Estimating the Efficiency Gain of Covariate-Adjusted Analyses in Future Clinical Trials Using External Data.

ABSTRACT: We present a framework for using existing external data to identify and estimate the relative efficiency of a covariate-adjusted estimator compared to an unadjusted estimator in a future randomized trial. Under conditions, these relative efficiencies approximate the ratio of sample sizes needed to achieve a desired power. We develop semiparametrically efficient estimators of the relative efficiencies for several treatment effect estimands of interest with either fully or partially observed outcomes, allowing for the application of flexible statistical learning tools to estimate the nuisance functions. We propose an analytic Wald-type confidence interval and a double bootstrap scheme for statistical inference. We demonstrate the performance of the proposed methods through simulation studies and apply these methods to estimate the efficiency gain of covariate adjustment in Covid-19 therapeutic trials.

Estimating covariate-adjusted measures of diagnostic accuracy based on pooled biomarker assessments.

Project description:There is a need for epidemiological and medical researchers to identify new biomarkers (biological markers) that are useful in determining exposure levels and/or for the purposes of disease detection. Often this process is stunted by high testing costs associated with evaluating new biomarkers. Traditionally, biomarker assessments are individually tested within a target population. Pooling has been proposed to help alleviate the testing costs, where pools are formed by combining several individual specimens. Methods for using pooled biomarker assessments to estimate discriminatory ability have been developed. However, all these procedures have failed to acknowledge confounding factors. In this paper, we propose a regression methodology based on pooled biomarker measurements that allow the assessment of the discriminatory ability of a biomarker of interest. In particular, we develop covariate-adjusted estimators of the receiver-operating characteristic curve, the area under the curve, and Youden's index. We establish the asymptotic properties of these estimators and develop inferential techniques that allow one to assess whether a biomarker is a good discriminator between cases and controls, while controlling for confounders. The finite sample performance of the proposed methodology is illustrated through simulation. We apply our methods to analyze myocardial infarction (MI) data, with the goal of determining whether the pro-inflammatory cytokine interleukin-6 is a good predictor of MI after controlling for the subjects' cholesterol levels.

Variable selection for covariate-adjusted semiparametric inference in randomized clinical trials.

Project description:Extensive baseline covariate information is routinely collected on participants in randomized clinical trials, and it is well recognized that a proper covariate-adjusted analysis can improve the efficiency of inference on the treatment effect. However, such covariate adjustment has engendered considerable controversy, as post hoc selection of covariates may involve subjectivity and may lead to biased inference, whereas prior specification of the adjustment may exclude important variables from consideration. Accordingly, how to select covariates objectively to gain maximal efficiency is of broad interest. We propose and study the use of modern variable selection methods for this purpose in the context of a semiparametric framework, under which variable selection in modeling the relationship between outcome and covariates is separated from estimation of the treatment effect, circumventing the potential for selection bias associated with standard analysis of covariance methods. We demonstrate that such objective variable selection techniques combined with this framework can identify key variables and lead to unbiased and efficient inference on the treatment effect. A critical issue in finite samples is validity of estimators of uncertainty, such as standard errors and confidence intervals for the treatment effect. We propose an approach to estimation of sampling variation of estimated treatment effect and show its superior performance relative to that of existing methods.

Covariate adjusted classification trees.

Project description:In studies that compare several diagnostic groups, subjects can be measured on certain features and classification trees can be used to identify which of them best characterize the differences among groups. However, subjects may also be measured on additional covariates whose ability to characterize group differences is not meaningful or of interest, but may still have an impact on the examined features. Therefore, it is important to adjust for the effects of covariates on these features. We present a new semi-parametric approach to adjust for covariate effects when constructing classification trees based on the features of interest that is readily implementable. An application is given for postmortem brain tissue data to compare the neurobiological characteristics of subjects with schizophrenia to those of normal controls. We also evaluate the performance of our approach using a simulation study.

