The effects of contemporaneous air pollution on COVID-19 morbidity and mortality.

ABSTRACT: We examine the relationship between contemporaneous fine particulate matter exposure and COVID-19 morbidity and mortality using an instrumental variable approach. Harnessing daily changes in county-level wind direction, we show that arguably exogenous fluctuations in local air quality impact the incidence of confirmed COVID-19 cases and deaths. We find that a one μ g/m 3 increase in PM 2.5, or 15% of the average PM 2.5 concentration in a county, increases the number of same-day confirmed cases by 1.8% from the mean case incidence in a county. A one μ g/m 3 increase in PM 2.5 increases the same-day death rate by just over 4% from the mean. These effects tend to increase in magnitude over longer time horizons and are robust to a host of sensitivity tests. When analyzing potential mechanisms, we also demonstrate that an additional unit of PM 2.5 increases COVID-19-related hospitalizations by 0.8% and use of intensive care units by 0.5% on the same day. Using individual case records, we also show that higher PM 2.5 exposure at the time of case confirmation increases risk of later mechanical ventilation and mortality. These results suggest that air pollution plays an important role in mediating the severity of respiratory syndromes such as COVID-19.

Project description:BackgroundAmbient air pollution is a known risk factor for several chronic health conditions, including pulmonary dysfunction. In recent years, studies have shown a positive association between exposure to air pollutants and the incidence, morbidity, and mortality of a COVID-19 infection, however the time period for which air pollution exposure is most relevant for the COVID-19 outcome is still not defined. The aim of this study was to analyze the difference in association when varying the time period of air pollution exposure considered on COVID-19 infection within the same cohort during the first wave of the pandemic in 2020.MethodsWe conducted a cross-sectional study analyzing the association between long- (10- and 2-years) and short-term (28 days, 7 days, and 2 days) exposure to NO2 and PM2.5 on SARS-CoV-2 incidence, morbidity, and mortality at the level of county during the first outbreak of the pandemic in spring 2020. Health data were extracted from the German national public health institute (Robert-Koch-Institute) and from the German Interdisciplinary Association for Intensive Care and Emergency Medicine. Air pollution data were taken from the APExpose dataset (version 2.0). We used negative binomial models, including adjustment for risk factors (age, sex, days since first COVID-19 case, population density, socio-economic and health parameters).ResultsWe found that PM2.5 and NO2 exposure 28 days before COVID-19 infection had the highest association with infection, morbidity as well as mortality, as compared to long-term or short-term (2 or 7 days) air pollutant exposure. A 1 μg/m3 increase in PM2.5 was associated with a 31.7% increase in incidence, a 20.6% need for ICU treatment, a 23.1% need for mechanical ventilation, and a 55.3% increase in mortality; an increase of 1 μg/m3 of NO2 was associated with an increase for all outcomes by 25.2 - 29.4%.ConclusionsOur findings show a positive association between PM2.5 and NO2 exposure and the clinical course of a SARS-CoV2 infection, with the strongest association to 28 days of exposure to air pollution. This finding provides an indication as to the primary underlying pathophysiology, and can therefore help to improve the resilience of societies by implementing adequate measures to reduce the air pollutant impact on health outcomes.Trial registrationNot applicable.

Project description:IntroductionEvidence from epidemiological studies based on individual-level data indicates that air pollution may be associated with coronavirus disease 2019 (COVID-19) severity. We aimed to test whether (1) long-term exposure to air pollution is associated with COVID-19-related hospital admission or mortality in the general population; (2) short-term exposure to air pollution is associated with COVID-19-related hospital admission following COVID-19 diagnosis; (3) there are vulnerable population subgroups; and (4) the influence of long-term air pollution exposure on COVID-19-related hospital admissions differed from that for other respiratory infections.MethodsWe constructed a cohort covering nearly the full population of Catalonia through registry linkage, with follow- up from January 1, 2015, to December 31, 2020. Exposures at residential addresses were estimated using newly developed spatiotemporal models of nitrogen dioxide (NO23), particulate matter ≤2.5 μm in aerodynamic diameter (PM2.5), particulate matter ≤10 μm in aerodynamic diameter (PM10), and maximum 8-hr-average ozone (O3) at a spatial resolution of 250 m for the period 2018-2020.ResultsThe general population cohort included 4,660,502 individuals; in 2020 there were 340,608 COVID-19 diagnoses, 47,174 COVID-19-related hospital admissions, and 10,001 COVID-19 deaths. Mean (standard deviation) annual exposures were 26.2 (10.3) μg/m3 for NO2, 13.8 (2.2) μg/m3 for PM2.5, and 91.6 (8.2) μg/m3 for O3. In Aim 1, an increase of 16.1 μg/m3 NO2 was associated with a 25% (95% confidence interval [CI]: 22%-29%) increase in hospitalizations and an 18% (10%-27%) increase in deaths. In Aim 2, cumulative air pollution exposure over the previous 7 days was positively associated with COVID-19-related hospital admission in the second pandemic wave (June 20 to December 31, 2020). Associations of exposure were driven by exposure on the day of the hospital admission (lag0). Associations between short-term exposure to air pollution and COVID-19-related hospital admission were similar in all population subgroups. In Aim 3, individuals with lower individual- and area-level socioeconomic status (SES) were identified as particularly vulnerable to the effects of long-term exposure to NO2 and PM2.5 on COVID-19-related hospital admission. In Aim 4, long-term exposure to air pollution was associated with hospital admission for influenza and pneumonia: (6%; 95% CI: 2-11 per 16.4-μg/m3 NO2 and 5%; 1-8 per 2.6-μg/m3 PM2.5) as well as for all lower respiratory infections (LRIs) (18%; 14-22 per 16.4-μg/m3 NO2 and 14%; 11-17 per 2.6-μg/m3 PM2.5) before the COVID-19 pandemic. Associations for COVID-19-related hospital admission were larger than those for influenza or pneumonia for NO2, PM2.5, and O3 when adjusted for NO2.ConclusionsLinkage across several registries allowed the construction of a large population-based cohort, tracking COVID-19 cases from primary care and testing data to hospital admissions, and death. Long- and short-term exposure to ambient air pollution were positively associated with severe COVID-19 events. The effects of long-term air pollution exposure on COVID-19 severity were greater among those with lower individual- and area-level SES.

