"I know why I am taking this pill": Young women navigation of disclosure and support for PrEP uptake and adherence in Eastern Cape Province, South Africa.

ABSTRACT: There is limited understanding of the dynamic interplay between adolescent girl's and young women's (AGYW) disclosure and social support for using oral pre-exposure prophylaxis (PrEP) and adherence. Towards this, we conducted interviews with 42 AGYW enrolled in The Community PrEP Study who exhibited either high or low blood concentrations of tenofovir-diphosphate (TFV-DP) in dried blood spots. Guided by Theories of Practice, interviews and analysis focused on AGYW perspectives and experiences with PrEP disclosure, support and adherence. AGYW with high TFV-DP blood concentrations described larger social support networks and disclosure events. In contrast, those with low TFV-DP blood concentrations described disclosing to fewer people, resulting in limited social support. Participants discussed partner support, however, this support was not described as consequential to adherence, irrespective of TFV-DP levels. Those with high levels of TFV-DP in their blood described the ability to navigate social scrutiny and changes in social support, while those with low levels of TFV-DP in their blood were more likely to question their own continued use of PrEP. To facilitate AGYW's prevention-effective use of PrEP, expanded skill-building for disclosure and resiliency against changes to social support should be examined as part of PrEP services.

Project description:IntroductionFEM-PrEP did not demonstrate a reduction in HIV acquisition because of low study pill adherence. Yet, plasma and intracellular drug concentrations indicated that some participants had evidence of recent pill use. We conducted a follow-up study to identify, among other topics, participants' reasons for taking the study pill.MethodsQualitative, semi-structured interviews (SSIs) were conducted with 88 FEM-PrEP participants. Participants were purposefully selected based on their adherence drug concentrations collected during FEM-PrEP and placed into three adherence interview groups: "high," "moderate," and "none/scarce." Participants in the high and moderate groups described reasons why they adhered most or some of the time, including factors that facilitated their adherence. Participants in all groups described what they believed made it possible for other FEM-PrEP participants to adhere. In addition, 224 FEM-PrEP participants reported on their reasons for taking the study pills through a quantitative, audio computer-assisted self-interview (ACASI). Thematic analysis and descriptive statistics were used to analyze the qualitative and quantitative data, respectively.ResultsFive themes were identified from the SSIs as facilitating factors of adherence: 1) participants' support for the research, 2) HIV risk reduction, 3) routine formation and use of tools, 4) adherence counseling, and 5) partner awareness and support. Participants described similar facilitators when they spoke about other participants' adherence. Among the 172 participants who reported in ACASI that they had taken a study pill, wanting to help answer the research question was the most frequently stated reason for taking the pills (94%, n = 161). We also found evidence of preventive misconception.ConclusionsAdherence was facilitated by personal motivations, such as risk reduction and interest in the research outcome, and by adherence strategies consisting of external cues, reminders, and support. These findings can inform future HIV prevention clinical trials and the rollout of effective antiretroviral-based HIV prevention technologies for women.

Project description:IntroductionAdolescent girls and young women (AGYW) face barriers that jeopardize their prevention-effective use of daily oral pre-exposure prophylaxis (PrEP). We sought to understand factors that influence AGYW's prolonged breaks in PrEP use, and their decisions to re-initiate or discontinue using PrEP in the context of a community-based adherence support intervention.MethodsIn-depth interviews (IDIs) were conducted between December 2019 and April 2021 with purposively selected AGYW (aged 16-25) enrolled in the Community PrEP Study (CPS) in Buffalo City Metro Health District, Eastern Cape Province, South Africa. AGYW were offered monthly PrEP for 24 months at two community-based study sites. Interview guides were informed by the Information-Motivation-Behavioural Skills Model, and data were analysed using illustrative code reports and a case analysis.ResultsA total of 603 participants were enrolled and initiated on PrEP in the parent study. Fifty-three IDIs were conducted with 50 CPS participants. Findings revealed that external factors (e.g. local movement, school holidays and medication side-effects) and social conflicts (e.g. discretion and partner mistrust) directly influenced breaks in PrEP usage. A decrease in one's self-perception of HIV risk prolonged the duration of these "PrEP breaks." Once PrEP refill visits were missed, some AGYW delayed returning for refills out of fear of being scolded by study staff. The differences between those participants who eventually re-initiated PrEP and those who disengaged from PrEP use can be attributed to social support and encouragement, level of familiarity with PrEP, risk perceptions, self-initiated discussions with staff and diminishing side effects.ConclusionsDespite implementing a community-based PrEP delivery platform and behavioural intervention that included support for daily oral PrEP adherence and disclosure, participants struggled with consistent daily oral PrEP use. Unpredictable life events, including local movement and schooling schedules, in addition to being judged for their perceived behaviours, pose a challenge for consistent pill pick-up for AGYW and habit formation. Long-acting injectable PrEP may mitigate a number of these external barriers. Interventions that integrate long-term planning skills, how to navigate existing social judgements and how to access sources of social support may further improve habit formation for PrEP use, regardless of its formulation.

