Novel organoid construction strategy for non-involuting congenital hemangioma for drug validation.

ABSTRACT:

Background

Non-involuting congenital hemangiomas (NICHs) are fully formed vascular tumors at birth with distinctive clinical, radiologic, and histopathological profiles. In the literature, there is no effective therapy strategy for patients with NICH except surgery. Currently, no cell line or animal model exists for studying the mechanism of NICH and drug validation. We plan to construct a new strategy by constructing NICH organoids for further study.

Result

Here, we report a novel NICH organoid system construction and optimization process. Both HE and immunohistological staining exactly matched NICH tissue. We further performed transcriptome analysis to elucidate the characteristics of NICH organoids. Both NICH tissue and NICH organoids manifested similar trends in download sites. NICH organoids display novel features to new cells derived from organoids and show spectacular multiplication capacity. In the preliminary verification, we found that cells splitting from NICH organoids were human endothelial cells. Drug validation demonstrated that trametinib, sirolimus, and propranolol showed no inhibitory effects on NICH organoids.

Conclusion

Our data show that this new NICH-derived organoid faithfully captured the features of this rare vascular tumor. Our study will boost further research on the mechanism of NICH and drug filtering in the future.

Project description:UnlabelledCongenital cytomegalovirus (CMV) infection is a leading cause of mental retardation and deafness in newborns. The guinea pig is the only small animal model for congenital CMV infection. A novel CMV vaccine was investigated as an intervention strategy against congenital guinea pig cytomegalovirus (GPCMV) infection. In this disabled infectious single-cycle (DISC) vaccine strategy, a GPCMV mutant virus was used that lacked the ability to express an essential capsid gene (the UL85 homolog GP85) except when grown on a complementing cell line. In vaccinated animals, the GP85 mutant virus (GP85 DISC) induced an antibody response to important glycoprotein complexes considered neutralizing target antigens (gB, gH/gL/gO, and gM/gN). The vaccine also generated a T cell response to the pp65 homolog (GP83), determined via a newly established guinea pig gamma interferon enzyme-linked immunosorbent spot assay. In a congenital infection protection study, GP85 DISC-vaccinated animals and a nonvaccinated control group were challenged during pregnancy with wild-type GPCMV (10(5) PFU). The pregnant animals carried the pups to term, and viral loads in target organs of pups were analyzed. Based on live pup births in the vaccinated and control groups (94.1% versus 63.6%), the vaccine was successful in reducing mortality (P = 0.0002). Additionally, pups from the vaccinated group had reduced CMV transmission, with 23.5% infected target organs versus 75.9% in the control group. Overall, these preliminary studies indicate that a DISC CMV vaccine strategy has the ability to induce an immune response similar to that of natural virus infection but has the increased safety of a non-replication-competent virus, which makes this approach attractive as a CMV vaccine strategy.ImportanceCongenital CMV infection is a leading cause of mental retardation and deafness in newborns. An effective vaccine against CMV remains an elusive goal despite over 50 years of CMV research. The guinea pig, with a placenta structure similar to that in humans, is the only small animal model for congenital CMV infection and recapitulates disease symptoms (e.g., deafness) in newborn pups. In this report, a novel vaccine strategy against congenital guinea pig cytomegalovirus (GPCMV) infection was developed, characterized, and tested for efficacy. This disabled infectious single-cycle (DISC) vaccine strategy induced a neutralizing antibody or a T cell response to important target antigens. In a congenital infection protection study, animals were protected against CMV in comparison to the nonvaccinated group (52% reduction of transmission). This novel vaccine was more effective than previously tested gB-based vaccines and most other strategies involving live virus vaccines. Overall, the DISC vaccine is a safe and promising approach against congenital CMV infection.

Project description:BackgroundDelirium often occurs in children with congenital heart disease in the early postoperative period, which is not conducive to the rehabilitation and prognosis. There is little evidence to prove the effectiveness and safety of drug treatment of delirium in children, and the prevention has become an important topic. The purpose of this study is to analyze the early risk factors of delirium in children after congenital heart surgery, establish a nomogram prediction model, and explore the application efficiency of the model, so as to provide reference for early prevention of delirium.MethodsA total of 362 children treated in the cardiac intensive care unit (CICU) of Shanghai Children's Medical Center after congenital heart surgery from February 15 to April 15, 2021 were enrolled for the construction of the model. Bedside nurses who received unified training used the Cornell Assessment of Pediatric Delirium (CAPD) to evaluate delirium and recorded sixteen preoperative- and intraoperative-related influencing factors. A nomogram prediction model was created using multivariate logistic regression. The prediction effect of the model was evaluated by C-index and Brier value, and 96 children from April 16 to May 15, 2021 were included for effect verification. The model's effectiveness was validated by comparing the occurrence of delirium in children predicted by the model with the actual occurrence.ResultsMultivariate logistic regression analysis showed that male gender [odds ratio (OR) =1.786, 95% confidence interval (CI): 1.018-3.134, P=0.043], age <6.5 months (OR =0.224, 95% CI: 0.126-0.399, P=0.000), disease severity ≥4 points (OR =6.955, 95% CI: 3.564-13.576, P=0.003), and operation time ≥148 min (OR =2.401, 95%CI: 1.336-4.315, P=0.000) were independent risk factors for delirium in children after cardiac surgery. The C-index of the nomogram prediction model was 0.808, sensitivity was 76.1%, specificity was 70%, and the Brier value was 0.142. The validation of the model showed that the model predicted 20 cases and the actual occurrence was 20 cases, of which 8 cases were false negative and 8 cases were false positive, and the sensitivity, specificity, and accuracy of the model were 60%, 89.5%, and 83.3%, respectively.ConclusionsThe prediction model constructed in this study could provide early prediction of the occurrence of delirium in children after congenital heart surgery to a certain extent.
