Evaluating the Impact of COVID-19 on Hospital Profit Compensation Activities: A Difference-in-Differences Event Study Analysis in China.

ABSTRACT: The impact of the 2019 coronavirus disease (COVID-19) pandemic is still being revealed, and little is known about the effect of COVID-19-induced outpatient and inpatient losses on hospital operations in many counties. Hence, we aimed to explore whether hospitals adopted profit compensation activities after the 2020 first-wave outbreak of COVID-19 in China. A total of 2,616,589 hospitalization records from 2018, 2019, and 2020 were extracted from 36 tertiary hospitals in a western province in China; we applied a difference-in-differences event study design to estimate the dynamic effect of COVID-19 on hospitalized patients' total expenses before and after the last confirmed case. We found that average total expenses for each patient increased by 8.7% to 16.7% in the first 25 weeks after the city reopened and hospital admissions returned to normal. Our findings emphasize that the increase in total inpatient expenses was mainly covered by claiming expenses from health insurance and was largely driven by an increase in the expenses for laboratory tests and medical consumables. Our study documents that there were profit compensation activities in hospitals after the 2020 first-wave outbreak of COVID-19 in China, which was driven by the loss of hospitalization admissions during this wave outbreak.

Project description:The popularization of financial literacy has become a global trend, with governments across the world expressing commitment to continuously enhancing the financial literacy of their citizens to improve the country's overall financial well-being. However, there is a lack of research evaluating the actual effects of financial literacy on Chinese households. This study first investigated the micro impact of financial literacy on the household stock profit level using data from the 2019 China Household Finance Survey. As most existing studies use factor analysis to measure financial literacy from a single dimension of financial knowledge, our study additionally used the entropy method to construct a composite evaluation system of financial literacy from four dimensions: financial skills, knowledge, attitudes, and behaviors. The ordinary least squares model was utilized as the primary regression model to estimate the correlation, and the average financial literacy of other households in the same community was selected as an instrumental variable. Further instrumental variable regression analysis was conducted using the two-stage least squares method. Three robustness tests were performed to ensure the reliability of the research findings. The results demonstrate that financial literacy significantly enhances household stock profit levels. The mediation effect analysis indicates that financial literacy affects stock profit levels through financial information attention. Moreover, financial literacy has a more substantial promoting effect on stock profit levels for households with members working for state-owned enterprises and those living in first-tier cities. This study confirms the value of financial literacy; identifies important channels for residents to increase their property income; and provides important guidance for the government, educational organizations, and financial institutions. This also injects more vigor into market participation to improve the persistently sluggish Chinese stock market.

Project description:BackgroundNonadherence to medication is a prevalent issue among patients with type 2 diabetes mellitus (T2DM). The family doctor system promotes continuous, integrated, and personalized primary care, which may improve medication adherence. However, more evidence is needed regarding causal association between family doctor system and medication adherence. This study is to assess the impact of the family doctor system on medication adherence among T2DM patients.MethodsThis cohort study utilized data between 2014 and 2019 from three separate administrative databases of an eastern city in China. Adult patients with T2DM who continuously registered with family doctors from 2015 to 2019 (n = 18,841) were assigned to the intervention group, while those who never registered during this period (n = 1,429) were classified as the control group. A difference-in-differences design was employed to compare medication adherence between registered and unregistered T2DM patients after the first stage of family doctor system in 2015 and the second stage in 2018. Propensity score matching was applied to ensure the robustness. The mean proportion of days covered (PDC), and proportion of patients with good adherence (PDC ≥ 80%) were the outcomes of interest. All recommended T2DM medications were included to calculate PDC.ResultsCompared to the unregistered group, PDC across all institutions for registered patients increased by 5.0% (p < 0.001) after the first stage of family doctor system, and by 5.9% (p < 0.001) after the second stage. The proportion of patients with good adherence increased by 9.5% (p < 0.001) and by 11.8% (p < 0.001) after two stages, respectively. The adherence improvement was more pronounced in community health centers. However, the overall good adherence rate among registered patients in 2019 remained relatively low, reaching only 59.77%.ConclusionsThe family doctor system significantly improved medication adherence among T2DM patients by providing patient-centered, continuous, and integrated primary services, especially in community health centers. Nevertheless, further efforts should be made to enhance medication adherence.

Project description:ObjectiveTo determine whether profit status is associated with differences in hospital days per patient, an outcome that may also be influenced by provider financial goals.Data sourcesUnited States Renal Data System Standard Analysis Files and Centers for Medicare and Medicaid Services cost reports.DesignWe compared the number of hospital days per patient per year across for-profit and nonprofit dialysis facilities during 2003. To address possible referral bias in the assignment of patients to dialysis facilities, we used an instrumental variable regression method and adjusted for selected patient-specific factors, facility characteristics such as size and chain affiliation, as well as metrics of market competition.Data extraction methodsAll patients who received in-center hemodialysis at any time in 2003 and for whom Medicare was the primary payer were included (N=170,130; roughly two-thirds of the U.S. hemodialysis population). Patients dialyzed at hospital-based facilities and patients with no dialysis facilities within 30 miles of their residence were excluded.ResultsOverall, adjusted hospital days per patient were 17+/-5 percent lower in nonprofit facilities. The difference between nonprofit and for-profit facilities persisted with the correction for referral bias. There was no association between hospital days per patient per year and chain affiliation, but larger facilities had inferior outcomes (facilities with 73 or more patients had a 14+/-1.7 percent increase in hospital days relative to facilities with 35 or fewer patients). Differences in outcomes among for-profit and nonprofit facilities translated to 1,600 patient-years in hospital that could be averted each year if the hospital utilization rates in for-profit facilities were to decrease to the level of their nonprofit counterparts.ConclusionsHospital days per patient-year were statistically and clinically significantly lower among nonprofit dialysis providers. These findings suggest that the indirect incentives in Medicare's current payment system may provide insufficient incentive for for-profit providers to achieve optimal patient outcomes.