Unbiased causal inference with Mendelian randomization and covariate-adjusted GWAS data.

Project description:Mendelian randomization (MR) facilitates causal inference with observational data using publicly available genome-wide association study (GWAS) results. In a GWAS, one or more heritable covariates may be adjusted for to estimate the direct effects of SNPs on a focal trait or to improve statistical power, which may introduce collider bias in SNP-trait association estimates, thus affecting downstream MR analyses. Numerical studies suggested that using covariate-adjusted GWAS summary data might introduce bias in univariable Mendelian randomization (UVMR), which can be mitigated by multivariable Mendelian randomization (MVMR). However, it remains unclear and even mysterious why/how MVMR works; a rigorous theory is needed to explain and substantiate the above empirical observation. In this paper, we derive some analytical results when multiple covariates are adjusted for in the GWAS of exposure and/or the GWAS of outcome, thus supporting and explaining the empirical results. Our analytical results offer insights to how bias arises in UVMR and how it is avoided in MVMR, regardless of whether collider bias is present. We also consider applying UVMR or MVMR methods after collider-bias correction. We conducted extensive simulations to demonstrate that with covariate-adjusted GWAS summary data, MVMR had an advantage over UVMR by producing nearly unbiased causal estimates; however, in some situations it is advantageous to apply UVMR after bias correction. In real data analyses of the GWAS data with body mass index (BMI) being adjusted for metabolomic principal components, we examined the causal effect of BMI on blood pressure, confirming the above points.

Covariate-adjusted hybrid principal components analysis for region-referenced functional EEG data.

Project description:Electroencephalography (EEG) studies produce region-referenced functional data via EEG signals recorded across scalp electrodes. The high-dimensional data can be used to contrast neurodevelopmental trajectories between diagnostic groups, for example between typically developing (TD) children and children with autism spectrum disorder (ASD). Valid inference requires characterization of the complex EEG dependency structure as well as covariate-dependent heteroscedasticity, such as changes in variation over developmental age. In our motivating study, EEG data is collected on TD and ASD children aged two to twelve years old. The peak alpha frequency, a prominent peak in the alpha spectrum, is a biomarker linked to neurodevelopment that shifts as children age. To retain information, we model patterns of alpha spectral variation, rather than just the peak location, regionally across the scalp and chronologically across development. We propose a covariate-adjusted hybrid principal components analysis (CA-HPCA) for EEG data, which utilizes both vector and functional principal components analysis while simultaneously adjusting for covariate-dependent heteroscedasticity. CA-HPCA assumes the covariance process is weakly separable conditional on observed covariates, allowing for covariate-adjustments to be made on the marginal covariances rather than the full covariance leading to stable and computationally efficient estimation. The proposed methodology provides novel insights into neurodevelopmental differences between TD and ASD children.

The use of external control data for predictions and futility interim analyses in clinical trials.

Project description:BackgroundExternal control (EC) data from completed clinical trials and electronic health records can be valuable for the design and analysis of future clinical trials. We discuss the use of EC data for early stopping decisions in randomized clinical trials (RCTs).MethodsWe specify interim analyses (IAs) approaches for RCTs, which allow investigators to integrate external data into early futility stopping decisions. IAs utilize predictions based on early data from the RCT, possibly combined with external data. These predictions at IAs express the probability that the trial will generate significant evidence of positive treatment effects. The trial is discontinued if this predictive probability becomes smaller than a prespecified threshold. We quantify efficiency gains and risks associated with the integration of external data into interim decisions. We then analyze a collection of glioblastoma (GBM) data sets, to investigate if the balance of efficiency gains and risks justify the integration of external data into the IAs of future GBM RCTs.ResultsOur analyses illustrate the importance of accounting for potential differences between the distributions of prognostic variables in the RCT and in the external data to effectively leverage external data for interim decisions. Using GBM data sets, we estimate that the integration of external data increases the probability of early stopping of ineffective experimental treatments by up to 25% compared to IAs that do not leverage external data. Additionally, we observe a reduction of the probability of early discontinuation for effective experimental treatments, which improves the RCT power.ConclusionLeveraging external data for IAs in RCTs can support early stopping decisions and reduce the number of enrolled patients when the experimental treatment is ineffective.

