Project description:BackgroundThe COVID-19 pandemic has had a deep global impact, not only from a social and economic perspective, but also with regard to human health and the environment. To restrict transmission of the virus, the Indian government enforced a complete nationwide lockdown except for essential services and supplies in phases from 25 March to 31 May 2020. Ambient air quality in and around New Delhi, one of the most polluted cities of world, was also impacted during this period.ObjectiveThe aim of the present study was to assess and understand the impact of four different lockdown phases (LD1, LD2, LD3 and LD4) on five air pollutants (particulate matter (PM) PM2.5, PM10, nitrogen oxide (NOx), sulfur dioxide (SO2) and ozone (O3)) compared to before lockdown (BLD) at 13 air monitoring stations in and around New Delhi.MethodsSecondary data on five criteria pollutants for 13 monitoring stations in and around New Delhi for the period 1 March to 31 May 2020 was accessed from the Central Pollution Control Bard, New Delhi. Data were statistically analyzed across lockdown phases, meteorological variables, and prevailing air sources around the monitoring stations.ResultsPollutant concentrations decreased during LD1 compared to BLD except for O3 at all stations. PM2.5 and PM10 remained either close to or higher than the National Ambient Air Quality Standards (NAAQS) due to prevailing high-speed winds. During lockdown phases, NO2 decreased, whereas O3 consistently increased at all stations. This was a paradoxical situation as O3 is formed via photochemical reactions among NOx and volatile organic compounds. Principal component analysis (PCA) extracted two principal components (PC1 and PC2) which explained up to 80% of cumulative variance in data. PM2.5, PM10 and NO2 were associated with PC1, whereas PC2 had loadings of either O3 only or O3 and SO2 depending upon monitoring station.ConclusionsThe present study found that air pollutants decreased during lockdown phases, but these decreases were specific to the site(s) and pollutant(s). The decrease in pollutant concentrations during lockdown could not be attributed completely to lockdown conditions as the planetary boundary layer increased two-fold during lockdown compared to the BLD phase. Such restrictions could be applied in the future to control air pollution but should be approached with caution.Competing interestsThe authors declare no competing financial interests.

Project description:BackgroundThe goal of our study was to compare the incidence of motor vehicle accidents (MVA), attempted or completed suicides, and injuries inflicted by others between individuals with and without epilepsy.MethodsIndividuals with and without epilepsy were identified using linked administrative databases between 1996 and 2003 in a Canadian health region with a 1.4 million population. We used a validated epilepsy case definition: anyone who had 2 physician claims, one hospitalization, or one emergency room visit coded with an International Classification of Diseases (ICD)-9-CM or ICD-10 epilepsy code any time during a 2-year period. Four subjects without epilepsy were matched to one patient with epilepsy by age (within 1 year) and sex. The incidence of MVAs, attempted or completed suicides, and inflicted injuries was assessed in 2003-2004. Outcomes were adjusted using the Elixhauser comorbidity index.ResultsA total of 10,240 individuals with epilepsy and 40,960 individuals without epilepsy were identified. Mean age was 39.0 ± 21.3 years (range 0.12-99.4) and 48.5% were female. One-year odds ratios before and after adjustment for comorbidity were 1.83 (95% confidence interval [CI] 1.33-2.54) and 1.38 (95% CI 0.97-1.96) for MVAs, 4.32 (95% CI 2.79-6.69) and 1.32 (95% CI 0.81-2.15) for attempted or completed suicides, and 3.54 (95% CI 2.66-4.72) and 1.46 (95% CI 1.04-2.03) for injuries inflicted by others.ConclusionIn this cohort-controlled population-based study, once important medical and psychiatric comorbidities were adjusted for, people with epilepsy were not more likely to attempt suicide or experience MVAs, but were still more likely to be assaulted compared to those without epilepsy.

Project description:The spread of the COVID-19 pandemic forced the administration to lock down in many countries globally to stop the spread. As the lockdown phase had only the emergency use of transportation and most of the industries were shut down, there was an apparent reduction in pollution. With the end of the lockdown period, pollution is returning to its regular emission in most places. Though the background was abnormally low in emissions (during the lockdown phase) and the reduced pollution changed the radiation balance in the northern hemispheric summer period, a modified pollution pattern is possible during the unlock phases of 2020. The present study analysed the unlock 1 and 2 stages (June-July) of the COVID-19 lockdown over India. The rainfall, surface temperature and cloud cover anomalies of 2020 for understanding the differences in pollutants variation were also analysed. The unlock phases show remarkable differences in trends and mean variations of pollutants over the Indian region compared to climatological variations. The results indicated changing high-emission regions over India to climatological variations and identified an AOD dipole with future emissions over India.

