Development and validation of a questionnaire to test Chinese patients' knowledge of inflammatory bowel disease.

ABSTRACT: A good understanding of a disease facilitates patient-centered management. We aimed to develop and validate a questionnaire to assess inflammatory bowel disease (IBD)-related knowledge and analyze the factors affecting patients' knowledge of IBD. We invited 15 experts to develop and modify an IBD knowledge questionnaires and 709 patients to test the reliability and validity of the questionnaires as well as analyze the factors related to the disease knowledge of patients with IBD. In internal consistency, Cronbach's α coefficients for the common items, ulcerative colitis (UC), and Crohn's disease (CD) knowledge questionnaires were 0.886, 0.89, and 0.886, respectively. In cross-item consistency, Spearman-Brown split coefficients of the common items, UC, and CD knowledge questionnaires were 0.843, 0.812, and 0.812, respectively. In time consistency, the test-retest reliability ICC was 0.862 (P < 0.001). The correlation between researcher scores, IBD-KNOW scores, and the original questionnaire scores was greater than 0.7 (P < 0.001). Multiple linear regression demonstrated that the factors, including disease type, age, body mass index, education level, income, treatment cost, duration of disease, and frequency of visits, affected the IBD patients' knowledge of the disease (P < 0.05). The IBD knowledge questionnaires had good reliability and validity and, therefore, can be used to assess patient knowledge of the disease.

Project description:BackgroundInflammatory bowel disease is an uncommon disease in developing nations whereby patient's knowledge on the disease may be limited. The CCKNOW questionnaire, a widely known questionnaire to assess patient's knowledge on the disease, may be too complex to comprehend for patients in developing countries. The aim of this study is to develop a new tool known as AIBDKQ questionnaire to evaluate the local inflammatory bowel disease patient's knowledge.MethodsThis was a prospective study carried out in four phases. In phase 1, three gastroenterologists with expertise in IBD generated a total of 21 questions related to the general knowledge of the disease in the English language. Phase 2 involved content and face validity whereby the questions were further validated by other gastroenterologists. In phase 3, the validated questions were translated into three languages namely Malay, Mandarin and Tamil which are commonly used in Malaysia. In phase 4 (statistical validity), administration of the questionnaires to patients and hospital staff were conducted to assess the construct validity, discriminative ability, predictive validity and reliability of the questionnaires.ResultsA total of 21 questions were generated initially. Further evaluation indicated that 20 items had adequate kappa and content validity index for relevance (CVI: 0.714 to 1, Kapp: 0.645 to 1) and clarity (CVI: 0.714 to 1, Kapp: 0.645 to 1). The questionnaires in four languages were administered to 213 patients to assess the construct validity. Six items were removed (three for low communality, one for small loading factors, two for cross loading), resulting in 16 final questions. Assessment with 34 hospital staff involving nurses, doctors and clerks showed significant differences in knowledge between the groups (F = 14.007, p < 0.001) and were able to discriminate doctors from nurses and clerks. Another group of 18 hospital staff administered with AIBDKQ and CCKNOW questionnaires showed a Pearson's correlation coefficient of 0.8 indicating strong correlation and concurrent predictive validity between the two questionnaires. Final assessment with 38 patients for reliability assessment revealed high intraclass correlation of the questionnaire among the four languages.ConclusionsThe AIBDKQ has an excellent discriminant ability and internal consistency with a strong correlation when compared to the standard CCKNOW questionnaire.

Project description:BackgroundThe Short Inflammatory Bowel Disease Questionnaire (SIBDQ) is a widely used instrument to assess Health-related Quality of Life (HRQoL) among inflammatory bowel disease (IBD) patients. Our aim was to translate and adapt the SIBDQ so that it could be adequately used in Portugal.MethodsThis is a prospective design cohort study undertaken at a tertiary hospital. This study took place simultaneously with the first part of the SexIDI study, a study aiming to assess the impact of IBD on patients' sexual QoL. The original SIBDQ was translated by two independent translators and adapted by an IBD expert panel following the opinions of a convenient sample of 5 IBD patients. Afterwards, IBD patients from the outpatient clinic were consecutively invited to fill the Portuguese version of the questionnaire (SIBDQ-PT) at three different timepoints (0, 2, 4 weeks). Ninety-two patients completed the SIBDQ-PT at baseline, whereas 33 did so after 2 and 4 weeks (approximately). Statistical analysis was performed using SPSS version 25, and the following aspects were analysed: reliability (through internal consistency, test-retest and intraclass correlation), validity (through exploratory factor analysis [EFA], and Pearson correlation coefficient for linear correlations), score distribution, and responsiveness analysis (through t-student tests).ResultsOverall, SIBDQ-PT was shown to have a high internal consistency (Cronbach's α = 0.80) and a high test-retest reliability (0.80 [CI 0.74-0.86] and 0.69 [CI 0.50-0.82]). EFA detected four dimensions-bowel, social, emotional and systemic. As expected, an overall SIBDQ-PT score was positively correlated with sexual satisfaction (r = 0.27; p < 0.05) and negatively correlated with depression (r = - 0.63; p < 0.01). Moreover, SIBDQ-PT was found to have an adequate score distribution, and to be responsive, as there was a significant subscore change for patients who reported an "overall worsening in general well-being" (0.93 ± 0.13 decrease; p < 0.01).ConclusionsThe Portuguese version of the SIBDQ hereby presented is a reliable, valid and responsive instrument that can be used to measure HRQoL among Portuguese IBD patients.

