SPRC19: A Database of State Policy Responses to COVID-19 in the United States.

ABSTRACT: SPRC19 is a new database that seeks to capture a wide range of state policy actions in response to COVID-19 in the United States. Since March 2020 we have monitored state governments' and multi-state associations' websites for executive orders, agency rules, new legislation, and court decisions. We categorize each policy action into one of 206 distinct policies, then document the branch of government, source document, announcement date, implementation date, and expiration date (if applicable). We also record whether the action represents the introduction of a new policy or the expansion or contraction of an existing policy. The current release of SPRC19, v3.0, captures over 13,000 distinct policy actions through April 2020, which constitutes thousands more actions than similar resources over the same time period.

Higher education responses to COVID-19 in the United States: Evidence for the impacts of university policy.

Project description:With a dataset of testing and case counts from over 1,400 institutions of higher education (IHEs) in the United States, we analyze the number of infections and deaths from SARS-CoV-2 in the counties surrounding these IHEs during the Fall 2020 semester (August to December, 2020). We find that counties with IHEs that remained primarily online experienced fewer cases and deaths during the Fall 2020 semester; whereas before and after the semester, these two groups had almost identical COVID-19 incidence. Additionally, we see fewer cases and deaths in counties with IHEs that reported conducting any on-campus testing compared to those that reported none. To perform these two comparisons, we used a matching procedure designed to create well-balanced groups of counties that are aligned as much as possible along age, race, income, population, and urban/rural categories-demographic variables that have been shown to be correlated with COVID-19 outcomes. We conclude with a case study of IHEs in Massachusetts-a state with especially high detail in our dataset-which further highlights the importance of IHE-affiliated testing for the broader community. The results in this work suggest that campus testing can itself be thought of as a mitigation policy and that allocating additional resources to IHEs to support efforts to regularly test students and staff would be beneficial to mitigating the spread of COVID-19 in a pre-vaccine environment.

State-level tracking of COVID-19 in the United States.

Project description:As of 1st June 2020, the US Centres for Disease Control and Prevention reported 104,232 confirmed or probable COVID-19-related deaths in the US. This was more than twice the number of deaths reported in the next most severely impacted country. We jointly model the US epidemic at the state-level, using publicly available death data within a Bayesian hierarchical semi-mechanistic framework. For each state, we estimate the number of individuals that have been infected, the number of individuals that are currently infectious and the time-varying reproduction number (the average number of secondary infections caused by an infected person). We use changes in mobility to capture the impact that non-pharmaceutical interventions and other behaviour changes have on the rate of transmission of SARS-CoV-2. We estimate that Rt was only below one in 23 states on 1st June. We also estimate that 3.7% [3.4%-4.0%] of the total population of the US had been infected, with wide variation between states, and approximately 0.01% of the population was infectious. We demonstrate good 3 week model forecasts of deaths with low error and good coverage of our credible intervals.

State variation in neighborhood COVID-19 burden across the United States.

Project description:BackgroundA lack of fine, spatially-resolute case data for the U.S. has prevented the examination of how COVID-19 infection burden has been distributed across neighborhoods, a key determinant of both risk and resilience. Without more spatially resolute data, efforts to identify and mitigate the long-term fallout from COVID-19 in vulnerable communities will remain difficult to quantify and intervene on.MethodsWe leveraged spatially-referenced data from 21 states collated through the COVID Neighborhood Project to examine the distribution of COVID-19 cases across neighborhoods and states in the U.S. We also linked the COVID-19 case data with data on the neighborhood social environment from the National Neighborhood Data Archive. We then estimated correlations between neighborhood COVID-19 burden and features of the neighborhood social environment.ResultsWe find that the distribution of COVID-19 at the neighborhood-level varies within and between states. The median case count per neighborhood (coefficient of variation (CV)) in Wisconsin is 3078.52 (0.17) per 10,000 population, indicating a more homogenous distribution of COVID-19 burden, whereas in Vermont the median case count per neighborhood (CV) is 810.98 (0.84) per 10,000 population. We also find that correlations between features of the neighborhood social environment and burden vary in magnitude and direction by state.ConclusionsOur findings underscore the importance that local contexts may play when addressing the long-term social and economic fallout communities will face from COVID-19.

