Evaluation of effectiveness and improvement factors of occupational health and safety management system in the Republic of Korea Navy based on AHP-entropy and IPA.

ABSTRACT: The Republic of Korea Navy (ROKN) has applied Occupational Health and Safety Management System (OHSMS), but the effectiveness of OHSMS is being questioned due to continuous industrial accidents that continue to occur. Although OHSMS, which has been generally applied in enterprises, has more potential for improper implementation in the military, there are few studies on OHSMS in the military. Therefore, this study verified the effectiveness of OHSMS in the ROKN and derived improvement factors. This study was conducted in a two-step process. First, we surveyed 629 workers at the ROKN workplaces to confirm the effectiveness of OHSMS by comparing occupational health and safety (OHS) efforts according to whether OHSMS was applied and the period of application. Second, 29 naval OHSMS experts evaluated the factors for improving OHSMS using two decision-making tools: Analytic Hierarchy Process (AHP)-entropy and Importance-Performance Analysis (IPA). The study results indicate that the OHS efforts of OHSMS-applied workplaces were similar to that of unapplied. Also, no better OHS efforts were identified in workplaces with more extended OHSMS application periods. There were five improvement factors of OHSMS applied to the ROKN workplaces, with the highest weight in the following order: consultation and participation of workers; resources; competence; hazard identification and risk assessment; and organizational roles, responsibilities, and authorities. The effectiveness of OHSMS in the ROKN was insufficient. Therefore, the ROKN needs focused improvement efforts on the five requirements to implement OHSMS practically. These results can be helpful information for the ROKN to apply OHSMS more effectively for industrial safety.

Project description:The purpose of this paper was to review the problems relating to Korea's occupational health services and suggest ways to improve them. Korea can be classified as a welfare state type of conservative corporatism partially interwoven with liberalism. While experiencing compressed economic growth, the economic sectors of developed (excess areas) and developing (deficient areas) countries are interwoven. Therefore, it is necessary to perfect conservative corporatism along with a complementary reinforcement of liberal contents and to apply a multilayered approach focusing on complementing the deficient areas. It is essential to form a national representative indicator related to occupational health, and a strategy for selection and concentration is needed. The proposed central indicator is the occupational health coverage rate (OHCR), which is the number of workers who have applied for mandatory occupational health services under the Occupational Safety and Health Act in the numerator with the total working population in the denominator. This paper proposes ways to raise the OHCR, which is currently at the level of 25%-40%, to 70%-80%, which is the level of Japan, Germany, and France. To achieve this target, it is necessary to focus on small businesses and vulnerable workers. This is an area of market failure and requires the active input of community-oriented public resources. For access to larger workplaces, the marketability of services should be strengthened and personal intervention using digital health resources should be actively attempted. Taking a national perspective, work environment improvement committees with tripartite (labor, management, and government) participation for improvement of the working environment need to be established at the center and in the regions. Through this, prevention funds linked to industrial accident compensation and prevention could be used efficiently. A national chemical substance management system must be established to monitor the health of workers and the general public.

Project description:Prion gene (PRNP) mutations are associated with diverse disease phenotypes, including familiar Creutzfeldt-Jakob Disease (CJD), Gerstmann-Sträussler-Scheinker disease (GSS), and fatal familial insomnia (FFI). Interestingly, PRNP mutations have been reported in patients diagnosed with Alzheimer's disease, dementia with Lewy bodies, Parkinson's disease, and frontotemporal dementia. In this review, we describe prion mutations in Asian countries, including Republic of Republic of Korea, China, and Japan. Clinical phenotypes and imaging data related to these mutations have also been introduced in detail. Several prion mutations are specific to Asians and have rarely been reported in countries outside Asia. For example, PRNP V180I and M232R, which are rare in other countries, are frequently detected in Republic of Korea and Japan. PRNP T188K is common in China, and E200K is significantly more common among Libyan Jews in Israel. The A117V mutation has not been detected in any Asian population, although it is commonly reported among European GSS patients. In addition, V210I or octapeptide insertion is common among European CJD patients, but relatively rare among Asian patients. The reason for these differences may be geographical or ethical isolation. In terms of clinical phenotypes, V180I, P102L, and E200K present diverse clinical symptoms with disease duration, which could be due to other genetic and environmental influences. For example, rs189305274 in the ACO1 gene may be associated with neuroprotective effects in cases of V180I mutation, leading to longer disease survival. Additional neuroprotective variants may be possible in cases featuring the E200K mutation, such as KLKB1, KARS, NRXN2, LAMA3, or CYP4X1. E219K has been suggested to modify the disease course in cases featuring the P102L mutation, as it may result in the absence of prion protein-positive plaques in tissue stained with Congo red. However, these studies analyzed only a few patients and may be too preliminary. The findings need to be verified in studies with larger sample sizes or in other populations. It would be interesting to probe additional genetic factors that cause disease progression or act as neuroprotective factors. Further studies are needed on genetic modifiers working with prions and alterations from mutations.
