Project description:The objective of this study was to investigate the use of novel surveillance tools in a malaria endemic region where prevalence information is limited. Specifically, online reporting for participatory epidemiology was used to gather information about malaria spread directly from the public. Individuals in India were incentivized to self-report their recent experience with malaria by micro-monetary payments.Self-reports about malaria diagnosis status and related information were solicited online via Amazon's Mechanical Turk. Responders were paid $0.02 to answer survey questions regarding their recent experience with malaria. Timing of the peak volume of weekly self-reported malaria diagnosis in 2010 was compared to other available metrics such as the volume over time of and information about the epidemic from media sources. Distribution of Plasmodium species reports were compared with values from the literature. The study was conducted in summer 2010 during a malaria outbreak in Mumbai and expanded to other cities during summer 2011, and prevalence from self-reports in 2010 and 2011 was contrasted.Distribution of Plasmodium species diagnosis through self-report in 2010 revealed 59% for Plasmodium vivax, which is comparable to literature reports of the burden of P. vivax in India (between 50 and 69%). Self-reported Plasmodium falciparum diagnosis was 19% and during the 2010 outbreak and the estimated burden was between 10 and 15%. Prevalence between 2010 and 2011 via self-reports decreased significantly from 36.9% to 19.54% in Mumbai (p = 0.001), and official reports also confirmed a prevalence decrease in 2011.With careful study design, micro-monetary incentives and online reporting are a rapid way to solicit malaria, and potentially other public health information. This methodology provides a cost-effective way of executing a field study that can act as a complement to traditional public health surveillance methods, offering an opportunity to obtain information about malaria activity, temporal progression, demographics affected or Plasmodium-specific diagnosis at a finer resolution than official reports can provide. The recent adoption of technologies, such as the Internet supports self-reporting mediums, and self-reporting should continue to be studied as it can foster preventative health behaviours.

Project description:The shift towards urban living is changing food demand. Past studies on India show significant urban-rural differences in food consumption. However, a scientific understanding of the underlying relationships between urbanization and food consumption is limited. This study provides the first detailed analysis of how urbanization influences both quantity and diversity of food consumption in India by harnessing the strength of multiple datasets, including consumer expenditure surveys, satellite imagery, and census data. Our statistical analysis shows three main findings. First, in contrast to existing studies, we find that much of the variation in food consumption quantity is due to income and not urbanization. After controlling for income and state-level differences, our results show that average consumption is higher in urban than rural areas for fewer than 10% of all commodities. That is, there is nearly no difference in average consumption between urban and rural residents. Second, we find the influence of urbanization as a population share on food consumption diversity to be statistically insignificant (p-value > 0.1). Instead, the results show that infrastructure, market access, percentage working women in urban areas, and norms and institutions have a statistically significant influence. Third, all covariates of food consumption diversity we tested were found to be associated with urbanization. This suggests that urbanization influences on food consumption are both indirect and multidimensional. These results show that increases in the urban population size alone do not explain changes in food consumption in India. If we are to understand how food consumption may change in the future due to urbanization, the study points to the need for a more complex and multidimensional understanding of the urbanization process that goes beyond demographic shifts.

Project description:India is the third-largest contributor to global energy-use and anthropogenic carbon emissions. India's urban energy transitions are critical to meet its climate goals due to the country's rapid urbanization. However, no baseline urban energy-use dataset covers all Indian urban districts in ways that align with national totals and integrate social-economic-infrastructural attributes to inform such transitions. This paper develops a novel bottom-up plus top-down approach, comprehensively integrating multiple field surveys and utilizing machine learning, to model All Urban areas' Energy-use (AllUrE) across all 640 districts in India, merged with social-economic-infrastructural data. Energy use estimates in this AllUrE-India dataset are evaluated by comparing with reported energy-use at three scales: nation-wide, state-wide, and city-level. Spatially granular AllUrE data aggregated nationally show good agreement with national totals (<2% difference). The goodness-of-fit ranged from 0.78-0.95 for comparison with state-level totals, and 0.90-0.99 with city-level data for different sectors. The relatively strong alignment at all three spatial scales demonstrates the value of AllUrE-India data for modelling urban energy transitions consistent with national energy and climate goals.

