Glycaemic Imbalances in Seizures and Epilepsy of Paediatric Age: A Literature Review.

ABSTRACT: Cerebral excitability and systemic metabolic balance are closely interconnected. Energy supply to neurons depends critically on glucose, whose fluctuations can promote immediate hyperexcitability resulting in acute symptomatic seizures. On the other hand, chronic disorders of sugar metabolism (e.g., diabetes mellitus) are often associated with long-term epilepsy. In this paper, we aim to review the existing knowledge on the association between acute and chronic glycaemic imbalances (hyper- and hypoglycaemia) with seizures and epilepsy, especially in the developing brain, focusing on clinical and instrumental features in order to optimize the care of children and adolescents and prevent the development of chronic neurological conditions in young patients.

Project description:This systematic literature review (SLR) assessed the humanistic and economic burden of focal epilepsy and primary generalized tonic-clonic seizures (PGTCS) in adults to evaluate these domains in both populations and identify evidence gaps to inform future research. A search was conducted on December 7, 2022, using MEDLINE and Embase to identify studies published from 2012 onwards reporting humanistic burden (patient-reported or caregiver-reported outcomes or utilities, qualitative evaluations), economic burden (productivity loss, caregiver and societal costs of epilepsy), and sleep-related outcomes. Of the 2830 citations identified, 136 were included. Most studies were in the focal epilepsy population; very few studies reported outcomes in the PGTCS population. The presence of epilepsy-specific instruments varied based on the domain evaluated. Epilepsy exerted considerable humanistic and economic burden. Indicators of poor disease control (e.g., high seizure frequency, resistance to anti-seizure medications, polypharmacy) increased epilepsy burden. Seizure frequency and type, disease severity, and polypharmacy also affected work productivity. Adults with epilepsy, particularly focal epilepsy, reported higher indirect costs, more sick days accrued, and early entry into retirement. Caregivers similarly reported high productivity loss and absenteeism related to caregiving duties. The results of this SLR highlight the high humanistic and economic burden of focal epilepsy and PGTCS, although limited data were available for the PGTCS population. The results include patient-reported outcome data specific to focal epilepsy and PGTCS, expanding the limited humanistic burden evidence identified in previous reviews, and show the effect of poor disease control on individuals' lives and as a driver of indirect costs. PLAIN LANGUAGE SUMMARY: Our systematic literature review identified studies that evaluated the impact of focal epilepsy and primary generalized tonic-clonic seizures on patients and their caregivers. We found that focal epilepsy negatively impacted patients' mental health and sleep and was associated with higher indirect costs and lower work productivity in people with more severe disease. The impact of primary generalized tonic-clonic seizures on patients was rarely reported, and future research is needed.

Project description:BackgroundAcute postoperative seizures (APOS) are common phenomena following resective epilepsy surgery and can be categorized as running-down (RDS) or running-up seizures (RUS). This differentiation is made retrospectively, considering their classification is based on seizure recurrence. However, early differentiation of RDS from RUS may prevent unnecessary escalation of anti-seizure medication or reoperation. This review provides an overview of the available literature on variables influencing the evolution to RDS/RUS in patients exhibiting acute or early postoperative seizures.MethodsA database search was performed addressing studies related to the running-down phenomenon and postoperative seizures in PubMed and Embase. Eligibility required a clear definition of acute or early postoperative seizures. Studies concerning any type of epilepsy surgery or pathology were accepted, excluding those related to high-grade malignancies.ResultsThe search yielded a total of n = 1,690 records. We included n = 21 studies with a total of n = 1,496 patients, which examined variables associated with long-term seizure outcome. Interictal epileptiform discharge presence/laterality, epileptogenic zone size, APOS frequency, and history of generalized tonic-clonic seizures, head trauma, or encephalitis were associated with seizure outcome. Ictal expression and timing of seizure recurrence appeared less relevant. However, these associations are uncertain due to conflicting results between studies, likely due to small sample sizes, a limited reporting of secondary variables, and heterogeneity in study population and methodology.ConclusionsThe variability in clinical outcome following APOS highlights the need for a refined classification of postoperative seizures. Future research should focus on constructing and validating a multifactorial model integrating EEG-derived variables, APOS frequency, and medical history to more accurately predict long-term seizure outcome following resective epilepsy surgery.

Project description:BackgroundSurgical site infections (SSIs) in children represent a common and serious postoperative complication. Surgical skin preparation is an essential preventive measure in every surgical procedure. The most commonly used antiseptic agents for surgical skin preparation are chlorhexidine gluconate and iodophors in alcohol-based solutions. In adult patients the use of chlorhexidine-containing antiseptic solutions for preoperative skin preparation has been advocated to reduce SSI rates. Our objective was to conduct a systematic literature review on use of antiseptic agents for surgical skin preparation in children less than 16 years of age.MethodsA systematic review of MEDLINE, EMBASE, CINAHL and CENTRAL was performed using both MeSH and free text terms and using the relevant Cochrane filter to identify full text randomized trials (RCTs) and comparative observational studies. Interventions of interest were the choice of main agent in antiseptic solutions (chlorhexidine/povidone-iodine/alcohol) compared with each other or with other antiseptic agents. Primary outcome was the reported rate of surgical site infections.ResultsIn total 8 studies were included in the review; 2 RCTs and 6 observational studies. Observational studies generally did not primarily investigate the association of different antiseptics with subsequent SSI. The identified randomised controlled trials included only 61 children in total, and were of low quality. Consequently, we did not conduct a formal meta-analysis. Since the publication of a comprehensive systematic review of perioperative measures for the prevention of SSI in 2016, no randomized controlled trials comparing antiseptic agents for surgical skin preparation in paediatric surgery have been conducted.ConclusionRobust evidence on the optimal skin antisepsis to reduce SSIs in children is lacking. Direct extrapolation of effects from trials involving adults is not appropriate as physiologic characteristics and risk factors for SSIs differ between adults and children. It is therefore essential to conduct high quality RCT investigating interventions to identify optimal measures to reduce SSI rates in children.Trial registrationProspero registration ( CRD42020166193 ).
