Coping Strategies for Pertussis Resurgence.

ABSTRACT: Pertussis (whooping cough) is a respiratory disease caused primarily by Bordetella pertussis, a Gram-negative bacteria. Pertussis is a relatively contagious infectious disease in people of all ages, mainly affecting newborns and infants under 2 months of age. Pertussis is undergoing a resurgence despite decades of high rates of vaccination. To better cope with the challenge of pertussis resurgence, we evaluated its possible causes and potential countermeasures in the narrative review. Expanded vaccination coverage, optimized vaccination strategies, and the development of a new pertussis vaccine may contribute to the control of pertussis.

Acellular pertussis vaccines and pertussis resurgence: revise or replace?

Project description:The resurgence of pertussis (whooping cough) in countries with high vaccination coverage is alarming and invites reconsideration of the use of current acellular pertussis (aP) vaccines, which have largely replaced the old, reactogenic, whole-cell pertussis (wP) vaccine. Some drawbacks of these vaccines in terms of limited antigenic composition and early waning of antibody levels could be anticipated by the results of in-trial or postlicensure human investigations of B- and T-cell responses in aP versus wP vaccine recipients or unvaccinated, infected children. Recent data in experimental models, including primates, suggest that generation of vaccines capable of a potent, though regulated, stimulation of innate immunity driving effective, persistent adaptive immune responses against Bordetella pertussis infection should be privileged. Adjuvants that skew Th1/Th17 responses or new wP (detoxified or attenuated) vaccines should be explored. Nonetheless, the high merits of the current aP vaccines in persuading people to resume vaccination against pertussis should not be forgotten.

Bordetella pertussis strains with increased toxin production associated with pertussis resurgence.

Project description:Before childhood vaccination was introduced in the 1940s, pertussis was a major cause of infant death worldwide. Widespread vaccination of children succeeded in reducing illness and death. In the 1990s, a resurgence of pertussis was observed in a number of countries with highly vaccinated populations, and pertussis has become the most prevalent vaccine-preventable disease in industrialized countries. We present evidence that in the Netherlands the dramatic increase in pertussis is temporally associated with the emergence of Bordetella pertussis strains carrying a novel allele for the pertussis toxin promoter, which confers increased pertussis toxin (Ptx) production. Epidemiologic data suggest that these strains are more virulent in humans. We discuss changes in the ecology of B. pertussis that may have driven this adaptation. Our results underline the importance of Ptx in transmission, suggest that vaccination may select for increased virulence, and indicate ways to control pertussis more effectively.

Can vaccine legacy explain the British pertussis resurgence?

Project description:Pertussis incidence has been rising in some countries, including the UK, despite sustained high vaccine coverage. We questioned whether it is possible to explain the resurgence without recourse to complex hypotheses about pathogen evolution, subclinical infections, or trends in surveillance efficiency. In particular, we investigated the possibility that the resurgence is a consequence of the legacy of incomplete pediatric immunization, in the context of cohort structure and age-dependent transmission. We constructed a model of pertussis transmission in England and Wales based on data on age-specific contact rates and historical vaccine coverage estimates. We evaluated the agreement between model-predicted and observed patterns of age-specific pertussis incidence under a variety of assumptions regarding the duration of immunity. Under the assumption that infection-derived immunity is complete and lifelong, and regardless of the duration of vaccine-induced immunity, the model consistently predicts a resurgence of pertussis incidence comparable to that which has been observed. Interestingly, no resurgence is predicted when infection- and vaccine-derived immunities wane at the same rate. These results were qualitatively insensitive to rates of primary vaccine failure. We conclude that the alarming resurgence of pertussis among adults and adolescents in Britain and elsewhere may simply be a legacy of historically inadequate coverage employing imperfect vaccines. Indeed, we argue that the absence of resurgence at this late date would be more surprising. Our analysis shows that careful accounting for age dependence in contact rates and susceptibility is prerequisite to the identification of which features of pertussis epidemiology want additional explanation.

The impact of past vaccination coverage and immunity on pertussis resurgence.

Project description:The resurgence of pertussis over the past decades has resulted in incidence levels not witnessed in the United States since the 1950s. The underlying causes have been the subject of much speculation, with particular attention paid to the shortcomings of the latest generation of vaccines. We formulated transmission models comprising competing hypotheses regarding vaccine failure and challenged them to explain 16 years of highly resolved incidence data from Massachusetts, United States. Our results suggest that the resurgence of pertussis is a predictable consequence of incomplete historical coverage with an imperfect vaccine that confers slowly waning immunity. We found evidence that the vaccine itself is effective at reducing overall transmission, yet that routine vaccination alone would be insufficient for elimination of the disease. Our results indicated that the core transmission group is schoolchildren. Therefore, efforts aimed at curtailing transmission in the population at large, and especially in vulnerable infants, are more likely to succeed if targeted at schoolchildren, rather than adults.

