Epidemiological characteristics of pulmonary tuberculosis among students in Guizhou, China: a retrospective study from 2010 to 2020.

ABSTRACT:

Objective

We described epidemiological characteristics of pulmonary tuberculosis (PTB) among students and evaluated susceptible populations and areas in Guizhou province and also to provide scientific suggestions for prevention and control.

Setting

Guizhou, China.

Design

This is a retrospective epidemiological study on PTB in students.

Methods

Data are from the China Information System for Disease Control and Prevention. We collected all PTB cases among the student population from 2010 to 2020 in Guizhou. Incidence, composition ratio and hotspot analysis were used to describe epidemiological and some clinical characteristics.

Results

A total of 37 147 new student PTB cases were registered among the population aged 5-30 years during 2010-2020. The proportions of men and women were 53.71% and 46.29%, respectively. Cases aged 15-19 years dominated (63.91%), and the proportion of ethnic groups was increasing during the period. Generally, the raw annual incidence of PTB among the population was increasing from 32.585 per 100 000 persons in 2010 to 48.872 per 100 000 persons in 2020 (c ² _trend=1283.230, p<0.001). March and April were the peak months of a year, and cases were clearly grouped in Bijie city. New cases were mainly identified via physical examination, and cases from active screening were still low (0.76%). Additionally, secondary PTB accounted for 93.68%, positive rate of pathogen was only 23.06%, and the recovery rate was 94.60%.

Conclusions

The population aged 15-19 years is the vulnerable population, and Bijie city is the susceptible area. BCG vaccination and promotion for active screening should be the priority of futural PTB prevention and control. Tuberculosis laboratory capacity should be improved.

Project description:ImportanceThe World Health Organization End TB (Tuberculosis) Strategy aims to decrease the global incidence and mortality of TB by 90% and 95%, respectively, as of 2035.ObjectiveTo characterize the recent epidemiological trend of pulmonary TB (PTB) in mainland China based on the national surveillance data.Design, setting, and participantsThis cross-sectional study collected demographic and clinical data of all patients reported in the national Tuberculosis Information Management System of China from January 1, 2005, to November 21, 2016. Data were analyzed from December 1, 2019, to July 31, 2020.ExposuresPulmonary TB was defined as bacteriologically confirmed or clinically diagnosed TB in the lung parenchyma or the tracheobronchial tree.Main outcomes and measuresTemporal and spatial variation of annual incidence and demographic features of PTB in mainland China.ResultsIn total, 10 582 903 patients with PTB were reported in mainland China from 2005 to 2016. The median age of patients with PTB was 46 (interquartile range [IQR], 30-61) years, and 28.53% were 60 years or older. Most patients with PTB were male (69.8%) and farmers or herders (70.0%). The mean (SD) incidence of PTB was 66.61 (8.09) per 100 000 population. The annual incidence decreased from 72.95 per 100 000 population in 2005 to 52.18 per 100 000 population in 2016, and the reduction was greater in the eastern and central regions (31.6%; from 69.43 to 47.48 per 100 000 population) than in the western region (21.0%; from 82.06 to 64.82 per 100 000 population). Xinjiang Uygur Autonomous Region (135.03 per 100 000 population), Guizhou Province (115.98 per 100 000 population), and the Tibet Autonomous Region (101.98 per 100 000 population) had the highest mean annual incidences. The median time from onset of illness to diagnosis decreased from 36 (IQR, 16-92) days from 2005 to 2007 to 31 (IQR, 15-63) days in 2008 and later (P < .001) and was longer in the western region than in the eastern and central regions (41 [IQR, 20-91] vs 30 [IQR, 13-61] days; P < .001).Conclusions and relevanceAlthough this study found that the incidence of PTB in mainland China showed a downward trend from 2005 to 2016, to achieve the World Health Organization 2035 goal, innovative and more efficient prevention and control strategies are needed, particularly among the most susceptible population, that is, farmers and herders in western China.

