How insulin resistance (IR) affects OSI and pregnancy outcomes during IVF remains unclear.&lt;h4>Methods&lt;/h4>This was a large retrospective, cohort study. A total of 2055 women with polycystic ovary syndrome (PCOS) undergoing the first fresh IVF cycle were enrolled.
