Using multiple analysis algorithms, we identified two molecular endotypes with divergent immune signatures in transcriptome of whole blood from a fHP cohort and validated in transcriptomes from whole blood, peripheral blood mononuclear cells, bronchoalveolar lavage, surgical lung biopsy tissue, and in the proteome from blood plasma. The lymphocytic endotype (L-fHP) comprising two-thirds of fHP cases showed enrichment of adaptive immune pathways and lymphocyte predominance, while the non-lymphocytic endotype (N-fHP) exhibited enrichment of innate pathways with neutrophil and macrophage predominance. The N-fHP cases had lower baseline pulmonary function, and reduced transplant-free survival compared to L-fHP cases, yet corticosteroid therapy was associated with poorer transplant-free survival in L-fHP and without impact in N-fHP. These results show that blood-based molecular endotyping can uncover prognostic and therapeutic insights and advance precision medicine otherwise inaccessible to traditional clinical phenotyping.</description><dates><publication>2026/06/12</publication></dates><accession>GSE318816</accession><cross_references><GSM>GSM9503768</GSM><GSM>GSM9503801</GSM><GSM>GSM9503767</GSM><GSM>GSM9503800</GSM><GSM>GSM9503803</GSM><GSM>GSM9503802</GSM><GSM>GSM9503769</GSM><GSM>GSM9503805</GSM><GSM>GSM9503804</GSM><GSM>GSM9503807</GSM><GSM>GSM9503806</GSM><GSM>GSM9503760</GSM><GSM>GSM9503762</GSM><GSM>GSM9503761</GSM><GSM>GSM9503764</GSM><GSM>GSM9503763</GSM><GSM>GSM9503766</GSM><GSM>GSM9503765</GSM><GSM>GSM9503757</GSM><GSM>GSM9503756</GSM><GSM>GSM9503759</GSM><GSM>GSM9503758</GSM><GSM>GSM9503870</GSM><GSM>GSM9503751</GSM><GSM>GSM9503872</GSM><GSM>GSM9503871</GSM><GSM>GSM9503750</GSM><GSM>GSM9503753</GSM><GSM>GSM9503873</GSM><GSM>GSM9503752</GSM><GSM>GSM9503755</GSM><GSM>GSM9503754</GSM><GSM>GSM9503823</GSM><GSM>GSM9503822</GSM><GSM>GSM9503789</GSM><GSM>GSM9503825</GSM><GSM>GSM9503824</GSM><GSM>GSM9503827</GSM><GSM>GSM9503826</GSM><GSM>GSM9503829</GSM><GSM>GSM9503828</GSM><GSM>GSM9503782</GSM><GSM>GSM9503781</GSM><GSM>GSM9503784</GSM><GSM>GSM9503783</GSM><GSM>GSM9503786</GSM><GSM>GSM9503785</GSM><GSM>GSM9503788</GSM><GSM>GSM9503821</GSM><GSM>GSM9503787</GSM><GSM>GSM9503820</GSM><GSM>GSM9503780</GSM><GSM>GSM9503819</GSM><GSM>GSM9503779</GSM><GSM>GSM9503812</GSM><GSM>GSM9503811</GSM><GSM>GSM9503778</GSM><GSM>GSM9503814</GSM><GSM>GSM9503813</GSM><GSM>GSM9503816</GSM><GSM>GSM9503815</GSM><GSM>GSM9503818</GSM><GSM>GSM9503817</GSM><GSM>GSM9503771</GSM><GSM>GSM9503770</GSM><GSM>GSM9503773</GSM><GSM>GSM9503772</GSM><GSM>GSM9503775</GSM><GSM>GSM9503774</GSM><GSM>GSM9503810</GSM><GSM>GSM9503777</GSM><GSM>GSM9503776</GSM><GSM>GSM9503809</GSM><GSM>GSM9503808</GSM><GSM>GSM9503845</GSM><GSM>GSM9503844</GSM><GSM>GSM9503847</GSM><GSM>GSM9503846</GSM><GSM>GSM9503849</GSM><GSM>GSM9503848</GSM><GSM>GSM9503841</GSM><GSM>GSM9503840</GSM><GSM>GSM9503843</GSM><GSM>GSM9503842</GSM><GSM>GSM9503834</GSM><GSM>GSM9503833</GSM><GSM>GSM9503836</GSM><GSM>GSM9503835</GSM><GSM>GSM9503838</GSM><GSM>GSM9503837</GSM><GSM>GSM9503839</GSM><GSM>GSM9503793</GSM><GSM>GSM9503792</GSM><GSM>GSM9503795</GSM><GSM>GSM9503794</GSM><GSM>GSM9503830</GSM><GSM>GSM9503797</GSM><GSM>GSM9503796</GSM><GSM>GSM9503799</GSM><GSM>GSM9503832</GSM><GSM>GSM9503831</GSM><GSM>GSM9503798</GSM><GSM>GSM9503791</GSM><GSM>GSM9503790</GSM><GSM>GSM9503867</GSM><GSM>GSM9503866</GSM><GSM>GSM9503748</GSM><GSM>GSM9503869</GSM><GSM>GSM9503868</GSM><GSM>GSM9503749</GSM><GSM>GSM9503861</GSM><GSM>GSM9503860</GSM><GSM>GSM9503863</GSM><GSM>GSM9503862</GSM><GSM>GSM9503865</GSM><GSM>GSM9503864</GSM><GSM>GSM9503856</GSM><GSM>GSM9503855</GSM><GSM>GSM9503858</GSM><GSM>GSM9503857</GSM><GSM>GSM9503859</GSM><GSM>GSM9503850</GSM><GSM>GSM9503852</GSM><GSM>GSM9503851</GSM><GSM>GSM9503854</GSM><GSM>GSM9503853</GSM><GPL>24676</GPL><GSE>318816</GSE><taxon>Homo sapiens</taxon></cross_references></HashMap>
