Ensemble machine learning algorithm for predicting acute kidney injury in patients admitted to the neurointensive care unit following brain surgery.

ABSTRACT: Acute kidney injury (AKI) is a common postoperative complication among patients in the neurological intensive care unit (NICU), often resulting in poor prognosis and high mortality. In this retrospective cohort study, we established a model for predicting AKI following brain surgery based on an ensemble machine learning algorithm using data from 582 postoperative patients admitted to the NICU at the Dongyang People's Hospital from March 1, 2017, to January 31, 2020. Demographic, clinical, and intraoperative data were collected. Four machine learning algorithms (C5.0, support vector machine, Bayes, and XGBoost) were used to develop the ensemble algorithm. The AKI incidence in critically ill patients after brain surgery was 20.8%. Intraoperative blood pressure; postoperative oxygenation index; oxygen saturation; and creatinine, albumin, urea, and calcium levels were associated with the postoperative AKI occurrence. The area under the curve value for the ensembled model was 0.85. The accuracy, precision, specificity, recall, and balanced accuracy values were 0.81, 0.86, 0.44, 0.91, and 0.68, respectively, indicating good predictive ability. Ultimately, the models using perioperative variables exhibited good discriminatory ability for early prediction of postoperative AKI risk in patients admitted to the NICU. Thus, the ensemble machine learning algorithm may be a valuable tool for forecasting AKI.

Project description:In recent years, the number of vulnerabilities discovered and publicly disclosed has shown a sharp upward trend. However, the value of exploitation of vulnerabilities varies for attackers, considering that only a small fraction of vulnerabilities are exploited. Therefore, the realization of quick exclusion of the non-exploitable vulnerabilities and optimal patch prioritization on limited resources has become imperative for organizations. Recent works using machine learning techniques predict exploited vulnerabilities by extracting features from open-source intelligence (OSINT). However, in the face of explosive growth of vulnerability information, there is room for improvement in the application of past methods to multiple threat intelligence. A more general method is needed to deal with various threat intelligence sources. Moreover, in previous methods, traditional text processing methods were used to deal with vulnerability related descriptions, which only grasped the static statistical characteristics but ignored the context and the meaning of the words of the text. To address these challenges, we propose an exploit prediction model, which is based on a combination of fastText and LightGBM algorithm and called fastEmbed. We replicate key portions of the state-of-the-art work of exploit prediction and use them as benchmark models. Our model outperforms the baseline model whether in terms of the generalization ability or the prediction ability without temporal intermixing with an average overall improvement of 6.283% by learning the embedding of vulnerability-related text on extremely imbalanced data sets. Besides, in terms of predicting the exploits in the wild, our model also outperforms the baseline model with an F1 measure of 0.586 on the minority class (33.577% improvement over the work using features from darkweb/deepweb). The results demonstrate that the model can improve the ability to describe the exploitability of vulnerabilities and predict exploits in the wild effectively.

Project description:BackgroundAcute kidney injury (AKI) represents a significant global health challenge, leading to increased patient distress and financial health care burdens. The development of AKI in intensive care unit (ICU) settings is linked to prolonged ICU stays, a heightened risk of long-term renal dysfunction, and elevated short- and long-term mortality rates. The current diagnostic approach for AKI is based on late indicators, such as elevated serum creatinine and decreased urine output, which can only detect AKI after renal injury has transpired. There are no treatments to reverse or restore renal function once AKI has developed, other than supportive care. Early prediction of AKI enables proactive management and may improve patient outcomes.ObjectiveThe primary aim was to develop a machine learning algorithm, NAVOY Acute Kidney Injury, capable of predicting the onset of AKI in ICU patients using data routinely collected in ICU electronic health records. The ultimate goal was to create a clinical decision support tool that empowers ICU clinicians to proactively manage AKI and, consequently, enhance patient outcomes.MethodsWe developed the NAVOY Acute Kidney Injury algorithm using a hybrid ensemble model, which combines the strengths of both a Random Forest (Leo Breiman and Adele Cutler) and an XGBoost model (Tianqi Chen). To ensure the accuracy of predictions, the algorithm used 22 clinical variables for hourly predictions of AKI as defined by the Kidney Disease: Improving Global Outcomes guidelines. Data for algorithm development were sourced from the Massachusetts Institute of Technology Lab for Computational Physiology Medical Information Mart for Intensive Care IV clinical database, focusing on ICU patients aged 18 years or older.ResultsThe developed algorithm, NAVOY Acute Kidney Injury, uses 4 hours of input and can, with high accuracy, predict patients with a high risk of developing AKI 12 hours before onset. The prediction performance compares well with previously published prediction algorithms designed to predict AKI onset in accordance with Kidney Disease: Improving Global Outcomes diagnosis criteria, with an impressive area under the receiver operating characteristics curve (AUROC) of 0.91 and an area under the precision-recall curve (AUPRC) of 0.75. The algorithm's predictive performance was externally validated on an independent hold-out test data set, confirming its ability to predict AKI with exceptional accuracy.ConclusionsNAVOY Acute Kidney Injury is an important development in the field of critical care medicine. It offers the ability to predict the onset of AKI with high accuracy using only 4 hours of data routinely collected in ICU electronic health records. This early detection capability has the potential to strengthen patient monitoring and management, ultimately leading to improved patient outcomes. Furthermore, NAVOY Acute Kidney Injury has been granted Conformite Europeenne (CE)-marking, marking a significant milestone as the first CE-marked AKI prediction algorithm for commercial use in European ICUs.