Project description:BackgroundPulmonary tuberculosis (PTB), as a respiratory infectious disease, poses significant risks of covert transmission and dissemination. The high aggregation and close contact among students in Chinese schools exacerbate the transmission risk of PTB outbreaks.ObjectiveThis study investigated the epidemiological characteristics, geographic distribution, and spatiotemporal evolution of student PTB in Chongqing, Southwest China, aiming to delineate the incidence risks and clustering patterns of PTB among students.MethodsPTB case data from students monitored and reported in the Tuberculosis Information Management System within the China Information System for Disease Control and Prevention were used for this study. Descriptive analyses were conducted to characterize the epidemiological features of student PTB. Spatial trend surface analysis, global and local spatial autocorrelation analyses, and disease rate mapping were performed using ArcGIS 10.3. SaTScan 9.6 software was used to identify spatiotemporal clusters of PTB cases.ResultsFrom 2016 to 2022, a total of 9920 student TB cases were reported in Chongqing, Southwest China, with an average incidence rate of 24.89/100,000. The incidence of student TB showed an initial increase followed by a decline, yet it remained relatively high. High school students (age: 13-18 years; 6649/9920, 67.03%) and college students (age: ≥19 years; 2921/9920, 29.45%) accounted for the majority of student PTB cases. Patient identification primarily relied on passive detection, with a high proportion of delayed diagnosis and positive etiological results. COVID-19 prevention measures have had some impact on reducing incidence levels, but the primary factor appears to be the implementation of screening measures, which facilitated earlier case detection. Global spatial autocorrelation analysis indicated Moran I values of >0 for all years except 2018, ranging from 0.1908 to 0.4645 (all P values were <.05), suggesting strong positive spatial clustering of student PTB cases across Chongqing. Local spatial autocorrelation identified 7 high-high clusters, 13 low-low clusters, 5 high-low clusters, and 4 low-high clusters. High-high clusters were predominantly located in the southeast and northeast parts of Chongqing, consistent with spatial trend surface analysis and spatiotemporal clustering results. Spatiotemporal scan analysis revealed 4 statistically significant spatiotemporal clusters, with the most likely cluster in the southeast (relative risk [RR]=2.87, log likelihood ratio [LLR]=574.29, P<.001) and a secondary cluster in the northeast (RR=1.99, LLR=234.67, P<.001), indicating higher reported student TB cases and elevated risks of epidemic spread within these regions.ConclusionsFuture efforts should comprehensively enhance prevention and control measures in high-risk areas of PTB in Chongqing to mitigate the incidence risk among students. Additionally, implementing proactive screening strategies and enhancing screening measures are crucial for early identification of student patients to prevent PTB outbreaks in schools.

Project description:BackgroundWe used data released by the government to analyze the epidemiological distribution of pulmonary tuberculosis in mainland China from 2004 to 2015, in order to provide a deeper understanding of trends in the epidemiology of pulmonary tuberculosis in China and a theoretical basis to assess the effectiveness of government interventions and develop more targeted prevention and control strategies.MethodsA discrete dynamic model was designed based on the epidemiological characteristics of pulmonary tuberculosis and fitted to data published by the government to estimate changes in indicators such as adequate contact rate, prevalence of non-treated pulmonary tuberculosis (abbreviated as prevalence), and infection rate. Finally, we performed sensitivity analyses of the effects of parameters on the population infection rate.ResultsThe epidemiological features of pulmonary tuberculosis in China include a pattern of seasonal fluctuations, with the highest rates of infection in autumn and winter. The adequate contact rate has increased slightly from an average of 0.12/month in 2010 to an average of 0.21/month in 2015. The prevalence in the population has continued to decrease from 3.4% in early 2004 to 1.7% in late 2015. The Mycobacterium tuberculosis (M. tuberculosis) infection rate in the population decreased gradually from 42.3% at the beginning of 2004 to 36.7% at the end of 2015. The actual number of new infections gradually decreased from 1,300,000/year in 2010 to 1,100,000/year in 2015. The actual number of new patients each year has been relatively stable since 2010 and remains at approximately 2,600,000/year.ConclusionsThe population prevalence and the M. tuberculosis infection rate have decreased year by year since 2004, indicating that the tuberculosis epidemic in China has been effectively controlled. However, pulmonary tuberculosis has become increasingly contagious since 2010. China should focus on the prevention and control of pulmonary tuberculosis during autumn and winter.

