Its pathogenesis is closely associated with metabolic dysregulation; however, the key molecular pathways by which acupuncture improves ovarian granulosa-cell development remain unclear. This study aimed to elucidate whether acupuncture ameliorates granulosa-cell dysfunction in PCOS by modulating the arginine metabolic axis. In our clinical study, acupuncture significantly improved sex hormone–binding globulin (SHBG) and AMH levels in women with PCOS, with favorable trends in insulin resistance and body mass index (BMI). Serum proteomics identified argininosuccinate lyase (ASL) as markedly upregulated in PCOS and significantly reduced after acupuncture, indicating arginine metabolism may play a role; targeted amino-acid metabolomics further revealed significant alterations in downstream arginine and ornithine. To validate these findings, we established a dihydrotestosterone (DHT)-induced PCOS rat model, in which ASL and AMH were elevated in ovarian granulosa cells. Ovarian spatial metabolomics indicated that spermine, a downstream polyamine in arginine metabolism, was significantly decreased following acupuncture. In vitro, ASL knockdown in DHT-treated KGN cells reduced AMH expression. Proteomic profiling of KGN cells combined with functional assays demonstrated that ASL knockdown alleviated DHT-induced mitochondrial dysfunction and reshaped arginine-related metabolic pathways. Finally, in vivo spermine supplementation reversed the beneficial effects of acupuncture, supporting a pivotal role for the “ASL–arginine metabolism–spermine” axis. Collectively, these results suggest that acupuncture may downregulate ASL and suppress spermine accumulation, thereby improving granulosa-cell mitochondrial function and the follicular microenvironment and reducing AMH levels, providing a testable metabolic–mitochondrial mechanism and potential biomarkers for acupuncture treatment of PCOS.","dates":{"publication":"Tue May 12 00:00:00 GMT+01:00 2026"},"accession":"PXD078258","cross_references":{"TAXONOMY":["9606"]}}
