Early response evaluation of PD-1 blockade in NSCLC patients through FDG-PET-CT and T cell profiling of tumor-draining lymph nodes.

ABSTRACT: Better biomarkers for programmed death - (ligand) 1 (PD-(L)1) checkpoint blockade in non-small cell lung cancer (NSCLC) are needed. We explored the predictive value of early response evaluation using Fluor-18-deoxyglucose positron emission tomography and pre- and on-treatment flowcytometric T-cell profiling in peripheral blood and tumor-draining lymph nodes (TDLN). The on-treatment evaluation was performed 7-14 days after the start of PD-1 blockade in NSCLC patients. These data were related to (pathological) tumor response, progression-free survival, and overall survival (OS). We found that increases in total lesion glycolysis (TLG) had a strong reverse correlation with OS (r = -0.93, p = 0.022). Additionally, responders showed decreased progressors and increased Treg frequencies on-treatment. Frequencies of detectable PD-1-expressing CD8⁺ T cells decreased in responders but remained stable in progressors. This was especially found in the TDLN. Changes in activated Treg rates in TDLN were strongly but, due to low numbers of data points, non-significantly correlated with ΔTLG and reversely correlated with OS.

Project description:BackgroundWe aimed to investigate the diagnostic value of dynamic 2-deoxy-2-[18F]-fluoro-D-glucose positron emission tomography-computed tomography in non-small cell lung cancer and fluoro-D-glucose hypermetabolic lymph nodes.MethodsPatients who made an active appointment for positron emission tomography-computed tomography were randomly enrolled by referring to previous imaging data and clinical information. Finally, 34 histopathologically confirmed non-small cell lung cancers (18 adenocarcinoma and 16 squamous cell carcinoma cases) were prospectively studied using dynamic and static 2-deoxy-2-[18F]-fluoro-D-glucose positron emission tomography-computed tomography imaging (the diagnostic study has not yet been registered on a clinical trial platform). In dynamic positron emission tomography images, a volume of interest, defined by the thoracic aorta, was selected for estimating the arterial input function. Patlak and irreversible two-tissue compartment model analyses were performed based on the pixel points to obtain first-order characteristic kinetic parameters for each lesion and hypermetabolic lymph node. The first-order characteristic kinetic parameters were obtained based on the basic data of dynamic positron emission tomography images in the corresponding model and the lesion delineation of region-of-interest based on computed tomography images, such as V_Median (the median gray intensity of V), k3_Entropy, VB_Entropy, K1_Uniformity, and ki_Uniformity. The first-order characteristic kinetic parameters were also modeled by logistic regression for the differential diagnosis of non-small cell lung cancer and hypermetabolic lymph nodes. Maximum and mean standard uptake values (SUVmax and SUVmean, respectively) were obtained from static positron emission tomography images. The diagnostic efficacy of the parameters was evaluated using the receiver operating characteristic curve and the DeLong test.ResultsThere was a significant difference in the V_Median values of adenocarcinoma and squamous cell carcinoma. The regression models for K1, k3, and V provided good predictions of adenocarcinoma and squamous cell carcinoma typology. Significant differences were observed in k3_Entropy, VB_Entropy, K1_Uniformity, and ki_Uniformity between benign and malignant lymph nodes. The regression model of Ki, VB, and k3 could make a good prediction for identifying benign and malignant lymph nodes.ConclusionsDynamic 2-deoxy-2-[18F]-fluoro-D-glucose positron emission tomography-computed tomography imaging showed high diagnostic value in the staging of non-small cell lung cancer and fluoro-D-glucose hypermetabolic lymph nodes, and can be of great use in non-small cell lung cancer lymph node staging and surgical decision-making.

