Blood RNA-seq was analysed in 38 individuals with ET and 37 controls. Differential expression, phenotype associations, co-expression networks, and cross-tissue concordance with ET cerebellar RNA-seq were assessed. Four genes were significantly upregulated in ET: CXCL5, HSPB1, CLEC7A, and PRDX6. Diagnostic effects were otherwise limited, but within ET, gene expression showed stronger associations with cognition and regional brain structure, including immune, mitochondrial, and erythroid/vascular transcriptional modules. Blood RNA-seq shows modest diagnostic differences in ET but more substantial transcriptional associations with phenotypic heterogeneity.</description><dates><publication>2026/09/16</publication></dates><accession>GSE344274</accession><cross_references><GSM>GSM9973989</GSM><GSM>GSM9973988</GSM><GSM>GSM9973987</GSM><GSM>GSM9973986</GSM><GSM>GSM9973985</GSM><GSM>GSM9973984</GSM><GSM>GSM9973983</GSM><GSM>GSM9973982</GSM><GSM>GSM9973981</GSM><GSM>GSM9973980</GSM><GSM>GSM9973979</GSM><GSM>GSM9973978</GSM><GSM>GSM9973977</GSM><GSM>GSM9973976</GSM><GSM>GSM9973975</GSM><GSM>GSM9973974</GSM><GSM>GSM9973973</GSM><GSM>GSM9973972</GSM><GSM>GSM9973971</GSM><GSM>GSM9973970</GSM><GSM>GSM9974023</GSM><GSM>GSM9974022</GSM><GSM>GSM9974021</GSM><GSM>GSM9974020</GSM><GSM>GSM9973969</GSM><GSM>GSM9973968</GSM><GSM>GSM9973967</GSM><GSM>GSM9973966</GSM><GSM>GSM9973965</GSM><GSM>GSM9973964</GSM><GSM>GSM9974019</GSM><GSM>GSM9973963</GSM><GSM>GSM9974018</GSM><GSM>GSM9973962</GSM><GSM>GSM9974017</GSM><GSM>GSM9973961</GSM><GSM>GSM9974016</GSM><GSM>GSM9973960</GSM><GSM>GSM9974015</GSM><GSM>GSM9974014</GSM><GSM>GSM9974013</GSM><GSM>GSM9974012</GSM><GSM>GSM9974011</GSM><GSM>GSM9974010</GSM><GSM>GSM9973999</GSM><GSM>GSM9974009</GSM><GSM>GSM9973998</GSM><GSM>GSM9973997</GSM><GSM>GSM9974008</GSM><GSM>GSM9974007</GSM><GSM>GSM9973996</GSM><GSM>GSM9974006</GSM><GSM>GSM9973995</GSM><GSM>GSM9973994</GSM><GSM>GSM9974005</GSM><GSM>GSM9974004</GSM><GSM>GSM9973993</GSM><GSM>GSM9973992</GSM><GSM>GSM9974003</GSM><GSM>GSM9974002</GSM><GSM>GSM9973991</GSM><GSM>GSM9974001</GSM><GSM>GSM9973990</GSM><GSM>GSM9974000</GSM><GPL>34284</GPL><GSE>344274</GSE><taxon>Homo sapiens</taxon></cross_references></HashMap>
