Dietary Intake of Multiple Nutrient Elements and Associated Health Effects in the Chinese General Population from a Total Diet Study.

ABSTRACT: Nutrient elements are essential for human health. The intake of nutrient elements (Na, K, Ca, Mg, P, Mn, Fe, Zn, Cu, Se, Mo, and Cr) in the general Chinese population was comprehensively evaluated via a recent total diet study (2016-2019), covering more than two-thirds of the total population. The contents of nutrient elements in 288 composite dietary samples were determined by ICP-MS. The dietary sources, regional distribution, the relationship with the earth's crust, dietary intake, and health effects were discussed. Plant foods were the main source of both macro-elements and trace elements, accounting for 68-96% of total intake. Trace elements in food were compatible with their abundance in the earth's crust. Na intake reduced by 1/4 over the last decade but was still high. The average intake of Ca, Mg, Zn, and Se did not reach the health guidance values, while the average intake of K, P, Mn, Fe, Cu, Mo, and Cr fell within a reasonable range. No element exceeded the UL. However, an imbalance was identified in the dietary Na/K ratio and Ca/P ratio. This paper provides a most recent and national-representative assessment of nutrient element intake, indicating the significance of salt reduction and dietary structure optimization for the population.

Project description:BackgroundDietary nutrient intake plays a significant role in carcinogenesis. Few studies have investigated the association between dietary nutrient intake and cervical intraepithelial neoplasia (CIN) risk in China.MethodsData on 2304 women from an ongoing cohort comprising 40,000 women from China in 2014 were included. Study randomly selected 218 out of 2304 people as subjects during 2019. All participants were surveyed through in-person interviews, physical examinations, and laboratory tests. Clinical data were obtained from physical examinations and laboratory tests. Dietary intakes were assessed using a semiquantitative food frequency questionnaire. Nutrition intakes from 26 food sources were calculated using a comprehensive validated database. Descriptive statistics were used to describe the frequency and proportion, and mean and standard deviation of the demographic characteristics. Characteristics were examined for significant differences, and Pearson chi-square tests were used for categoric variables. Logistic regression was used to obtain odds ratios (ORs) and confidence intervals (CIs) for CIN risk in each nutrient intake quartile relative to that in the highest quartile.ResultsThe food frequency questionnaire exhibited acceptable reproducibility and reasonable validity in assessing nutrient intakes among these women. After adjusting for multiple confounders, several dietary nutrients showed significant associations with CIN2+ risk. Low dietary folate intake was associated with the risk of CIN2+ (first versus fourth quartile: OR = 1.55, 95% CI 1.03-2.33). Similar results were also observed for vitamin B6 (OR = 1.63, 95% CI 1.08-2.46), vitamin C (OR = 1.59, 95% CI 1.05-2.42), niacin (OR = 1.65, 95% CI 1.08-2.51), and vitamin K (second versus fourth quartile: OR = 1.60, 95% CI 1.05-2.44).ConclusionsLow folate; vitamin B6, C, and K; and niacin intakes were associated with CIN2+ risk. Nutrients may influence the development of higher grade CIN and cervical cancer. Project description:BackgroundOsteoporosis, characterized by reduced bone mineral density (BMD) increases the risk of all-cause mortality. Assessments of whether dietary selenium intake is related to bone health are scarce, with few relevant studies limited by a small sample size. The aim of the present study was to investigate the association between dietary selenium intake and BMD levels in the National Health and Nutrition Examination Survey (NHANES) database.MethodsWe extracted and aggregated data from 4 cycles of the NHANES [2005-2010, 2013-2014]. Dietary selenium intake was obtained from 24-hour dietary recall interviews. BMD measurement, including the femur, femur neck, trochanter and intertrochanter of the femur, and lumbar spine, was performed using dual-energy X-ray absorptiometry. The multivariable linear regression model for the association between dietary selenium intake and BMD and the generalized additive model (GAM) for the dose-response relationship were used.ResultsA total of 21,939 participants were included. The mean age was 40.68±22.42 years, and 51.28% were male. In the multivariable adjustment model, participants with the highest quintiles of dietary selenium intake (Q5) were associated with increased BMD levels in the total femur (β=0.014, 95% CI: 0.008-0.020, P<0.001), femur neck (β=0.010, 95% CI: 0.004-0.016, P=0.001), trochanter (β=0.011, 95% CI: 0.005-0.017, P<0.001), intertrochanter (β=0.017, 95% CI: 0.010-0.025, P<0.001), and lumbar spine (β=0.013, 95% CI: 0.005-0.020, P<0.001) compared with those in quintile 1 (Q1). The dose-response relationship showed an inverted U-shape relationship between dietary selenium intake and BMD levels (P for nonlinearity <0.05). Participants tended to have increased levels of BMD as the dietary selenium intake increased when dietary selenium was below the turning point, and then a negative relationship was observed when dietary intake was higher than the turning point.ConclusionsOur study indicated that dietary selenium intake exhibited an inverted U-shape trend in relation to BMD levels, which demonstrates the need for selenium status in the body to be considered when discussing the role of dietary selenium intake in BMD. Future studies are needed to confirm these findings and explore the underlying biological mechanism.