Project description:Coastal areas are urbanizing at unprecedented rates, particularly in low- and middle-income countries. Combinations of long-standing and emerging problems in these urban areas generate vulnerability for human well-being and ecosystems alike. This baseline study provides a spatially explicit global systematization of these problems into typical urban vulnerability profiles for the year 2000 using largely sub-national data. Using 11 indicator datasets for urban expansion, urban population growth, marginalization of poor populations, government effectiveness, exposures and damages to climate-related extreme events, low-lying settlement, and wetlands prevalence, a cluster analysis reveals a global typology of seven clearly distinguishable clusters, or urban profiles of vulnerability. Each profile is characterized by a specific data-value combination of indicators representing mechanisms that generate vulnerability. Using 21 studies for testing the plausibility, we identify seven key profile-based vulnerabilities for urban populations, which are relevant in the context of global urbanization, expansion, and climate change. We show which urban coasts are similar in this regard. Sensitivity and exposure to extreme climate-related events, and government effectiveness, are the most important factors for the huge asymmetries of vulnerability between profiles. Against the background of underlying global trends we propose entry points for profile-based vulnerability reduction. The study provides a baseline for further pattern analysis in the rapidly urbanizing coastal fringe as data availability increases. We propose to explore socio-ecologically similar coastal urban areas as a basis for sharing experience and vulnerability-reducing measures among them.

Project description:PurposeTo determine the prevalence, causes and associated demographic factors related to visual impairment amongst the urban population of New Delhi, India.MethodsA population-based, cross-sectional study was conducted in East Delhi district using cluster random sampling methodology. This Rapid Assessment of Visual Impairment (RAVI) survey involved examination of all individuals aged 40 years and above in 24 randomly selected clusters of the district. Visual acuity (VA) assessment and comprehensive ocular examination were done during the door-to-door survey. A questionnaire was used to collect personal and demographic information of the study population. Blindness and Visual Impairment was defined as presenting VA < 3/60 and < 6/18 in the better eye, respectively. Descriptive statistics were computed along with multivariable logistic regression analysis to determine associated factors for visual impairment.ResultsOf 2421 subjects enumerated, 2331 (96.3%) were available for ophthalmic examination. Among those examined, 49.3% were males. The prevalence of visual impairment (VI) in the study population, was 11.4% (95% C.I. 10.1, 12.7) and that of blindness was 1.2% (95% C.I. 0.8, 1.6). Uncorrected refractive error was the leading cause of VI accounting for 53.4% of all VI followed by cataract (33.8%). With multivariable logistic regression, the odds of having VI increased with age (OR = 24.6[95% C.I.: 14.9, 40.7]; p < 0.001). Illiterate participants were more likely to have VI [OR = 1.5 (95% C.I.: 1.1,2.1)] when compared to educated participants.ConclusionsThe first implementation of the RAVI methodology in a North Indian population revealed that the burden of visual impairment is considerable in this region despite availability of adequate eye care facilities. Awareness generation and simple interventions like cataract surgery and provision of spectacles will help to eliminate the major causes of blindness and visual impairment in this region.

Project description:We study the scaling of (i) numbers of workers and aggregate incomes by occupational categories against city size, and (ii) total incomes against numbers of workers in different occupations, across the functional metropolitan areas of Australia and the USA. The number of workers and aggregate incomes in specific high-income knowledge economy-related occupations and industries show increasing returns to scale by city size, showing that localization economies within particular industries account for superlinear effects. However, when total urban area incomes and/or gross domestic products are regressed using a generalized Cobb-Douglas function against the number of workers in different occupations as labour inputs, constant returns to scale in productivity against city size are observed. This implies that the urbanization economies at the whole city level show linear scaling or constant returns to scale. Furthermore, industrial and occupational organizations, not population size, largely explain the observed productivity variable. The results show that some very specific industries and occupations contribute to the observed overall superlinearity. The findings suggest that it is not just size but also that it is the diversity of specific intra-city organization of economic and social activity and physical infrastructure that should be used to understand urban scaling behaviours.