Project description:BackgroundIn some patients, coronavirus disease 2019 (COVID-19) is accompanied by loss of smell and taste, and this has been reportedly associated with exposure to air pollutants. This study investigated the relationship between the occurrence of chemosensory dysfunction in COVID-19 patients and air pollutant concentrations in Korea.MethodsInformation on the clinical symptom of chemosensory dysfunction, the date of diagnosis, residential area, age, and sex of 60,194 confirmed COVID-19 cases reported to the Korea Disease Control and Prevention Agency from January 20 to December 31, 2020 was collected. In addition, the daily average concentration of air pollutants for a week in the patients' residential area was collected from the Ministry of Environment based on the date of diagnosis of COVID-19. A binomial logistic regression model, using age and gender, standardized smoking rate, number of outpatient visits, 24-hour mean temperature and relative humidity at the regional level as covariates, was used to determine the effect of air pollution on chemosensory dysfunction.ResultsSymptoms of chemosensory dysfunction were most frequent among patients in their 20s and 30s, and occurred more frequently in large cities. The logistic analysis showed that the concentration of particulate matter 10 (PM10) and 2.5 (PM2.5) up to 2 days before the diagnosis of COVID-19 and the concentration of sulfur dioxide (SO2), nitrogen dioxide (NO2), carbon monoxide (CO), and ozone (O3) at least 7 days before the diagnosis of COVID-19 affected the development of chemosensory dysfunction. In the logistic regression model adjusted for age, sex, standardized smoking rate, number of outpatient visits, and daily average temperature and relative humidity, it was found that an increase in the interquartile range of PM10, PM2.5, SO2, NO2, and CO on the day of diagnosis increased the incidence of chemosensory dysfunction 1.10, 1.10, 1.17, 1.31, and 1.19-fold, respectively. In contrast, the O3 concentration had a negative association with chemosensory dysfunction.ConclusionHigh concentrations of air pollutants such as PM10, PM2.5, SO2, NO2, and CO on the day of diagnosis increased the risk of developing chemosensory dysfunction from COVID-19 infection. This result underscores the need to actively prevent exposure to air pollution and prevent COVID-19 infection. In addition, policies that regulate activities and products that create high amounts of harmful environmental wastes may help in promoting better health for all during COVID-19 pandemic.

Project description:BackgroundExposure to ambient air pollution is related to 4.2 million premature deaths per year worldwide and is associated with a variety of adverse health outcomes, such as respiratory and cardiovascular morbidity. Furthermore, exposure to air pollution can increase human sensitivity to respiratory pathogens via damage to the respiratory tract or via airborne transmission on the surface of particulate matter, and might be an additional factor influencing COVID-19 morbidity and mortality rates. The aim of this study was to examine the association between populations' exposure to air pollution and the morbidity and mortality rates from COVID-19.MethodsWe examined the association between population-weighted long-term exposure to PM2.5 and NOx, and the morbidity and mortality over time following the detection of the first COVID-19 positive case in 36 OECD countries. Pearson and Spearman correlations between daily COVID-19 morbidity and mortality (Jan-Jun 2020) on the 10th, 20th, 40th, 60th and 80th days since first confirmed case in the country, and demographic, health, economic, and environmental data were calculated. Multivariate linear regression were used to examine the associations between demographic, health, economic and air pollution features and the rate of confirmed cases and deaths on the 60th and 80th days following the first confirmed case.ResultsPM2.5 concentrations in 2015-2017 were positively correlated with COVID-19 morbidity and mortality on the 10th, 20th, 40th and 60th days since the first confirmed case in all countries. NOx concentrations in 2015-2017 and country's density (population/Km2) were positively correlated with COVID-19 morbidity and mortality on the 60th day. All multivariate linear regressions consisting PM2.5 concentrations models were statistically significant. Our models also emphasize the importance of the relative number of hospital beds in decreasing the morbidity and mortality of COVID-19.ConclusionsThe adverse health outcomes stemming from long-term exposure to various air pollutants has long been known to the scientific community. According to our results and previously published studies, it appears that long-term exposure to air pollutants concentrations exceeding WHO guidelines, such as PM2.5 and NOx, might exacerbate morbidity and mortality rates from COVID-19. These results should raise a red flag globally among decision makers about the urgent need to reduce air pollution and its harmful effects.