Project description:ImportanceLumber disc herniation surgery can reduce pain and disability. However, a sizable minority of individuals experience minimal benefit, necessitating the development of accurate prediction models.ObjectiveTo develop and validate prediction models for disability and pain 12 months after lumbar disc herniation surgery.Design, setting, and participantsA prospective, multicenter, registry-based prognostic study was conducted on a cohort of individuals undergoing lumbar disc herniation surgery from January 1, 2007, to May 31, 2021. Patients in the Norwegian Registry for Spine Surgery from all public and private hospitals in Norway performing spine surgery were included. Data analysis was performed from January to June 2023.ExposuresMicrodiscectomy or open discectomy.Main outcomes and measuresTreatment success at 12 months, defined as improvement in Oswestry Disability Index (ODI) of 22 points or more; Numeric Rating Scale (NRS) back pain improvement of 2 or more points, and NRS leg pain improvement of 4 or more points. Machine learning models were trained for model development and internal-external cross-validation applied over geographic regions to validate the models. Model performance was assessed through discrimination (C statistic) and calibration (slope and intercept).ResultsAnalysis included 22 707 surgical cases (21 161 patients) (ODI model) (mean [SD] age, 47.0 [14.0] years; 12 952 [57.0%] males). Treatment nonsuccess was experienced by 33% (ODI), 27% (NRS back pain), and 31% (NRS leg pain) of the patients. In internal-external cross-validation, the selected machine learning models showed consistent discrimination and calibration across all 5 regions. The C statistic ranged from 0.81 to 0.84 (pooled random-effects meta-analysis estimate, 0.82; 95% CI, 0.81-0.84) for the ODI model. Calibration slopes (point estimates, 0.94-1.03; pooled estimate, 0.99; 95% CI, 0.93-1.06) and calibration intercepts (point estimates, -0.05 to 0.11; pooled estimate, 0.01; 95% CI, -0.07 to 0.10) were also consistent across regions. For NRS back pain, the C statistic ranged from 0.75 to 0.80 (pooled estimate, 0.77; 95% CI, 0.75-0.79); for NRS leg pain, the C statistic ranged from 0.74 to 0.77 (pooled estimate, 0.75; 95% CI, 0.74-0.76). Only minor heterogeneity was found in calibration slopes and intercepts.ConclusionThe findings of this study suggest that the models developed can inform patients and clinicians about individual prognosis and aid in surgical decision-making.

Project description:Hospital-acquired infections are common in neurointensive care units. We sought to review interventions which may reduce infection rates in neurocritically ill populations. We conducted a systematic review of studies targeting adult patients in neuro-intensive care units (neuro-ICUs) with an intervention designed to prevent ICU-acquired infections. Our outcome of interest was change in the prevalence or rates of infection between active and control arms of these studies. We excluded studies based on the following criteria: no English full-text version available; pediatric population; non-neurosciences ICU population; pre- or intraoperative methods to prevent infection; lack of discrete data for infection rates/prevalence; studies that were purely observational in nature and did not test an intervention; and studies performed in resource limited settings. We initially retrieved 3716 results by searching the following databases: PubMed/MEDLINE, EMBASE via Ovid, and Cochrane CENTRAL via Ovid. No date or language limits were used in the search. Computerized deduplication was conducted using EndNote followed by a confirmatory manual review resulting in 3414 citations. An additional 19 manuscripts were identified through review of references. The screening process followed a standard protocol, using two screeners at the title/abstract level to determine relevance and at the full-text level to determine eligibility for inclusion. The 3427 titles/abstracts were independently screened by two board-certified neurointensivists to determine relevance for full-text review, and 3248 were rejected. The remaining 179 abstracts were reviewed in full text using predetermined inclusion/exclusion criteria. Ultimately, 75 articles met our inclusion criteria and were utilized in the final analysis. The reviewed literature highlights the need for collaborative, multi-disciplinary, and multi-pronged approaches to reduce infections. Rates of VRI, SSI, VAP, CAUTI, and CLABSI can approach zero with persistence and a team-based approach.
