This study evaluates the clinical predictive value of lactate and explores its mechanism involving protein lactylation. Machine learning algorithms applied to multicenter intensive care databases identified preoperative lactate as a dominant predictor of CPB-AKI risk. To validate these predictions biologically, in vivo rat CPB and in vitro cellular models were utilized. Experimental results demonstrated that CPB induces glycolytic reprogramming, lactate accumulation, and elevated global pan-lysine lactylation. Inhibiting lactate dehydrogenase A (LDHA)-mediated lactate production was associated with decreased protein lactylation, lower oxidative stress, and reduced local tissue inflammation, thereby mitigating renal injury. In conclusion, our interpretable ML models identify lactate as a top clinical predictor of CPB-AKI. Mechanistically, the experimental data indicate an association among lactate accumulation, protein lactylation, and renal injury. The lactate-lactylation axis represents a potential mechanistic pathway and candidate therapeutic target that requires further validation.","dates":{"publication":"2026/08/25"},"accession":"GSE338239","cross_references":{"GSM":["GSM9869783","GSM9869782","GSM9869787","GSM9869786","GSM9869785","GSM9869784"],"GPL":["30103"],"GSE":["338239"],"taxon":["Rattus norvegicus"]}}