Project description:BackgroundAt present, there is a need for more substantial real-world evidence on the factors influencing the effectiveness of tuberculosis (TB) treatment in HIV/TB co-infected patients.ObjectivesThis retrospective study aims to identify factors affecting TB treatment effectiveness in HIV/TB co-infected patients.DesignRetrospective cross-sectional study.MethodsWe included 461 HIV/TB co-infected patients, deriving 742 samples based on each initial positive TB test period. A total of 7788 valid treatment records corresponding to 17 TB drug compositions and 150 clinical indicators (each > 100 records) were used for analysis. Data mining techniques were employed, including consensus clustering, Fisher's exact test, stratified analysis, multivariate logistic regression analysis, and three modeling approaches (logistic regression, support vector machine, and random forest).ResultsThe TB treatment effectiveness of CD4+ T cell count ⩽ 42 is significantly lower than that of the sample group > 42 (aOR: 1.77, 95% CI: 1.15-2.74, p = 0.010). The TB treatment effectiveness of the "rifabutin and levofloxacin alone or in combination" group is significantly higher than that of the "other first- and second-line anti-TB drugs in combination" group (aOR: 0.10, 95% CI: 0.01-0.64, p = 0.022). Significant differences exist in factors between TB treatment effective and ineffective groups, including age (aOR: 2.12, 95% CI: 1.10-4.20, p = 0.027), pre-treatment high-density lipoprotein (HDL) cholesterol (aOR: 0.47, 95% CI: 0.25-0.89, p = 0.022), pre-treatment CD8+ T cell count (aOR: 0.55, 95% CI: 0.33-0.90, p = 0.019), pre-treatment neutrophil percentage (aOR: 0.68, 95% CI: 0.48-0.96, p = 0.030), rifabutin (aOR: 1.59, 95% CI: 1.09-2.32, p = 0.016), and cycloserine (aOR: 0.21, 95% CI: 0.03-0.77, p = 0.041). The best area under the receiver operating characteristic curve of the test set under three modeling methods is 0.560-0.763. Rate of lymphocyte percentage recovering to normal is significantly higher in the TB treatment-effective group than in the treatment-ineffective group (aOR: 1.83, 95% CI: 1.09-3.10, p = 0.022).ConclusionCD4+ T cell count of 42/μL assists TB treatment effectiveness evaluation. Rifabutin and levofloxacin show more therapeutic benefits. Lymphocyte percentage can serve as an effective TB therapeutic and diagnostic target. Age, pre-treatment factors (HDL cholesterol, CD8+ T cell count, and neutrophil percentage), rifabutin, and cycloserine are significantly associated with TB treatment effectiveness. Factors affecting TB treatment effectiveness for HIV/TB co-infected patients need more evidence.

Project description:Hand, foot, and mouth disease (HFMD) is well recognized as one of the major threats to children's health globally. The increasing complexity of the etiology of HFMD still challenges disease control in China. There is little surveillance of the molecular epidemiological characteristics of the enteroviruses (EVs) that cause HFMD in Neijiang city or the Sichuan Basin area in Southwest China. In this study, demographic and epidemiological information for 14,928 probable HFMD cases was extracted and analyzed to describe the epidemic features of HFMD in Neijiang city from Jan 2010 to Dec 2017. The swab samples of select probable HFMD cases from 2012 to 2017 were tested by reverse transcription (RT) real-time PCR to identify the serotype distribution of EVs, and 110 randomly selected RT-real-time PCR positive samples were then amplified and analyzed for the VP1 or VP4 regions of EVs to further analyze the phylogenetic characteristics of the circulating strains in this area. The eight-year average annual incidence was 49.82 per 100,000 in Neijiang. The incidence rates varied between 19.51 and 70.73 per 100,000, demonstrating peaks of incidence in even-number years (2012, 2014 and 2016). The median age of the probable cases was 27 months and the interquartile range (25th to 75th percentile) of ages for the probable HFMD cases was between 14 and 42 months. The male-to-female ratio of the probable HFMD cases was 1.47:1, and scattered children were the major population classification (81.7%). Two epidemic peaks were observed: one major peak between April and July and the other lesser peak between October and December. Of 6513 probable cases tested with RT-real-time PCR, 4015 (61.6%) were positive for enterovirus with the serotype distribution as follows: EV71+, 30.1% (n = 1210); CV-A16+, 28.7% (n = 1154) and a sole pan-enterovirus+, 41.1% (n = 1651). A total of 91 cases (82.7%, 91/110) were successfully amplified and underwent phylogenetic analysis: all EV71+ cases were C4a serotype (n = 23/30); all CV-A16+ cases were B2b serotype (n = 24/30); of 42 sole pan-enterovirus+ samples, 20 were CV-A6, 14 were CV-A10 and the rest within this group were CV-A4 (n = 4), CV-A8 (n = 2), CV-A9 (n = 1) and CV-B3 (n = 1). Our findings provide important evidence that aids the improvement of strategies for vaccination against HFMD and comprehensive disease control in China.
