Knotless Implant for Arthroscopic Hip Capsule Closure

ABSTRACT: The hip joint capsule plays a significant role in maintaining hip stability, including translation and rotation. Hip capsular closure or plication has been shown to increase stability of the joint following capsulotomy in hip arthroscopy for the treatment of femoroacetabular impingement syndrome (FAIS) and/or associated labral tears. This technique article describes a knotless method of closing the hip capsule. Project description:The hip joint capsule is a critical static stabilizer of the hip. During hip arthroscopy, the capsule is breached to gain exposure to treat femoroacetabular impingement lesions. There have been recent concerns regarding hip instability after hip arthroscopy, and relatively few techniques have been described for atraumatic arthroscopic closure of the hip capsule. We describe an atraumatic, inside-out, 2-portal technique to repair the capsule.

Arthroscopic Hip Labral Reconstruction and Augmentation Using Knotless Anchors.

Project description:Biomechanical stability is the primary function of the acetabular labrum. It provides a hip suction seal and optimal joint function. Labral tears are a common reason for hip arthroscopy, to improve patient function and to prevent long-term degenerative arthropathy. Arthroscopic labral repair has shown significantly better outcomes in return to premorbid activity levels when compared with labral debridement. Injury to the acetabular labrum is a challenge and can lead to long-term complications. In this scenario, arthroscopic labral reconstruction has shown good results regarding patient subjective and objective outcomes. We describe a technique for complete arthroscopic labral reconstruction using tensor fascia lata allograft.

Project description:We describe a modification to a standard labral repair technique that uses a knotless suture anchor. This technique modification allows for improved suture management and maneuverability during suture passing and tensioning. This technique can be reproduced and has the potential to be implemented in other procedures.

Arthroscopic Hip Capsule Reconstruction for Anterior Hip Capsule Insufficiency in the Revision Setting.

Project description:Iatrogenic hip instability is increasingly recognized as a cause of persistent pain and disability after hip arthroscopy. Many authors currently advocate capsular repair to reduce postoperative instability. However, anatomic deficiencies in the anterosuperior capsule can prevent a functional capsular repair, particularly in the revision setting. Capsular reconstruction has been shown to restore biomechanical stabilization in cadaveric models and improve short-term patient outcomes in patients with primary hip arthroscopy failure. Arthroscopic hip capsular reconstruction is technically challenging, largely owing to complex suture management and difficulties with graft placement and sizing. This article describes the capsular reconstruction technique, detailing the technical aspects of anterosuperior capsular defect identification; capsular preparation; suture management; and dermal allograft sizing, preparation, and positioning.

Capsule Closure of Periportal Capsulotomy for Hip Arthroscopy.

Project description:Multiple approaches for management of the hip capsule during hip arthroscopy for femoroacetabular impingement syndrome have been reported. Capsular closure is advocated in the setting of larger capsulotomies, including interportal and T-capsulotomies, to reduce the risk of iatrogenic instability or microinstability of the hip. The periportal capsulotomy technique has been described for conservative management of the capsule that would not necessitate closure. However, hip arthroscopy for patients with ligamentous laxity or joint hypermobility may warrant capsule closure or plication even with use of conservative capsulotomy techniques. We introduce a technique for closure of periportal capsulotomy as a means to repair or plicate the hip capsule in the at-risk hypermobile patient.

Arthroscopic capsule reconstruction in the hip using iliotibial band allograft.

Project description:The hip capsule has been identified as an important static stabilizer of the hip joint. Despite the intrinsic bony stability of the hip socket, the capsule plays a key role in hip stability, particularly at the extremes of motion, and the iliofemoral ligament is the most important stabilizer in extension and external rotation. Patients who do not undergo capsular closure or plication may continue to complain of hip pain and dysfunction postoperatively, likely because of microinstability or muscle invagination into the capsular defect, and high-resolution magnetic resonance imaging or magnetic resonance arthrography will identify the capsular defect. Seen primarily in the revision setting, capsular defects can cause recurrent stress at the chondrolabral junction. An attempt at secondary closure can be challenging because of capsular limb adherence to the surrounding soft tissues. Therefore reconstruction may be the only possible surgical solution for this problem. We describe our new surgical technique for arthroscopic hip capsular reconstruction using iliotibial band allograft.

Arthroscopic Shoelace Capsular Closure Technique in the Hip Using Ultratape.

Project description:Hip arthroscopy is promising tool for assessing and treating intra-articular pathologies including labral tears, cartilage injuries, and ligamentum teres tears of the hip. Interportal capsulotomy allows for better visualization and accessibility of the arthroscope and working instruments. The hip capsule has been defined as a crucial stabilizer of the hip joint. Thus, capsular closure is recognized as an important procedure to prevent postoperative instability after hip arthroscopic surgery. Despite the routine capsular closure during hip arthroscopy, there is a small subset of patients who complain of hip pain and dysfunction after surgery most likely because of disruption of hip closure site after routine complete capsular closure with strong suture for treating hip instability. This technical note describes the arthroscopic shoelace capsular suture technique using Ultratape for treating femoroacetabular impingement with capsular laxity and borderline hip dysplasia.

Arthroscopic Hip Capsular Plication With Augmentation Using a Bioinductive Collagen Implant.

Project description:With advancements in arthroscopic techniques and instrumentation, hip arthroscopy has become an increasingly used technique to treat soft-tissue and osseous pathologies about the hip. Patient predisposition to labral and capsular injuries can present as femoroacetabular impingement or hip dysplasia, sometimes in combination. Capsular management continues to be a topic of debate, with capsular repair becoming the standard of care in most cases. Furthermore, in cases of borderline dysplasia and microinstability, considerations for not only capsular repair but with plication has shown significant clinical success. Although plication in this setting has shown promise, given a 20% failure rate, we suggest capsular augmentation to bolster the repair. We present a technique of capsular augmentation using a bioinductive collagen implant (Smith & Nephew) to improve the capsular integrity following repair and plication. The benefits of this implant are easy delivery through standard arthroscopic portals and secure fixation to the capsular tissue. These implants have a proven track record in the shoulder and serve as a scaffold for improved tissue quality, and their application in hip arthroscopy has potential by increasing the integrity of the capsular repair. Future studies are needed to address the clinical outcomes of this technique.

Arthroscopic Knotless Peripheral Ulnar-Sided TFCC Repair.

Project description:This article describes the indications and technique for all-arthroscopic knotless repair of a peripheral tear to the triangular fibrocartilage complex (TFCC). The advantage of this technique is that it allows repair of the articular disk back to the fovea of the ulna without any suture knots to irritate the patient. The learning curve is steep, but once learned, this technique can be performed very quickly and is faster. There are no knots to irritate the patient, and in the author's opinion, there is quicker pain relief than with other techniques.

A Systematic Approach to Arthroscopic Femoroplasty With Conservative Management of the Hip Capsule.

Project description:As hip arthroscopy has become increasingly used to treat femoroacetabular impingement, the importance of a complete femoroplasty to properly address cam impingement has been demonstrated. In doing so, different capsulotomy techniques have been described for gaining access to the hip joint as well as the peripheral compartment for cam resection. The periportal capsulotomy technique allows joint access while preserving the structural integrity of the iliofemoral ligament, obviating the need for capsular closure. We present a systematic approach and surgical technique for performing a complete arthroscopic femoroplasty while maintaining conservative hip capsule management through a periportal capsulotomy.
