Dietary behavior and its influencing factors among experienced shiftwork nurses: a secondary analysis.

ABSTRACT:

Purpose

This study investigated the dietary behavior of experienced shiftwork nurses and aimed to identify factors related to dietary behavior.

Methods

This study was a secondary analysis based on the Shift Work Nurses' Health and Turnover study (2018-2021) among Korean nurses. In total, 247 experienced (>12 months) shiftwork nurses were included in this study. The participants' dietary behavior, depression, level of occupational stress, fatigue, physical activity, and general characteristics were measured. Descriptive statistics, Pearson correlation coefficients, independent t-test, one-way analysis of variance, the Kruskal-Wallis test, and multiple regression analysis were conducted.

Results

The dietary behavior score of the participants using the Mini-Dietary Assessment Index was 29.35±5.67. Thirty percent of the participants were depressed, the participants experienced moderate occupational stress, and 74.1% of the participants engaged in an inadequate amount of physical activity. The factors influencing shiftwork nurses' dietary behavior were having child(ren) (β=.16, p=.027), depression (β=-.13, p=.032), level of occupational stress related to occupational climate (β=-.13, p=.035), and an inadequate amount of physical activity (β=-.17, p=.006). These factors explained 10.4% of the variance in experienced shiftwork nurses' dietary behavior scores.

Conclusion

Experienced nurses with child(ren) tended to have healthier diets. However, a higher level of occupational stress related to occupational climate, depression, and engaging in an inadequate amount of physical activity were associated with a higher risk of having an unhealthy diet. Therefore, strategies are needed to encourage physical activity and alleviate adverse occupational climate and depression among experienced nurses.

Project description:AimsTo compare nurses' non-optimal eating behaviours across different shifts, to examine whether non-day shifts were related to deviation from optimal dietary behaviours compared with day shifts and whether such deviation was related to non-optimal macronutrient intake.DesignThis is a 4-day intensive longitudinal study.MethodsA convenience sample of hospital nurses was recruited in Taiwan. From September 2018 through January 2019, 120 participants completed 4-days of 24-h dietary recalls. One-way ANOVA and Kruskal-Wallis H test were used to compare differences in energy and macronutrient intake and frequency of meals and snacking, respectively. Generalized linear regressions examined (1) the associations between shiftwork schedules and non-optimal eating behaviours and (2) associations between non-optimal eating and high energy contribution of non-optimal macronutrients.ResultsNurses consumed less energy on evening and night shifts compared with day shifts. However, energy intake from snacking was higher on evening and night shifts relative to day shifts. Nurses consumed less meals but had higher snacking frequency on non-day shifts. In addition, high energy intake from snacking was positively associated with high energy intake from saturated fat.ConclusionsNurses were more likely to have non-optimal eating behaviours on non-day shifts, which may contribute to an increased intake of saturated fat; thus, increasing their risk of chronic diseases. Strategies to improve non-day shift nurses' non-optimal eating behaviours may be beneficial to their health.ImpactShiftwork is known to affect nurses' eating behaviours; however, which shift is associated with unhealthy eating remains inconclusive. Despite lower energy intakes, nurses had higher intake by snacking on evening and night shifts. High snacking intake was associated with a high intake of saturated fat. Hospitals can increase the availability of healthy foods on evening or night shifts, which may improve non-day shift nurses' non-optimal eating behaviours.

Project description:ObjectiveThis study aimed to examine the latent profile of nurses' mental workload (MWL) and explore the influencing factors via a person-centred approach.MethodsFrom March to July 2023, a quantitative cross-sectional study was carried out to investigate 526 Chinese clinical nurses from five tertiary hospitals in Sichuan Province, China, by using demographic information, the Perceived Social Support Scale, Simplified Coping Skill Questionnaire, and NASA-Task Load Index. Latent profile analyses were performed using Mplus 7.3 software. Pearson's chi-squared and logistic regression analysis was done using SPSS 24.0 software.ResultsThree profiles of mental workload were identified based on the nurses' responses to the mental workload assessment, designated as "low MWL-high self-rated (n = 70, 13.3%)", "moderate MWL (n = 273, 51.9%)", and "high MWL-low self-rated (n = 183, 34.8%)". Based on the analysis of the three subtypes, nurses with working years < 5 years (χ 2 = 12.135, P < 0.05), no children (χ 2 = 16.182, P < 0.01), monthly income < 6000 (χ 2 = 55.231, P < 0.001), poor health status (χ 2 = 39.658, P < 0.001), no psychological training in the past year (χ2 = 56.329, P < 0.001) and suffering from workplace violence (χ 2 = 19.803, P < 0.001) were significantly associated with MWL. Moreover, the multivariate logistic regression analysis showed that negative coping styles (OR = 1.146, 95% CI: 1.060-1.238, P = 0.001) were accompanied by higher MWL while negatively associated with perceived social support (OR = 0.927, 95% CI: 0.900-0.955, P < 0.001).ConclusionOur results showed that the MWL of nurses could be classified into three subtypes. Monthly income, health status, psychological training, workplace violence, negative coping style, and perceived social support were the factors influencing MWL. Managers can employ personalised intervention strategies according to the individual characteristics of different subgroups to reduce nurses' MWL.

Project description:Background Effective teamwork among nurses could help reduce patient mortality and improve patient satisfaction. Previous studies have revealed factors influencing nursing teamwork, including internal factors, types of hospitals and departments, demographic factors of nurses, and scheduling. However, the factors influencing teamwork among oncology nurses have not yet been analyzed in domestic studies. This study investigated the status quo and influencing factors of teamwork among oncology nurses in order to inform strategies for improving clinical treatment effect and survival time of cancer patients. Methods Nurses from the oncology department were recruited through convenience sampling. The survey tools included a general information questionnaire, professional identity scale, missed nursing care scale, and nursing teamwork scale. SPSS 25.0 and Amos 24.0 were used to verify the reliability and validity of each scale and to modify them. A structural equation model was constructed to analyze the model fit and each path coefficient. The structural equation model was used to analyze the factors influencing nursing teamwork in the oncology department, and a multigroup structural equation model was used to analyze whether the degree of nurses’ participation in enhanced recovery after surgery (ERAS) was a moderating variable of nursing teamwork. Results A total of 583 valid questionnaires were collected from participants, and the total score for nursing teamwork was 126.86±15.62. The comprehensive influence path coefficients of professional identity and missed nursing care on nursing teamwork were as follows: team leadership (0.454) > trust and support (0.407) > team mental model (0.348). The coefficients of structural path H4 (professional identity → trust and support), H5 (professional identity → team leadership), and H7 (missed nursing care → team mental model) in the 2 group structural equation models based on the degree of nurses’ participation in ERAS showed significant differences [Δχ2 =7.033, Degrees of freedom (DF)=4, P=0.000]. Conclusions The professional identity of oncology nurses had a direct positive impact on team leadership, trust and support, team mental model, and missed nursing care. The degree of nursing staff’s participation in ERAS had a moderating effect on nursing teamwork.
