Adolescent pregnancy in Mongolia: Evidence from Mongolia Social Indicator Sample Survey 2013-2018.

ABSTRACT:

Introduction

Adolescent pregnancy is a major public health and social concern which pose enormous pregnancy and delivery-related risks for both the mother and their neonate. This study aims to estimate adolescent pregnancy and determine the factors associated with adolescent pregnancy in Mongolia.

Methods

This study pooled data from 2013 and 2018 Mongolia, Social Indicator Sample Surveys (MSISS). A total of 2808 adolescent girls aged 15-19 years with socio-demographic information were included in this study. Adolescent pregnancy is defined as pregnancy in a female 19 years of age or younger. Multivariable logistic regression analysis was performed to determine factors associated with adolescent pregnancy in Mongolia.

Results

Adolescent pregnancy was estimated at 57.62 [95% Confidence Interval (CI): 44.41, 70.84] per 1000 adolescent girls aged 15-19 years. Multivariable analyses reported higher adolescent pregnancy in the countryside [Adjusted Odds Ratios (AOR) = 2.07 (95%CI: 1.08, 3.96)], with increasing age [AOR = 11.50 (95%CI: 6.64, 19.92)], among adolescent girls who used contraception methods [AOR = 10.80 (95%CI: 6.34, 18.40)], among adolescent girls from the poorest households [AOR = 3.32 (95%CI: 1.39, 7.93)], and among adolescent girls who drank alcohol [AOR = 2.10 (95%CI: 1.22, 3.62)].

Discussion

Identifying the factors associated with adolescent pregnancy is crucial in reducing adolescent pregnancy and improving the sexual and reproductive health as well as the social and economic well-being of adolescents thus setting Mongolia on the path to achieving the Sustainable Development Goals (SDGs) 3 by 2030.

Project description:Understanding the dynamics of social risk factors in the occurrence of adolescent motherhood is vital in designing more appropriate prevention initiatives in low-income and middle-income countries (LMICs). We aimed this study to examine the transition of social risk factors and their association with adolescent motherhood in LMICs since the initiation of the MDGs. We analysed 119967 adolescent girls (15-19 years) from 40-nationally representative Demographic Health Surveys in 20 LMICs that had at least two surveys: a survey in 1996-2003(baseline, near MDGs started) and another in 2014-2018(endline). Adolescent motherhood (having a live birth or being pregnant before age 20) was the outcome of interest, whereas social risk factors including household wealth, girls' level of education, and area of residence were the exposures. The association between adolescent motherhood and the social risk factors, as well as changes in the strength of the association over time were observed using multilevel logistic regression analysis. On an average, the proportion of adolescent mothers without education decreased by -15·61% (95% CI: -16·84, -14·38), whereas the poorest adolescent mother increased by 5·87% (95% CI: 4·74, 7·00). The national prevalence of adolescent motherhood remained unchanged or increased in 55·00% (11/20) of the studied countries. Comparing baseline to endline, the overall adjusted odds ratio (AOR) of adolescent motherhood increased for both poorest (AOR = 1·42, 95% CI: 1·28, 1·59) and rural residences (AOR = 1·09, 95% CI: 1·01, 1·17), and decreased, but not statistically significant for the low level of education (AOR = 0·92, 95% CI: 0·84, 1·01 for no education). Our study concludes that social risk factors of the adolescent mother had shifted in different directions during MDGs and SDGs eras, and adolescent mothers remained more disadvantaged than non-mothers in LMICs. Efforts need to be enhanced to improve adolescent girls' education. Intervention should be prioritised in disadvantaged communities to delay adolescent first birth and prevent adolescent motherhood in LMICs.

Project description:BackgroundThe literature posits that HIV knowledge is associated with precautious sexual behaviour and practice. We hypothesised and investigated the association between knowledge of HIV transmission and adolescent pregnancy in Ghana, given that the relationship between HIV knowledge and adolescent pregnancy has not been extensively studied.MethodsWe did analyses on 5836 cases (weighted as 5121) of 15-24 years old reproductive age women in the female dataset of the 2017 Multiple Cluster Indicator Survey. Adolescent pregnancy was operationalized as reproductive age women between 15-24 years who became pregnant before the age of 18 years. Accurate HIV knowledge was measured by computing the scores of correct responses on six questions exploring women's knowledge about HIV transmission. We accounted for sample design and weight before performing a Chi-square test of independence and Poisson regression.ResultsThe results indicate that having lower scores on the HIV transmission knowledge scale was correlated with a higher probability of girls becoming pregnant before their 18th birthday. After controlling for the moderating effect of socio-demographic characteristics of the participants, we found that accurate HIV transmission knowledge loses its statistical significance in determining adolescent pregnancy. The factors that remain significant in the adjusted model were formal education status, household wealth, and region of residence. HIV transmission knowledge was statistically significantly related to adolescent pregnancy in the model after the education level variable was omitted. This observation was due to the significant effect of school education on other variables in the model. This result demonstrates that HIV knowledge has a major impact on adolescent pregnancy, but this effect is predicated by formal education attainment of the adolescent girl.ConclusionsGiven the results, adolescent pregnancy issues can be resolved by the government and other development partners by adequately educating adolescents about HIV transmissions. Also, because they have the potential to reduce pregnancy among adolescents in Ghana, we recommend that programs and initiatives should address existing disparities in formal educational attainment and household wealth.
