Time to Publication for Randomized Clinical Trials Presented as Abstracts at Three Gastroenterology and Hepatology Conferences in 2017.

ABSTRACT:

Background & aims

Results of randomized clinical trials are often first presented as conference abstracts, but these abstracts may be difficult to find, and trial results included in the abstract may not be followed by subsequent journal publications. In a review of abstracts submitted to eight major medical and surgical conferences in 2017, we identified 237 abstracts reporting primary results of randomized clinical trials accepted for presentation at three major gastroenterology and hepatology conferences. The aims of this new analysis were to determine the publication rate for these abstracts and the proportion of publications that included trial registration numbers in the publication abstract.

Methods

Clinical trial registries, PubMed, Europe PMC, and Google Scholar were searched through November 1, 2021, for publications reporting trial results for the selected abstracts. Publications were reviewed to determine if they included a trial registration number and if the registration number was in the abstract.

Results

Publications were found for 157 abstracts (66%) within four years of the conference. Publications were found more frequently for the 194 abstracts reporting results of registered trials (144, 74%) than for the 43 abstracts reporting unregistered trials (13, 30%), but only 67% of these 144 publications included the registration number in the publication abstract. Ten unpublished trials had summary results posted on ClinicalTrials.gov.

Conclusions

Clinical trial results could be more accessible if all trials were registered, authors included registration numbers in both conference and journal abstracts, and journal editors required the inclusion of registration numbers in publication abstracts for registered clinical trials.

Project description:Research presented at conferences may increase context-specific evidence in low- and middle-income countries (LMICs), where global childhood disease burden is greatest and where massive relative deficits in research persist. Publication of studies presented at conferences is necessary for complete results dissemination. Our objective was to determine the frequency of publication of pediatric global health conference abstracts and to identify factors associated with publication. We conducted a cross-sectional study of abstracts that reported pediatric research conducted in at least one LMIC presented at seven major scientific conferences in 2017, 2018, and 2019. We used PubMed, EMBASE and Google Scholar to search for publications of the results presented as abstracts. We created a Kaplan-Meier curve to determine the cumulative incidence of publications and used predetermined abstract-level factors to create a multivariable Cox proportional hazard model to identify factors associated with time to publication. There were 8,105 abstracts reviewed and 1,433 (17.7%) reported pediatric research conducted in one or more LMICs. The probability of publication of pediatric global health abstracts was 33.6% (95% confidence interval [CI] 31.2-36.1%) at 24 months and 46.6% (95% CI 44.0-49.3%) at 48 months. Abstracts that reported research conducted in East Asia and Pacific (adjusted hazard ratio [aHR] 3.06, 95% CI 1.74-5.24), South Asia (aHR 2.25, 95% CI 1.30-3.91%), and upper-middle-income countries (1.50, 95% CI 1.12-2.02) were published sooner than those that reported research in LMICs in Europe and Central Asia and lower-middle-income countries, respectively. Fewer than half of pediatric global health abstracts were published in peer-reviewed journals up to four years after presentation at international conferences. Efforts are urgently needed to promote the widespread and long-lasting dissemination of pediatric research conducted in LMICs presented as abstracts to provide a more robust evidence base for both clinical care and policy related to child health.

Project description:ImportanceConference proceedings are platforms for early communication and dissemination of relevant and timely topics of interest. More than half of abstracts presented at biomedical conferences fail to be published in full, resulting in wasted time and resources.ObjectiveTo systematically review reports evaluating the proportion of abstracts presented at eye and vision conferences that are subsequently published in full and investigate factors associated with publication.Data sourcesMEDLINE, Embase, the Cochrane Central Register of Controlled Trials, Web of Science, and reference lists of included reports were systematically searched from inception to January 11, 2019.Study selectionReports that examined the proportion of abstracts presented at eye and vision conferences and subsequently published in peer-reviewed journals 24 or more months later.Data extraction and synthesisTwo reviewers independently assessed study eligibility, abstracted data, and evaluated the risk of bias. A meta-analysis was conducted to determine the proportion of abstracts published in full and assess factors associated with subsequent full publication.Main outcomes and measuresProportion of abstracts presented at eye and vision conferences subsequently published in full.ResultsThere were 19 reports covering 12 261 abstracts presented at 11 unique eye and vision conferences. The overall risk of bias of the reports was low. The weighted proportion of abstracts published in full was 38.0% (95% CI, 31.7%-44.3%) and 54.9% (95% CI, 34.6%-73.7%) among reports restricted to abstracts describing randomized clinical trials. Nine reports (47.4%) investigated the proportion of abstracts subsequently published by ophthalmic subspecialties, ranging from 28.3% (oculoplastics: 95% CI, 17.2%-42.9%) to 42.7% (glaucoma: 95% CI, 34.7%-51.0%). Oral presentation (risk ratio, 1.45; 95% CI, 1.20-1.76) and basic science (risk ratio, 1.25; 95% CI, 1.05-1.47) were significantly associated with higher full publication; factors not significantly associated with full publication included positive results, randomized clinical trial vs other study design, multicenter study, and industry funding.Conclusion and relevanceMore than 60% of abstracts presented at eye and vision conferences were not published in full within 2 years of conference presentation. Failure to disseminate research studies in peer-reviewed journals is not desired, especially when involving human participants.