State-level variation of initial COVID-19 dynamics in the United States.

Project description:During an epidemic, metrics such as R0, doubling time, and case fatality rates are important in understanding and predicting the course of an epidemic. However, if collected over country or regional scales, these metrics hide important smaller-scale, local dynamics. We examine how commonly used epidemiological metrics differ for each individual state within the United States during the initial COVID-19 outbreak. We found that the detected case number and trajectory of early detected cases differ considerably between states. We then test for correlations with testing protocols, interventions and population characteristics. We find that epidemic dynamics were most strongly associated with non-pharmaceutical government actions during the early phase of the epidemic. In particular, early social distancing restrictions, particularly on restaurant operations, was correlated with increased doubling times. Interestingly, we also found that states with little tolerance for deviance from enforced rules saw faster early epidemic growth. Together with other correlates such as population density, our results highlight the different factors involved in the heterogeneity in the early spread of COVID-19 throughout the United States. Although individual states are clearly not independent, they can serve as small, natural experiments in how different demographic patterns and government responses can impact the course of an epidemic.

The imperative of state capacity in public health crisis: Asia's early COVID-19 policy responses.

Project description:Preexisting political institutions influence governments' responses to public health crises in different ways, creating national variations. This article investigates how state capacity, a country's fundamental ability to organize bureaucracy and allocate societal resources, affects the timing and configuration of governments' COVID-19 policy responses. Through comparative case study analysis of five of China's neighboring countries early in the COVID-19 crisis, the paper shows that more-capable states (Singapore, South Korea, Taiwan) initiated crisis response faster, mobilized national resources more extensively, and utilized diverse policy tools when the virus risk level was still low. In contrast, low-capacity states (Thailand and Indonesia) were more reactive in handling the crisis, limited their focus to border-related measures, and were more constrained in the types of tools they could employ. The paper points to the importance of studying the COVID-19 response process rather than the outcome (i.e., confirmed cases/deaths) when unpacking the impacts of political institutions in public health crises.

Bayesian Inference of State-Level COVID-19 Basic Reproduction Numbers across the United States.

Project description:Although many persons in the United States have acquired immunity to COVID-19, either through vaccination or infection with SARS-CoV-2, COVID-19 will pose an ongoing threat to non-immune persons so long as disease transmission continues. We can estimate when sustained disease transmission will end in a population by calculating the population-specific basic reproduction number ℛ0, the expected number of secondary cases generated by an infected person in the absence of any interventions. The value of ℛ0 relates to a herd immunity threshold (HIT), which is given by 1-1/ℛ0. When the immune fraction of a population exceeds this threshold, sustained disease transmission becomes exponentially unlikely (barring mutations allowing SARS-CoV-2 to escape immunity). Here, we report state-level ℛ0 estimates obtained using Bayesian inference. Maximum a posteriori estimates range from 7.1 for New Jersey to 2.3 for Wyoming, indicating that disease transmission varies considerably across states and that reaching herd immunity will be more difficult in some states than others. ℛ0 estimates were obtained from compartmental models via the next-generation matrix approach after each model was parameterized using regional daily confirmed case reports of COVID-19 from 21 January 2020 to 21 June 2020. Our ℛ0 estimates characterize the infectiousness of ancestral strains, but they can be used to determine HITs for a distinct, currently dominant circulating strain, such as SARS-CoV-2 variant Delta (lineage B.1.617.2), if the relative infectiousness of the strain can be ascertained. On the basis of Delta-adjusted HITs, vaccination data, and seroprevalence survey data, we found that no state had achieved herd immunity as of 20 September 2021.

Driving Sustainability in the United States: Efficacy of State-Level Biodiesel Policy Approaches.

