Normally, these changes reverse after pregnancy, but impaired reverse remodeling (RR) increases future cardiovascular risk. Pregnant individuals with HIV may face added risk, potentially from chronic viremia or antiretroviral therapy (ART). Many ART regimens include nucleoside reverse transcriptase inhibitors (NRTIs), which can cause mitochondrial toxicity and affect heart function. Using a rat model, we found that NRTI-containing ART disrupts pregnancy-related cardiac RR by inducing mitochondrial changes in the heart and liver, causing dyslipidemia and reduced cardiac function.