Project description:Purpose The aim of the present study was to test whether the coefficient of variation (CoV) of 18F-FDG PET/CT images of metastatic lymph nodes and primary tumors may predict clinical outcome in patients with advanced non-small cell lung cancer (NSCLC). Materials and Methods Fifty-eight NSCLC patients who had undergone 18F-FDG PET/CT at diagnosis were evaluated. SUVmax, SUVmean, CoV, MTV and TLG were determined in targeted lymph nodes and corresponding primary tumors along with Total MTV (MTVTOT) and Whole-Body TLG (TLGWB) of all malignant lesions. Univariate analysis was performed using Cox proportional hazards regression whereas the Kaplan-Meier method and log-rank tests were used for survival analysis. Results Fifty-eight metastatic lymph nodes were analyzed and average values of SUVmax, SUVmean, CoV, MTV and TLG were 11.89 ± 8.54, 4.85 ± 1.90, 0.37 ± 0.16, 46.16 ± 99.59 mL and 256.84 ± 548.27 g, respectively, whereas in primary tumors they were 11.92 ± 6.21, 5.47 ± 2.34, 0.36 ± 0.14, 48.03 ± 64.45 mL and 285.21 ± 397.95 g, respectively. At univariate analysis, overall survival (OS) was predicted by SUVmax (p = 0.0363), SUVmean (p = 0.0200) and CoV (p = 0.0139) of targeted lymph nodes as well as by CoV of primary tumors (p = 0.0173), MTVTOT (p = 0.0007), TLGWB (p = 0.0129) and stage (p = 0.0122). Using Kaplan-Meier analysis, OS was significantly better in patients with CoV of targeted lymph nodes ≤ 0.29 than those with CoV > 0.29 (p = 0.0147), meanwhile patients with CoV of primary tumors > 0.38 had a better prognosis compared to those with CoV ≤ 0.38 (p = 0.0137). Finally, we combined the CoV values of targeted lymph nodes and primary tumors in all possible arrangements and a statistically significant difference was found among the four survival curves (p = 0.0133). In particular, patients with CoV of targeted lymph nodes ≤ 0.29 and CoV of primary tumors > 0.38 had the best prognosis. Conclusions The CoV of targeted lymph nodes combined with the CoV of primary tumors can predict prognosis of NSCLC patients.

Project description:IntroductionImmunotherapy has improved outcomes for patients with non-small cell lung cancer (NSCLC), yet durable clinical benefit (DCB) is experienced in only a fraction of patients. Here, we test the hypothesis that radiomics features from baseline pretreatment 18F-FDG PET/CT scans can predict clinical outcomes of NSCLC patients treated with checkpoint blockade immunotherapy.MethodsThis study included 194 patients with histologically confirmed stage IIIB-IV NSCLC with pretreatment PET/CT images. Radiomics features were extracted from PET, CT, and PET+CT fusion images based on minimum Kullback-Leibler divergence (KLD) criteria. The radiomics features from 99 retrospective patients were used to train a multiparametric radiomics signature (mpRS) to predict DCB using an improved least absolute shrinkage and selection operator (LASSO) method, which was subsequently validated in both retrospective (N = 47) and prospective test cohorts (N = 48). Using these cohorts, the mpRS was also used to predict progression-free survival (PFS) and overall survival (OS) by training nomogram models using multivariable Cox regression analyses with additional clinical characteristics incorporated.ResultsThe mpRS could predict patients who will receive DCB, with areas under receiver operating characteristic curves (AUCs) of 0.86 (95%CI 0.79-0.94), 0.83 (95%CI 0.71-0.94), and 0.81 (95%CI 0.68-0.92) in the training, retrospective test, and prospective test cohorts, respectively. In the same three cohorts, respectively, nomogram models achieved C-indices of 0.74 (95%CI 0.68-0.80), 0.74 (95%CI 0.66-0.82), and 0.77 (95%CI 0.69-0.84) to predict PFS and C-indices of 0.83 (95%CI 0.77-0.88), 0.83 (95%CI 0.71-0.94), and 0.80 (95%CI 0.69-0.91) to predict OS.ConclusionPET/CT-based signature can be used prior to initiation of immunotherapy to identify NSCLC patients most likely to benefit from immunotherapy. As such, these data may be leveraged to improve more precise and individualized decision support in the treatment of patients with advanced NSCLC.
