>biostudies-literature</repository><pubmed_title>Case report: Hemorrhagic fever with renal syndrome presenting as hemophagocytic lymphohistiocytosis.</pubmed_title><pmcid>PMC9790895</pmcid><pubmed_authors>Bogaert S</pubmed_authors><pubmed_authors>Schauwvlieghe A</pubmed_authors><pubmed_authors>De Smet MAJ</pubmed_authors><pubmed_authors>Dendooven A</pubmed_authors><pubmed_authors>Druwe P</pubmed_authors><pubmed_authors>Depuydt P</pubmed_authors></additional><is_claimable>false</is_claimable><name>Case report: Hemorrhagic fever with renal syndrome presenting as hemophagocytic lymphohistiocytosis.</name><description>Hemophagocytic lymphohistiocytosis may occur in patients with genetic predisposition and in sporadic cases due to malignancy or infection.