Project description:The United States has significant greenhouse gas and criteria pollutant emissions that lead to global warming, human health, ozone, and smog issues, partially attributed to its diesel-consuming transport fleet. Until fleet electrification reaches cost parity with internal combustion engines, biodiesel use can reduce these negative impacts. In this study, we analyzed and categorized the biodiesel-supporting policies of each U.S. state using manual inductive coding to compare them against state-level biodiesel consumption and production. Through statistical modeling, we determined the efficacy of these policy approaches. The policy analysis identified that biodiesel policies that support infrastructure development and biodiesel production correlate significantly with increased biodiesel consumption at the state level. We also show that a combination of these policy categories correlates significantly with overall higher biodiesel consumption. Our methodological approach and policy analysis findings reveal valuable insight into the efficacy and outcomes from existing biofuel policies in the United States.

United but divided: Policy responses and people's perceptions in the EU during the COVID-19 outbreak.

Project description:To understand the public sentiment toward the measures used by policymakers for COVID-19 containment, a survey among representative samples of the population in seven European countries was carried out in the first two weeks of April 2020. The study addressed people's support for containment policies, worries about COVID-19 consequences, and trust in sources of information. Citizens were overall satisfied with their government's response to the pandemic; however, the extent of approval differed across countries and policy measures. A north-south divide in public opinion was noticeable across the European states. It was particularly pronounced for intrusive policy measures, such as mobile data use for movement tracking, economic concerns, and trust in the information from the national government. Considerable differences in people's attitudes were noticed within countries, especially across individual regions and age groups. The findings suggest that the epidemic acts as a stressor, causing health and economic anxieties even in households that were not directly affected by the virus. At the same time, the burden of stress was unequally distributed across regions and age groups. Based on the data collected, we draw lessons from the containment stage and identify several insights that can facilitate the design of lockdown exit strategies and future containment policies so that a high level of compliance can be expected.

Correlations Between COVID-19 Cases and Google Trends Data in the United States: A State-by-State Analysis.

Project description:ObjectiveTo evaluate whether a digital surveillance model using Google Trends is feasible for obtaining accurate data on coronavirus disease 2019 and whether accurate predictions can be made regarding new cases.MethodsData on total and daily new cases in each US state were collected from January 22, 2020, to April 6, 2020. Information regarding 10 keywords was collected from Google Trends, and correlation analyses were performed for individual states as well as for the United States overall.ResultsAmong the 10 keywords analyzed from Google Trends, face mask, Lysol, and COVID stimulus check had the strongest correlations when looking at the United States as a whole, with R values of 0.88, 0.82, and 0.79, respectively. Lag and lead Pearson correlations were assessed for every state and all 10 keywords from 16 days before the first case in each state to 16 days after the first case. Strong correlations were seen up to 16 days prior to the first reported cases in some states.ConclusionThis study documents the feasibility of syndromic surveillance of internet search terms to monitor new infectious diseases such as coronavirus disease 2019. This information could enable better preparation and planning of health care systems.

State-level impact of social distancing and testing on COVID-19 in the United States.

Project description:Social distancing measures have been implemented in the United States (US) since March 2020, to mitigate the spread of SARS-CoV-2, the causative agent of COVID-19. However, by mid-May most states began relaxing these measures to support the resumption of economic activity, even as disease incidence continued to increase in many states. To evaluate the impact of relaxing social distancing restrictions on COVID-19 dynamics and control in the US, we developed a transmission dynamic model and calibrated it to US state-level COVID-19 cases and deaths from March to June 20th, 2020, using Bayesian methods. We used this model to evaluate the impact of reopening, social distancing, testing, contact tracing, and case isolation on the COVID-19 epidemic in each state. We found that using stay-at-home orders, most states were able to curtail their COVID-19 epidemic curve by reducing and achieving an effective reproductive number below 1. But by June 20th, 2020, only 19 states and the District of Columbia were on track to curtail their epidemic curve with a 75% confidence, at current levels of reopening. Of the remaining 31 states, 24 may have to double their current testing and/or contact tracing rate to curtail their epidemic curve, and seven need to further restrict social contact by 25% in addition to doubling their testing and contact tracing rates. When social distancing restrictions are being eased, greater state-level testing and contact tracing capacity remains paramount for mitigating the risk of large-scale increases in cases and deaths.
