Compound Kushen Injection (Fufang Kushen Zhusheye, CKI), comprising Radix Sophorae Flavescentis (Kushen) and Rhizoma Smilacis Glabrae (Baituling), is widely used as an adjuvant medicine in the treatment of cancer for pain relief and reduction of cancer therapy side effects. This study assessed the therapeutic potential and explored the associated transcriptomic changes of CKI and its powder form, Compound Kushen Powder (CKP), in alleviating GIM. Methods: Both drugs were evaluated in a rat model of radiation-induced GIM. Following irradiation and administration of either CKP or CKI, transcriptomic analysis of the jejunum and colon mucosa was performed to identify potential biological pathways and generate hypotheses regarding their mode of action. Results: Administration of either CKP or CKI to irradiated rats significantly reduced the severity of GIM symptoms. Transcriptomic profiling identified both shared and distinct pathways affected by the treatments. The transcriptomic profile of CKP-treated tissues showed enrichment in gene sets related to innate immune activation and inflammatory signalling, affecting genes such as Socs3 and Tlr4. Conversely, CKI treatment was associated with tissue repair and oxidative stress resistance through genes including Osm, Epha2, and Stat3. Both forms of the herbal extract correlated with an upregulated stress response and stimulus regulation in the GIM model. Discussion: These findings highlight overlapping transcriptomic changes for CKP and CKI, suggesting potential shared pathways associated with symptom suppression in radiation-induced GIM, along with extract-specific associations on distinct pathways acting on different sets of genes.
