Body composition, physical activity, and quality of life in pediatric patients with inflammatory bowel disease on anti-TNF therapy-an observational follow-up study.

ABSTRACT:

Background

Poor outcome of inflammatory bowel disease (IBD) is associated with malnutrition. Our aim was to compare body composition (BC) and physical activity (PA) between patients with IBD and healthy controls, and to assess the changes in BC, PA and health related quality of life (HRQoL) in children with IBD during anti-TNF therapy.

Methods

32 children with IBD (21 with Crohn's disease (CD), (age: 15.2 ± 2.6 years, 9 male) and 11 with ulcerative colitis (UC), (age: 16.4 ± 2.2 years, 5 male) participated in this prospective, observational follow up study conducted at Semmelweis University, Hungary. As control population, 307 children (age: 14.3 ± 2.1) (mean ± SD) were included. We assessed BC via bioelectric impedance, PA and HRQoL by questionnaires at initiation of anti-TNF therapy, and at two and six months later. The general linear model and Friedman test were applied to track changes in each variable.

Results

During follow-up, the fat-free mass Z score of children with CD increased significantly (-0.3 vs 0.1, p = 0.04), while the BC of patients with UC did not change. PA of CD patients was lower at baseline compared to healthy controls (1.1 vs. 2.4), but by the end of the follow up the difference disappeared.

Conclusions

The fat-free mass as well as PA of CD patients increased during the first six months of anti-TNF treatment. As malnutrition and inactivity affects children with IBD during an important physical and mental developmental period, encouraging them to engage in more physical activity, and monitoring nutritional status should be an important goal in patient care.

Project description:Background: Recently, students' fitness has been declining, and high physical fitness level is crucial in establishing optimal physical/mental health and academic performance. The purpose of this study was to explore the relationship between body composition and upper limb physical fitness and the specific aspects of low physical fitness level in Chinese students. Exploring the development and impact factors for upper limb physical fitness can provide a theoretical basis for the health management strategy of students. Methods: Study 1 collected data from 183 male students over 4 years and used Hierarchical Linear Model (HLM) to explore the quadratic predictive role of body composition on upper limb physical fitness. To further explored which aspects of upper limb physical fitness were affected by body composition, study 2 conducted an experimental investigation among 42 male students, comparing different kinds of upper limb physical fitness within two different body composition groups. Results: Studies found (1) from 2015 to 2018, students' Body-Mass-Index (BMI) showed an upward trend, and BMI differences were significant from year to year. While the upper limb physical fitness showed a downward trend. There were significant differences in the number of pull-outs between 2015 and 2016, 2015 and 2017, and 2015 and 2018. (2) The quadratic term of BMI could predict the upper body physical fitness in the same year and the following year. That is, when BMI was medium, the upper body fitness of the same year and the following year was the best. (3) Chinese students with excellent body composition had greater grip strength, drape height and anaerobic power than those with average body composition. Conclusion: In recent years, male students' BMI has been increasing, and the upper body physical fitness has been decreasing. Furthermore, body composition can predict the upper body mass in the same year and the second year, and male students with better body composition also had greater grip strength, drape height and anaerobic power in their upper limbs.

Project description:This study sought to analyze the influence of occupational stress on the body composition of hospital workers after one year of follow-up. This prospective cohort study included 218 workers from one of the leading private hospitals in the municipality of Santo Antônio de Jesus, Recôncavo da Bahia region, Northeast Brazil. Body composition was analyzed by proxy (Body Mass Index and Waist Circumference) and Bioelectrical Impedance Analysis. The primary exposure adopted in the present study was the perception of occupational stress, assessed with the adapted and reduced version of the Job Content Questionnaire evaluating demand and control dimensions. The covariates were work characteristics; biological characteristics; sociodemographic characteristics and lifestyle. Statistical analyses were performed using descriptive, bivariate and multivariate analysis. At the first stage of the study, we identified that 55.96% (n = 122) of workers had high work demand and 25.22% (n = 55) had low control. Among those who had high demand and low control at the beginning of the study, the majority were women, non-white, with low educational and income levels, sleeping less than 7 h/day. After 12 months of follow-up, the median value for demand continued as 13 (IQR: 5-25) and for control, it increased to 16 (IQR: 9-23). In this second moment of the study, 62.38% (n = 136) of workers showed high demand and 45.87% (n = 100) low control. The characteristics of workers with high demand and low control were similar to those of the first moment. The results indicate that high demand and low control at work are risk factors for changes in body mass index, fat mass and fat-free mass in hospital workers. This study shows the importance and need for clinical and epidemiological assessments regarding the body composition of professionals working in hospitals, since high rates of overweight and obesity are triggers of chronic health problems such as dyslipidemia, diabetes mellitus and cardiovascular diseases, among others. Therefore, managers must promote adequate working conditions and understand the need for periodic body composition assessments.