Robust covariate-adjusted log-rank statistics and corresponding sample size formula for recurrent events data.

Project description:Recurrent events data are frequently encountered in clinical trials. This article develops robust covariate-adjusted log-rank statistics applied to recurrent events data with arbitrary numbers of events under independent censoring and the corresponding sample size formula. The proposed log-rank tests are robust with respect to different data-generating processes and are adjusted for predictive covariates. It reduces to the Kong and Slud (1997, Biometrika 84, 847-862) setting in the case of a single event. The sample size formula is derived based on the asymptotic normality of the covariate-adjusted log-rank statistics under certain local alternatives and a working model for baseline covariates in the recurrent event data context. When the effect size is small and the baseline covariates do not contain significant information about event times, it reduces to the same form as that of Schoenfeld (1983, Biometrics 39, 499-503) for cases of a single event or independent event times within a subject. We carry out simulations to study the control of type I error and the comparison of powers between several methods in finite samples. The proposed sample size formula is illustrated using data from an rhDNase study.

MODELLING COUNTY LEVEL BREAST CANCER SURVIVAL DATA USING A COVARIATE-ADJUSTED FRAILTY PROPORTIONAL HAZARDS MODEL.

Project description:Understanding the factors that explain differences in survival times is an important issue for establishing policies to improve national health systems. Motivated by breast cancer data arising from the Surveillance Epidemiology and End Results program, we propose a covariate-adjusted proportional hazards frailty model for the analysis of clustered right-censored data. Rather than incorporating exchangeable frailties in the linear predictor of commonly-used survival models, we allow the frailty distribution to flexibly change with both continuous and categorical cluster-level covariates and model them using a dependent Bayesian nonparametric model. The resulting process is flexible and easy to fit using an existing R package. The application of the model to our motivating example showed that, contrary to intuition, those diagnosed during a period of time in the 1990s in more rural and less affluent Iowan counties survived breast cancer better. Additional analyses showed the opposite trend for earlier time windows. We conjecture that this anomaly has to be due to increased hormone replacement therapy treatments prescribed to more urban and affluent subpopulations.

On High-Dimensional Covariate Adjustment for Estimating Causal Effects in Randomized Trials with Survival Outcomes.

Project description:The purpose of this work is to improve the efficiency in estimating the average causal effect (ACE) on the survival scale where right-censoring exists and high-dimensional covariate information is available. We propose new estimators using regularized survival regression and survival Random Forest (RF) to adjust for the high-dimensional covariate to improve efficiency. We study the behavior of the adjusted estimators under mild assumptions and show theoretical guarantees that the proposed estimators are more efficient than the unadjusted ones asymptotically when using RF for the adjustment. In addition, these adjusted estimators are n - consistent and asymptotically normally distributed. The finite sample behavior of our methods is studied by simulation. The simulation results are in agreement with the theoretical results. We also illustrate our methods by analyzing the real data from transplant research to identify the relative effectiveness of identical sibling donors compared to unrelated donors with the adjustment of cytogenetic abnormalities.

Interface engineering for gain perovskite photodetectors with extremely high external quantum efficiency.

Project description:Efficient CH3NH3PbI3 photodetectors (PDs) with an extremely high gain of the maximum external quantum efficiency (EQE) of 140 000% within the ultraviolet region to the near infrared region (NIR) and an extremely high responsivity (R) under a low bias of -5 V were successfully fabricated. The fabricated devices manifested outstanding environmental stability with only 10% degradation of EQE after being exposed to air for 24 h. These obtained results indicate the promising potential of perovskite PDs for visible light communication applications.