Project description:BackgroundAppropriate dietary intake can have a significant influence on athletic performance. There is a growing consensus on sports nutrition and professionals working with athletes often provide dietary education. However, due to the limitations of existing sports nutrition knowledge questionnaires, previous reports of athletes' nutrition knowledge may be inaccurate.MethodsAn updated questionnaire has been developed based on a recent review of sports nutrition guidelines. The tool has been validated using a robust methodology that incorporates relevant techniques from classical test theory (CTT) and Item response theory (IRT), namely, Rasch analysis.ResultsThe final questionnaire has 89 questions and six sub-sections (weight management, macronutrients, micronutrients, sports nutrition, supplements, and alcohol). The content and face validity of the tool have been confirmed based on feedback from expert sports dietitians and university sports students, respectively. The internal reliability of the questionnaire as a whole is high (KR = 0.88), and most sub-sections achieved an acceptable internal reliability. Construct validity has been confirmed, with an independent T-test revealing a significant (p < 0.001) difference in knowledge scores of nutrition (64 ± 16%) and non-nutrition students (51 ± 19%). Test-retest reliability has been assured, with a strong correlation (r = 0.92, p < 0.001) between individuals' scores on two attempts of the test, 10 days to 2 weeks apart. Three of the sub-sections fit the Rasch Unidimensional Model.ConclusionsThe final version of the questionnaire represents a significant improvement over previous tools. Each nutrition sub-section is unidimensional, and therefore researchers and practitioners can use these individually, as required. Use of the questionnaire will allow researchers to draw conclusions about the effectiveness of nutrition education programs, and differences in knowledge across athletes of varying ages, genders, and athletic calibres.

Project description:Background: There is no validated tool to assess patients' knowledge of oral anticoagulant therapy in atrial fibrillation in China. Methods: Using a standard translation program, the Jessa Atrial fibrillation Knowledge Questionnaire (JAKQ) was translated into Chinese. The reliability of the JAKQ was assessed by internal consistency (Cronbach's α coefficient), repeatability (test-retest reliability), and sensitivity tests. Effectiveness was assessed by hypothesizing that a lower JAKQ score was a risk factor for bleeding. A total of 447 patients with atrial fibrillation (AF) who were hospitalized between July 2019 and December 2021 were studied and followed up. Participants were followed up 1, 3, 6, and 12 months after enrollment. Bleeding during follow-up was recorded. Data were obtained from hospital databases and telephone follow-up. Result: A total of 447 patients with AF completed JAKQ. The mean age of patients was 67.7 ± 10.2 years. The median JAKQ score was 31.3% (12.5-43.8). The Cronbach's α coefficient of JAKQ was 0.616-0.637, and the test-retest reliability value was 0.902 (p < 0.001). Multivariate logistic regression showed that the higher knowledge level of AF was associated with secondary education or above, an income of more than 2000 yuan, and a history of AF of more than 1 year. Bleeding was associated with a lower JAKQ score, hypertension, and a history of bleeding. Non-bleeding patients on VKA had a better understanding of how often INR should be monitored and what to do if an OAC dose was missed. Conclusion: The Chinese version of JAKQ shows good reliability and validity, indicating that it is a valuable tool for AF and oral anticoagulation (OAC) knowledge assessment. It can be used in clinical practice to guide educational activities and improve the effectiveness and safety of treatment. It was shown that Chinese patients with AF have insufficient knowledge about AF and OAC. Lower JAKQ scores are associated with bleeding, so targeted education is necessary. Targeted educational efforts should focus on patients recently diagnosed with AF and those with lower formal education and income.

Project description:ObjectiveTo develop and validate a questionnaire assessing knowledge, attitudes and practices in the dietary management of IBS.Subjects/methodsAn initial pool of 151 questions was generated addressing three domains (knowledge, attitudes, practices). Academic/senior clinical dietitians (n = 5) provided written feedback and a focus group (n = 4 gastroenterology dietitians) was undertaken to evaluate content and face validity of the question-items. Items considered irrelevant were removed and the refined questionnaire was administered to dietitians with different levels of IBS experience (n = 154) for further psychometric testing. Item reduction analysis was assessed by item difficulty index, discrimination index and point-biserial correlation. Construct validity was assessed via principal component analysis (PCA) and the 'known-groups' method. Internal reliability was assessed by Kuder-Richarson Formula 20 and Cronbach's alpha coefficient and external reliability by interclass correlation coefficient among participants who completed the instrument at baseline and two weeks later (n = 28).ResultsFace and content validity resulted in the removal of 61 items from the initial 151 items. Psychometric testing was applied to the refined 90-item questionnaire administered to participating dietitians, resulting in the final 46-item questionnaire. Six factors were extracted by PCA with varimax rotation explaining 59.2% of the total variance. Partial confirmatory factor analysis showed an acceptable model fit (χ2/df = 2.11, CFI = 0.97, TLI = 0.96, RMSEA = 0.08, SRMR = 0.05). Significant differences were found in sum scores among dietitians with different levels of IBS experience. Internal reliability was >0.7 for each factor. External reliability was >0.6 for each factor and >0.7 for overall items of each domain.ConclusionA validated questionnaire to use in practice and research to assess knowledge, attitudes and practices in the dietary management of IBS has been developed.