Project description:India is one of the worst affected countries during the COVID-19 pandemic. We carried out comparative analyses of the COVID-19 situation in the Maharashtra state, India for the first and second waves. Epidemiological and demographics data were obtained from open sources and the Government of Maharashtra. Mathematical modeling and analyses were conducted to estimate the epidemiological parameters like basic reproduction number (R0) for the first wave at different times. The districts with a higher percentage of the urban population recorded a higher attack rate during the first wave. However, during the second wave, the rural population was more affected. The effective reproduction number (Re) was estimated for the second wave at different times. The second wave affected more individuals than the first wave due to various factors such as strictness of restrictions or the lack of it and the emergence of new strains. Project description:BackgroundThe COVID-19 infections and deaths have largely been uneven within and between countries. With 17% of the world's population, India has so far had 13% of global COVID-19 infections and 8.5% of deaths. Maharashtra accounting for 9% of India's population, is the worst affected state, with 19% of infections and 33% of total deaths in the country until 23rd December 2020. Though a number of studies have examined the vulnerability to and spread of COVID-19 and its effect on mortality, no attempt has been made to understand its impact on mortality in the states of India.MethodUsing data from multiple sources and under the assumption that COVID-19 deaths are additional deaths in the population, this paper examined the impact of the disease on premature mortality, loss of life expectancy, years of potential life lost (YPLL), and disability-adjusted life years (DALY) in Maharashtra. Descriptive statistics, a set of abridged life tables, YPLL, and DALY were used in the analysis. Estimates of mortality indices were compared pre- and during COVID-19.ResultCOVID-19 attributable deaths account for 5.3% of total deaths in the state and have reduced the life expectancy at birth by 0.8 years, from 73.2 years in the pre-COVID-19 period to 72.4 years by the end of 2020. If COVID-19 attributable deaths increase to 10% of total deaths, life expectancy at birth will likely reduce by 1.4 years. The probability of death in 20-64 years of age (the prime working-age group) has increased from 0.15 to 0.16 due to COVID-19. There has been 1.06 million additional loss of years (YPLL) in the state, and DALY due to COVID-19 has been estimated to be 6 per thousand.ConclusionCOVID-19 has increased premature mortality, YPLL, and DALY and has reduced life expectancy at every age in Maharashtra.

Project description:The lockdowns that came with policies to reduce the spread of COVID-19 in 2020 required some 90% of museums and historic properties across the globe to be closed. Lowered visitor numbers and reduced staffing levels allowed a range of fauna to make their way indoors, bringing an increase in birds, rodents and insect pests. Silverfish are shy, so benefit from low occupancy in museums and present a potential vector for damage to books and paper. This study is the first to report changes in insect populations in museums and examines six years (2015-2020) trapping data for silverfish and similar insects (Lepismatidae): Lepisma saccharinum, Ctenolepisma calvum, Ctenolepisma longicaudatum and Ctenolepisma lineatum from: (i) the Technisches Museum Wien, (ii) Schönbrunn Palace, (iii) Hofburg Museum and a shorter record from (iv) Weltmuseum Wien. Analysis of the trap contents gives an impression that the number of insects caught had increased over time, but 2020 was distinctive and gave typically higher insect numbers during the COVID-19 lockdown compared to other years, especially for Lepisma saccharinum. Individual traps caught up to 100 silverfish in only a few weeks. Because silverfish usually need between four months to one year to become mature, we assume that it was increased activity during museum closure and not higher reproduction which led to higher numbers. The parts of the museums showing increased populations under lockdown were similar to the areas where they were more frequent in earlier years. This means that such areas deserve continued monitoring even when the museum is closed. No damage to paper objects were reported in the museums investigated.

Project description:PURPOSE:Obstructive sleep apnea (OSA) has been associated with a 2- to 7-fold risk of motor vehicle accidents (MVAs). Continuous positive airway pressure (CPAP) treatment may reduce MVA risk. We further explored this issue in long-term CPAP users and untreated controls. METHODS:We used both before-after and case-control study designs. The observational cohort consisted of CPAP-treated and untreated patients matched for gender, age, and apnea-hypopnea index. All MVAs reported to the police were identified. RESULTS:A total of 2060 patients (75.8% male, mean age 56.0?±?10.5 years) were included. The CPAP-treated patients (N?=?1030) were screened for MVAs for a median of 9.0 years before and after treatment. The median CPAP usage was 6.4 h/day. The control patients (N?=?1030) were screened for MVAs for a median of 6.5 years after discontinuation of CPAP. No significant differences were observed between the incidences of MVAs per 1000 person years before treatment (3.2), after treatment (3.9), or in controls (2.6). Compared with controls, patients who had MVA after treatment had a higher body mass index (BMI), but did not differ in terms of other baseline characteristics, sleep study data, or accident conditions. In the majority of these patients, daytime sleepiness was reduced, whereas BMI tended to increase during treatment. CONCLUSIONS:The MVA incidence did not change after CPAP treatment. Among the patients who had MVA, BMI was the only baseline characteristic that differed between the groups and tended to further increase after CPAP treatment. Differences in sleep study data or accident conditions were not observed.

Project description:IntroductionThe world has been engulfed with the pandemic of the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) which have created significant impact in the emergency surgical health delivery including acute appendicitis. The main aim of this study was to compare the demographic and clinical parameters between two cohorts before the onset of lockdown and within the pandemic.MethodsA retrospective analysis was performed between two groups A and B, who presented with acute appendicitis three months prior to and after initiation of lockdown on March 24 2020 respectively in one of the tertiary centers of Nepal. These two cohorts were compared in demographics, clinicopathological characteristics and surgical aspects of acute appendicitis.ResultsThere were 42 patients in group A and 50 patients in group B. Mean age of the patients was 31.32±17.18 years with male preponderance in group B (N = 29). Mean duration of pain increased significantly in group B [57.8±25.9(B) vs 42.3±25.0(A) hours, P = 0.004] along with mean duration of surgery [51.06±9.4(B) vs 45.27±11.8(A) minutes, P = 0.015]. There was significant decrease in post-operative hospital stay among group B patients [3.04±1.1(B) vs 3.86±0.67(A) days, P = 0.0001]. Complicated cases increased in group B including appendicular perforation in 10 cases. Similarly, mean duration of presentation to hospital significantly increased in group B patients with perforation [69.6±21.01 vs 51.57±17.63 hours, P = 0.008].ConclusionDuring the adversity of the current pandemic, increased number of cases of acute appendicitis can be dealt with surgery as the chances of late presentation and complexity of the lesion exists.
