Plasma metabolic fingerprints for large-scale screening and personalized risk stratification of metabolic syndrome.

ABSTRACT: Direct diagnosis and accurate assessment of metabolic syndrome (MetS) allow for prompt clinical interventions. However, traditional diagnostic strategies overlook the complex heterogeneity of MetS. Here, we perform metabolomic analysis in 13,554 participants from the natural cohort and identify 26 hub plasma metabolic fingerprints (PMFs) associated with MetS and its early identification (pre-MetS). By leveraging machine-learning algorithms, we develop robust diagnostic models for pre-MetS and MetS with convincing performance through independent validation. We utilize these PMFs to assess the relative contributions of the four major MetS risk factors in the general population, ranked as follows: hyperglycemia, hypertension, dyslipidemia, and obesity. Furthermore, we devise a personalized three-dimensional plasma metabolic risk (PMR) stratification, revealing three distinct risk patterns. In summary, our study offers effective screening tools for identifying pre-MetS and MetS patients in the general community, while defining the heterogeneous risk stratification of metabolic phenotypes in real-world settings.

Project description:BackgroundThe Breast and Ovarian Analysis of Disease Incidence and Carrier Estimation Algorithm (BOADICEA) incorporates the effects of common genetic variants, from polygenic risk scores, pathogenic variants in major breast cancer (BC) susceptibility genes, lifestyle/hormonal risk factors, mammographic density, and cancer family history to predict risk levels of developing breast and ovarian cancer. While offering multifactorial risk assessment to the population could be a promising avenue for early detection of BC, obstacles to its implementation including fear of genetic discrimination (GD), could prevent individuals from undergoing screening.MethodsThe aim of our study was two-fold: determine the extent of legal protection in Canada available to protect information generated by risk prediction models such as the BOADICEA algorithm through a literature review, and then, assess individuals' knowledge of and concerns about GD in this context by collecting data through surveys.ResultsOur legal analysis highlighted that while Canadian employment and privacy laws provide a good level of protection against GD, it remains uncertain whether the Genetic Non-Discrimination Act (GNDA) would provide protection for BC risk levels generated by a risk prediction model. The survey results of 3,055 participants who consented to risk assessment in the PERSPECTIVE I&I project showed divergent perspectives of how the law would protect BC risk level in the context of employment and that a high number of participants did not feel that their risk level was protected from access and use by life insurers. Indeed, 49,1% of participants reckon that the level of breast cancer risk could have an impact on a woman's ability to buy insurance and 58,9% of participants reckon that a woman's insurance might be cancelled if important health information (including level of breast cancer risk) is not given when buying or renewing life or health insurance.ConclusionThe results indicate that much work needs to be done to improve and clarify the extent of protection against GD in Canada and to inform the population of how the legal framework applies to risk levels generated by risk prediction models.

Project description:BackgroundLittle is known about the relationship between the metabolite profile of plasma from pre-operative prostate cancer (PCa) patients and the risk of PCa progression. In this study we investigated the association between pre-operative plasma metabolites and risk of biochemical-, local- and metastatic-recurrence, with the aim of improving patient stratification.MethodsWe conducted a case-control study within a cohort of PCa patients recruited between 1996 and 2015. The age-matched primary cases (n = 33) were stratified in low risk, high risk without progression and high risk with progression as defined by the National Comprehensive Cancer Network. These samples were compared to metastatic (n = 9) and healthy controls (n = 10). The pre-operative plasma from primary cases and the plasma from metastatic patients and controls were assessed with untargeted metabolomics by LC-MS. The association between risk of progression and metabolite abundance was calculated using multivariate Cox proportional-hazard regression and the relationship between metabolites and outcome was calculated using median cut-off normalized values of metabolite abundance by Log-Rank test using the Kaplan Meier method.ResultsMedium-chain acylcarnitines (C6-C12) were positively associated with the risk of PSA progression (p = 0.036, median cut-off) while long-chain acylcarnitines (C14-C16) were inversely associated with local (p = 0.034) and bone progression (p = 0.0033). In primary cases, medium-chain acylcarnitines were positively associated with suberic acid, which also correlated with the risk of PSA progression (p = 0.032, Log-Rank test). In the metastatic samples, this effect was consistent for hexanoylcarnitine, L.octanoylcarnitine and decanoylcarnitine. Medium-chain acylcarnitines and suberic acid displayed the same inverse association with tryptophan, while indoleacetic acid, a breakdown product of tryptophan metabolism was strongly associated with PSA (p = 0.0081, Log-Rank test) and lymph node progression (p = 0.025, Log-Rank test). These data were consistent with the increased expression of indoleamine 2,3 dioxygenase (IDO1) in metastatic versus primary samples (p = 0.014). Finally, functional experiments revealed a synergistic effect of long chain fatty acids in combination with dihydrotestosterone administration on the transcription of androgen responsive genes.ConclusionsThis study strengthens the emerging link between fatty acid metabolism and PCa progression and suggests that measuring levels of medium- and long-chain acylcarnitines in pre-operative patient plasma may provide a basis for improving patient stratification.