Project description:BackgroundWith increasing urbanization in India, child growth among urban poor has emerged as a paramount public health concern amidst the continuously growing slum population and deteriorating quality of life. This study analyses child undernutrition among urban poor and non-poor and decomposes the contribution of various factors influencing socio-economic inequality. This paper uses data from two recent rounds of National Family Health Survey (NFHS-3&4) conducted during 2005-06 and 2015-16.MethodsThe concentration index (CI) and the concentration curve (CC) measure socio-economic inequality in child growth in terms of stunting, wasting, and underweight. Wagstaff decomposition further analyses key contributors in CI by segregating significant covariates into five groups-mother's factor, health-seeking factors, environmental factors, child factors, and socio-economic factors.ResultsThe prevalence of child undernutrition was more pronounced among children from poor socio-economic strata. The concentration index decreased for stunting (- 0.186 to - 0.156), underweight (- 0.213 to - 0.162) and wasting (- 0.116 to - 0.045) from 2005 to 06 to 2015-16 respectively. The steepness in growth was more among urban poor than among urban non-poor in every age interval. Maternal education contributed about 19%, 29%, and 33% to the inequality in stunting, underweight and wasting, respectively during 2005-06. During 2005-06 as well as 2015-16, maternal factors (specifically mother's education) were the highest contributory factors in explaining rich-poor inequality in stunting as well as underweight. More than 85% of the economic inequality in stunting, underweight, and wasting among urban children were explained by maternal factors, environmental factors, and health-seeking factors.ConclusionAll the nutrition-specific and nutrition-sensitive interventions in urban areas should be prioritized, focusing on urban poor, who are often clustered in low-income slums. Rich-poor inequality in child growth calls out for integration and convergence of nutrition interventions with policy interventions aimed at poverty reduction. There is also a need to expand the scope of the Integrated Child Development Services (ICDS) program to provide mass education regarding nutrition and health by making provisions of home visits of workers primarily focusing on pregnant and lactating mothers.

Project description:Anthropogenic pollution plays an important part in deteriorating the water quality of rivers all over the world, especially in urban areas of Africa where water quality monitoring is still seriously constrained by the limited test facility and capability. In this study, for evaluating the impact of urbanization on the river water quality, we investigated four typical urban rivers of Tanzania through the upper-urban-down gradient assessment approach and analyzed by water quality index (WQI) and statistical methods. The physicochemical indices monitored in these rivers revealed that the contents of those indicators of TN, TP, PO43-, NH4+, CODMn, and NO3- were accumulated significantly in the lower reaches of the cities, which indicated the life-type pollution characteristics in such urban rivers of Africa. The following main conclusions are achieved from this study. The water quality of 30% of the investigated river sections is in the medium to good status based on the subjective WQI with sensory factors included. Moreover, the sections with obvious water quality decline are mainly limited to the river segments within the urban central area, and severe pollution of water bodies is closely related to large cities, indicating an increasing pollution tendency with the quickly growing population. Therefore, to help formulate water pollution control policies in response to the rapid urban expansion in African countries, it is necessary to adopt an economical and feasible method to carry out early monitoring of surface water quality timely.

Project description:Global increases in temperatures and urbanization are impacting the epidemiology of mosquito-borne diseases. Urbanization processes create suitable habitats for vector mosquitoes in which there are a reduced number of predators, and human hosts are widely available. We hypothesize that mosquito vector species, especially Aedes aegypti, are locally concentrated primarily in those specific habitats at the neighborhood levels that provide suitable conditions and environmental resources needed for mosquito survival. Determining how mosquito vector species composition and abundance depend on environmental resources across habitats addresses where different types of vector control need to be applied. Therefore, our goal was to analyze and identify the most productive aquatic habitats for mosquitoes in Miami-Dade County, Florida. Immature mosquito surveys were conducted throughout Miami-Dade County from April 2018 to June 2019, totaling 2,488 inspections. Mosquitoes were collected in 76 different types of aquatic habitats scattered throughout 141 neighborhoods located in the urbanized areas of Miami-Dade County. A total of 44,599 immature mosquitoes were collected and Ae. aegypti was the most common and abundant species, comprising 43% of all specimens collected. Aedes aegypti was primarily found in buckets, bromeliads, and flower pots, concentrated in specific neighborhoods. Our results showed that aquatic habitats created by anthropogenic land-use modifications (e.g., ornamental bromeliads, buckets, etc.) were positively correlated with the abundance of Ae. aegypti. This study serves to identify how vector mosquitoes utilize the resources available in urban environments and to determine the exact role of these specific urban features in supporting populations of vector mosquito species. Ultimately, the identification of modifiable urban features will allow the development of targeted mosquito control strategies optimized to preventatively control vector mosquitoes in urban areas.