Project description:Cotinine, the primary metabolite of nicotine, can be utilized as a marker for active smoking and as an indicator of exposure to secondhand smoke. However, the direct relationship between dietary flavonoid intake and serum cotinine levels remains a subject of ongoing investigation. In this study, we utilized data from the National Health and Nutrition Examination Survey (NHANES) 2007-2010 and 2017-2018 to assess the association between dietary flavonoid intake and serum cotinine levels in adults through multiple linear regression analysis. A weighted quantile sum (WQS) regression model was used to assess the association of the mixture of six dietary flavonoids with serum cotinine levels in adults, which could represent the overall effect of the mixture of six dietary flavonoids. We also conducted stratified analyses by smoke status to explore multiple linear regression associations between different flavonoid intake and serum cotinine levels. A total of 14,962 adults were included in the study. Compared to the group with the lowest dietary flavonoid intake, total flavonoid intake in the second (β = -0.29 [-0.44, -0.14]), third (β = -0.41 [-0.58, -0.24]), and highest groups (β = -0.32 [-0.49, -0.16]) was inversely related to the levels of serum cotinine after adjusting the full model. An RCS model showed that when the total dietary flavonoid intake was less than 99.61 mg/day, there was a negative linear association between dietary flavonoid intake and the serum cotinine. The WQS regression model also showed that the intake of a mixture of six dietary flavonoids was significantly negatively correlated with serum cotinine levels (β = -0.54 [-0.61, -0.46], p <0.01), with anthocyanins having the greatest effect (weights = 32.30%). Our findings imply a significant correlation between dietary flavonoid intake and serum cotinine levels among adults. The consumption of a combination of six dietary flavonoids was consistently linked to lower serum cotinine levels, with anthocyanins displaying the most pronounced impact.

Project description:BackgroundUnfortified plant-based diets are devoid of vitamin B12, and supply low intakes of iodine, zinc, selenium, and calcium. To disentangle the complex interplay between nutritional adequacy and nutrient intakes from supplements and foods in plant-based diets, data from a Germany-based cross-sectional study examining the nutritional status of omnivores, lacto-ovo-vegetarians and vegans was re-analyzed. Special emphasis was put on potentially under-consumed nutrients in plant-based diets, including vitamin A and choline.MethodsA novel tool focusing on under-consumed micronutrients was employed to shed a new light on nutrient supply and dietary exposure to critical nutrients in plant-based diets: The Total Nutrient Index (TNI). The TNI extends existing measures of diet quality by considering nutrient intake data from both foods and supplements. The TNI covers calcium, magnesium, potassium, choline and vitamins A, C, D, and E. The TNI was compared between omnivores, vegetarians and vegans, with a special focus on its micronutrient component scores and with regard to dietary supplement contributions.ResultsData from 108 participants was analyzed. The vegan and the omnivorous diet resulted in similar TNI scores (73.70 ± 19.68 and 72.77 ± 17.88), whereas lacto-ovo-vegetarians scored lower (68.50 ± 17.10). The contribution of supplements to the TNI was higher in vegans and omnivores (median contribution: 12.50 (16.80) and 10.81 (18.23) score points, respectively) as compared to lacto-ovo-vegetarians (3.42 (12.50) score points). High micronutrient component score contributions to the TNI were found for vitamin D supplements (all dietary groups), vitamin C supplements (omnivores and vegans) and magnesium supplements (all groups).ConclusionsSupplementation has a profound impact on nutrient supplies in individuals on a plant-based diet. This study reiterates the need to quantitatively assess nutrient intakes from supplements to assess diet quality of plant-based dietary patterns. We posit that defining diet-specific TNI scores is important for a precise evaluation of diet quality, whether in omnivore or in the spectrum of plant-based diets.