Project description:Water scarcity remains a major threat to most rural areas in Africa due to heavy reliance on rainfed agriculture. This prompts the need to document and understand the determinants of traditional knowledge of water conservation practices. Our aim here is to document and identify the determinants of people's knowledge of traditional knowledge of water conservation in the South Africa's Eastern Cape province. A mixed research method was used. Purposive approach was used to collect data, i.e., elderly people were targeted during the face-to-face pre-COVID-19 pandemic interviews on the basis that traditional knowledge increases with age. However, people of different age groups were also included in the study especially when we employed only online questionnaire during the pandemic. We used questionnaire to collect all the data, and 93 people responded in total to our questions. We documented 10 traditional water conservation technologies. We identified rainfall variation, water quality issues, and increased water demand as the major challenges linked to these technologies, while easy access and the amount of water harvested are reported as benefits of these technologies. We found that traditional knowledge is not influenced by age (β = -0.006 ± 0.01, P = 0.64) or gender (β = -0.16 ± 0.25, P = 0.64) but rather correlates positively with geographic location, irrespective of the starting point of the distance measurement (distance from Port Elizabeth city: β = 0.002 ± 0.0008, P = 0.004; distance from Ngqushwa village: β = 0.0024 ± 0.0009, P = 0.008). Counterintuitively, formally educated people tend to have more traditional knowledge, but this is likely linked to the modern technologies (online survey and social media platforms) used to collect data during the COVID-19 pandemic. We suggest that traditional knowledge that has sustained life for centuries in rural communities must be integrated into water resource management to address water scarcity issues in rural Africa.

Project description:Traditional ecological knowledge among indigenous communities plays an important role in retaining cultural identity and achieving sustainable natural resource management. Hundreds of millions of people mostly in developing countries derive a substantial part of their subsistence and income from plant resources. The aim of this study was to assess useful plant species diversity, plant use categories and local knowledge of both wild and cultivated useful species in the Eastern Cape province, South Africa.The study was conducted in six villages in the Eastern Cape province, South Africa between June 2014 and March 2017. Data on socio-economic characteristics of the participants, useful plants harvested from the wild, managed in home gardens were documented by means of questionnaires, observation and guided field walks with 138 participants.A total of 125 plant species belonging to 54 genera were recorded from the study area. More than half of the species (59.2%) are from 13 families, Apiaceae, Apocynaceae, Araliaceae, Asparagaceae, Asphodelaceae, Asteraceae, Fabaceae, Lamiaceae, Malvaceae, Myrtaceae, Poaceae, Rosaceae and Solanaceae. More than a third of the useful plants (37.6%) documented in this study are exotic to South Africa. About three quarters of the documented species (74.4%) were collected from the wild, while 20.8% were cultivated and 4.8% were spontaneous. Majority of the species (62.4%) were used as herbal medicines, followed by food plants (30.4%), ethnoveterinary medicine (18.4%), construction timber and thatching (11.2%). Other minor plant use categories (1-5%) included firewood, browse, live fence, ornamentals, brooms and crafts.This study demonstrated that local people in the Eastern Cape province harbour important information on local vegetation that provides people with food, fuel and medicines, as well as materials for construction and the manufacturing of crafts and many other products. This study also demonstrated the dynamism of traditional ecological knowledge, practices and beliefs of local people demonstrated by the incorporation of exotic plants in their diets and indigenous pharmacopoeia.
