Project description:Transcriptomic profiles of synovial biopsies of rheumatoid arthritis (RA) patients who were recruited into the R4RA randomised clinical trial. Patients were randomised to treatment with rituximab or tocilizumab. All patients fulfilled the 2010 ACR/EULAR classification criteria for RA and were eligible for treatment with rituximab therapy according to UK NICE guidelines, i.e. failing or intolerant to csDMARD therapy and at least one biologic therapy (excluding trial IMPs) were recruited when fulfilling the trial inclusion/exclusion criteria. 10x single cell gene expression and V(D)J immune profiling of T cells from the blood, synovial fluid and synovial tissue in psoriatic arthritis patients

Project description:This study shows the pronounced expansion of CD8 T cell clones at sites of active inflammation within the joints of psoriatic arthritis (PsA) patients compared to blood, and characterises the gene expression of expanded clones using droplet based single cell RNA sequencing. Mononuclear cells were separated from freshly acquired paired peripheral blood and synovial fluid samples from 3 PsA patients using density gradient separation, then enriched for CD4 and CD8 T cells using FACS. Additionally, CD45+ leukocytes were enriched by FACS from the cryopreserved synovial tissue of 2 further PsA patients. All cells were then processed using a 10x Chromium Controller and sequenced by Illumina HiSeq 4000 (peripheral blood / synovial fluid) or NovaSeq with S2 flowcell (synovial tissue).

Comprehensive transcriptome analysis of synovium and peripheral blood reveals major disease heterogeneity early in the RA disease process prior to therapeutic intervention

Project description:We report the first comparative analysis between histology, RNA-seq of synovium and matched peripheral blood, and clinico-radiological parameters in early rheumatoid arthritis (RA). Using a novel modular approach, we describe underlying pathways associated with three pre-dominant RA pathotypes. Myeloid was associated with macrophages, lymphoid with B and plasma cells, and fibroid with minimal inflammatory cell infiltration. Synovium RNA-seq was better correlated with the pathotypes than blood RNA-seq, but peripheral blood signatures, including type I interferon, were detected as associated with particular myeloid or lymphoid pathotypes. This study describes the molecular heterogeneity of RA and provides major new insights into the cross compartmental molecular pathways that underlie RA.

Single-cell characterization of anti-LAG3+anti-PD1 treatment in melanoma patients

Project description:We evaluated blood samples from 6 patients with metastatic melanoma treated with anti-LAG3+anti-PD1 (160+480 mg) in a phase I trial (NCT01968109) using single-cell RNA and T cell receptor (TCR) sequencing (scRNA+TCRαβ-seq, 10X 5') combined with other multiomics profiling (flow, cytokine, TCRb-seq) from a larger cohort of 40 patients. This data set include three time points, including baseline, 1 month, and 3 month. The sorting is CD45+.

Evaluation of transcriptomic profiles of peripheral blood myeloid cell subsets of RA patients in remission prior to cDMARD withdrawal, and at the point of flare / 6 months after in sustained remission

Project description:To test the contribution of distinct ST-DC populations to either autoimmunity and inflammation or sustained disease remission, we evaluated the phenotypes of their blood precursors in a human model of disease flare following treatment withdrawal in RA patients in disease remission (the BioRRA study, (Baker et al., 2019)). We investigated the frequency and phenotype of PB DC2 and PB-DC3 of RA patients (n=12) in sustained clinical and ultrasound remission achieved with cDMARDs (Baker et al., 2019) by carrying out, and analysing single cell sequencing data at baseline remission levels, and upon endpoint after treatment withdrawal. We identified that PB DCs differ transcriptionally, but not proportionally, at the baseline of patients who sustain remission, versus those who go on to flare, and at endpoint. We propose that the transcriptomic profile of these peripheral blood DCs could be used as a biomarker of flare in remission RA patients.

Discovery and Confirmation of a Protein Biomarker Panel with Potential to Predict Response to Biologic Therapy in Psoriatic Arthritis

Project description:An integrated discovery to targeted proteomics approach was used to investigate the protein profiles of good and non–responders to anti-TNF-alpha and T-cell inhibitor treatments in PsA patients. Reverse phase liquid chromatography coupled to tandem mass spectrometry was used to generate protein profiles of synovial tissue obtained at baseline from 10 PsA patients who then commenced anti-TNF-alpha therapy (adalimumab). Targeted proteomics using multiple reaction monitoring was used to confirm and pre-validate a potential protein biomarker panel in 18 and 7 PsA patient samples respectively.
