Factors Associated with Body Image and Self-Esteem in Mastectomized Breast Cancer Survivors.

ABSTRACT: Body image (BI) and self-esteem (SE) are two fundamental aspects in the evolution of breast cancer (BC), mainly due to surgery, treatment, and the patient's conception of BI. A dissatisfaction with BI and low SE decreases the subject's quality of life and increases the risk of recurrence and mortality by BC. The aim of this study is to find out if there is any degree of association between the sociodemographic data of the sample and their BI and SE. A cross-sectional, descriptive study was conducted with 198 women diagnosed with BC, aged 30-80 years, in Mexico. Women's BI and SE were assessed using two questionnaires, Hopwood Body Image Scale (S-BIS) and Rosenberg Self-Esteem Scale (RSES). The results show significant differences in several items when the variable sense of humor is taken into account, indicating that women with a sense of humor report higher satisfaction with their BI and higher SE. The age also indicates a significantly better BI in women over 50 years of age, as well as the education level variable, where those women who had studied up to secondary reported higher satisfaction with their BI; the family history shows that those women without a family history report better SE. All these data are supported by stepwise regression, which shows that educational level and sense of humor are predictors of BI, and family history along with breast reconstruction and sense of humor are predictors as of SE. In conclusion, it is important to take into account the characteristics of women with BC, particularly age and sense of humor, in order to reduce the impact of the disease on their BI and SE with the help of a multidisciplinary team.

Project description:BackgroundThe majority of breast cancer survivors (BCSs) experience body image concerns following treatment. Body Image distress (BID) is associated with psychological distress and diminished quality of life. A web-based self-compassion focused writing activity (My Changed Body - MyCB) reduces BID in BCSs, yet limited research exists on participant characteristics associated with such intervention adherence. Self-compassion-based meditations are also efficacious in reducing BID in non-BCS populations. This parallel, double-blind pilot randomised controlled trial aimed to assess the feasibility and acceptability of MyCB, with and without an additional meditation component, on BID and related psychological outcomes in BCSs. The trial was registered with the Australian and New Zealand Clinical Trials Registry (#ACTRN12619001693112).MethodsBCSs were randomly allocated to MyCB (n = 39), MyCB + Meditation (MyCB + M) (n = 17) or an expressive writing (EW) active control arm (n = 23). The primary outcome was BID. Secondary outcomes were body appreciation, affect (positive and negative), psychological distress (depression, anxiety and stress) and self-compassion (state and trait). Assessments were completed online at baseline, post-intervention and 1-month.ResultsAdherence to the MyCB writing (45%) and meditation (50%) was modest, and acceptability was high for both MyCB and MyCB + M. Intent to treat linear mixed model analyses indicated: Post-intervention - state self-compassion and positive affect increased for MyCB compared to EW; 1-month: BID scores decreased across all conditions; trait self-compassion increased and anxiety decreased for MyCB + M compared to MyCB and EW.ConclusionThese findings provide preliminary evidence for the efficacy and potential clinical use of the MyCB brief web-based self-compassion intervention alone and with the addition of meditation, to increase self-compassion and psychological wellbeing in BCSs.

Project description:PurposeTo synthesise the effectiveness of exercise interventions on self-perceived body image, self-esteem and self-efficacy in women diagnosed with breast cancer who are undergoing or have completed primary adjuvant treatments.MethodsA systematic review was conducted with meta-analysis and meta-regressions. Five electronic databases were searched from inception to June 2023, and hand searches were performed to explore the reference lists of similar systematic reviews. The established selection criteria were randomised clinical trials that evaluated any type of physical exercise intervention with self-perceived body image, self-esteem and self-efficacy as outcomes. No restrictions were imposed with respect to the control group. Main characteristics were extracted for each study. Meta-analyses, meta-regressions and sensitivity analyses were performed. The certainty of evidence for each outcome was graded using the GRADE approach. The risk of bias was evaluated using the RoB2 Cochrane tool.ResultsTwenty studies, comprising 19 different samples (n = 2030), were included. In general, meta-analysis indicated that physical exercise interventions were not superior to controls for improving self-esteem and body image in women diagnosed with breast cancer. However, subgroup meta-analysis showed a significant difference in self-esteem improvement for resistance exercise (SMD = 0.31; 95% CI = 0.07, 0.55; p = 0.01; I2 = 0%) and supervised exercise (SMD = 0.25; 95% CI = 0.08, 0.42; p = 0.0004; I2 = 0%) compared with controls. Self-efficacy results were scarce and controversial. In addition, serious concerns were mainly detected in terms of the risk of bias and indirectness of the evidence, which caused the certainty of evidence to be very low for all outcomes.ConclusionSupervised exercise and resistance training appear to be effective exercise modalities for improving self-esteem in women diagnosed with breast cancer. In contrast, exercise interventions are not significantly associated with improvements in body image, while results on self-efficacy are controversial. However, due to the study's limitations, further research is needed.

Project description:IntroductionAdolescence is the period in which individual and social identity, including self-esteem and body image perception, is consolidated. During the COVID-19 pandemic, lockdowns were ordered, and adolescents experienced an abrupt disruption in their lives. The aim of this study was to assess the impact of the COVID-19 pandemic (pre- or postpandemic period) on self-esteem and body image.MethodologyA cross-sectional study of Spanish adolescents was conducted between 2016 and 2023. Participants were categorized according to the COVID-19 period (pre- or postpandemic period). The Rosenberg Questionnaire and Body Shape Questionnaire (BSQ) were administered to evaluate self-esteem and satisfaction with body image.ResultsA total of 627 adolescents were analysed. Of these adolescents, 297 (47.3%) were included in the postpandemic group. In both girls (32.75 ± 5.23 vs. 28.19 ± 6.59; p < 0.001) and boys (33.53 ± 4.08 vs. 32.18 ± 4.87; p = 0.005), significant differences in the Rosenberg Questionnaire and BSQ scores were observed between the prepandemic group and the postpandemic group. Adolescents in the postpandemic group (adjusted OR [aOR]: 5.24; 95% CI: 2.61-10.51; p < 0.001) had a greater risk of low self-esteem. Both good and regular-poor self-perceived health were significantly associated with female sex. Moreover, adolescents in the postpandemic group (aOR: 3.06; 95% CI: 1.94-4.82; P < 0.001) and female adolescents with self-perceived good and fair-poor health had an increased risk of mild self-esteem. Additionally, the postpandemic period (aOR: 5.42 95% CI: 2.96-9.93; p < 0.001), female sex, obesity and regular-poor self-perceived health were significantly associated with dissatisfaction about body shape.ConclusionsAdolescents in the postpandemic group had worse self-esteem and body image scores, and adolescents in this group were more likely to be female and had a greater risk of low self-esteem and body dissatisfaction.