Combating pertussis resurgence: One booster vaccination schedule does not fit all.

Project description:Pertussis has reemerged as a major public health concern in many countries where it was once considered well controlled. Although the mechanisms responsible for continued pertussis circulation and resurgence remain elusive and contentious, many countries have nevertheless recommended booster vaccinations, the timing and number of which vary widely. Here, using a stochastic, age-stratified transmission model, we searched for cost-effective booster vaccination strategies using a genetic algorithm. We did so assuming four hypothesized mechanisms underpinning contemporary pertussis epidemiology: (I) insufficient coverage, (II) frequent primary vaccine failure, (III) waning of vaccine-derived protection, and (IV) vaccine "leakiness." For scenarios I-IV, successful booster strategies were identified and varied considerably by mechanism. Especially notable is the inability of booster schedules to alleviate resurgence when vaccines are leaky. Critically, our findings argue that the ultimate effectiveness of vaccine booster schedules will likely depend on correctly pinpointing the causes of resurgence, with misdiagnosis of the problem epidemiologically ineffective and economically costly.

Transcriptomics of coping strategies in free-swimming Lepeophtheirus salmonis (Copepoda)

Project description:To understand the sensitivity of larval lice to changing environmental parameters we applied a 38K oligo microarray and characterized transcriptome responses after 24 hour exposures to reduced salinity (30-10 parts per thousand (ppt)) or varied temperature (16-4M-BM-:C). To further characterize the effects of salinity, we profiled expression changes over a range of seawater with single increment differences (30-25 ppt). Three separate projects. Low resolution salinity (aka wide range or LR_SAL) contains 2 duplicate experiments, each with pools of lice (~500 copepodid lice per beaker; n=3 beakers per condition), incubated for 24 hr at 10M-BM-:C diluted to 30, 25, 20, or 10 parts per thousand (ppt). Temperature study conducted as above, but with a constant salinity of 30 ppt and changed temperature of 4, 10, or 16M-BM-:C. High resolution salinity conducted as above, but with no experiment replication, but with biological repication of n=6 beakers per condition, and with a range of 25, 26, 27, 28, 29, 30 ppt salinity. Please note that each experiment was normalized separately.

Resurgence of Bordetella pertussis, including one macrolide-resistant isolate, France, 2024.

Project description:As other European countries, France is experiencing a resurgence of pertussis in 2024. Between 1 January and 31 May 2024, 5,616 (24.9%) positive Bordetella pertussis qPCR tests were identified, following a 3-year period of almost null incidence. Of 67 cultured and whole genome sequenced B. pertussis isolates, 66 produced pertactin and 56 produced FIM2, in contrast to pre-COVID-19 years. One isolate of genotype Bp-AgST4 was resistant to macrolides. Pertussis resurgence may favour isolates that produce FIM2 and pertactin.

Resurgence of Pertussis Infections in Shandong, China: Space-Time Cluster and Trend Analysis.

Project description:Although vaccination is effective in preventing infection, pertussis remains endemic worldwide, including China. To lead better targeted prevention strategies, we examined dynamics of spatial and temporal patterns of pertussis transmission in Shandong, China, from 2009 to 2017. We used space-time cluster analysis, logistic regression analysis, and regression tree model to detect the changes in spatial patterns of pertussis infections in Shandong Province, China, between periods (2009-2011, 2012-2014, and 2015-2017). The yearly pertussis incidence rates dramatically increased by 16.8 times from 2009 to 2017. Shifting patterns of peaks of pertussis infections were observed over both time (from June-July to August-September) and space (from Linyi to Jinan), with increasing RR from 4.1 (95% CI: 2.3-7.4) (2009-2011) to 6.1 (95% CI: 5.6-6.7) (2015-2017) and obvious coincidence of peak time. West Shandong had larger odds of increased infections over the study period (odds ratio: 1.52 [95% CI: 1.05-2.17]), and pertussis had larger odds of spreading to east (odds ratio: 2.32 [95% CI: 1.63-3.31]) and north (odds ratio: 1.69 [95% CI: 1.06-2.99]) over time. Regression tree model indicated that the mean difference in yearly average pertussis incidence between 2009-2011 and 2015-2017 increased by more than 4-fold when the longitudes of counties are < 118.0°E. The geographic expansion of pertussis infection may increase the risk of epidemic peaks, coinciding with increased infections in the future. The findings might offer evidence for targeting preventive measures to the areas most in need to minimize the impact of the disease.
