It is particularly appropriate for those with contraindications to advanced imaging and also may reduce the risk of incorrect diagnoses in those situations in which imaging is associated with low sensitivity/specificity. The purpose of this article is to provide a standardized diagnostic approach to needle arthroscopy of the shoulder.

Project description:Biceps tendinopathy is a common cause of chronic anterior shoulder pain characterized by altered joint mechanics with considerable deficits in range of motion secondary to pain. The benefits of in-office nano-arthroscopy (IONA) include the ability to diagnosis and treat biceps tendinopathy, quicker patient recovery, reduced cost, and improved patient satisfaction. The purpose of this technical report is to describe the technique for performing IONA for biceps tendinopathy (biceps tenotomy/biceps tenodesis), with special consideration for obtaining adequate local anesthesia, proper indications, adequate visualization, and the advantages of performing these procedures in the office rather than the operating room.

Project description:Diagnostic needle arthroscopy performed in office is a safe and cost-effective method for accurate evaluation of intra-articular pathology, improving clinical decision making. This minimally invasive approach is an effective alternative to traditional diagnostic techniques of diagnostic surgical arthroscopy and magnetic resonance imaging (MRI). Needle arthroscopy is considered low-risk, with an extremely low complication rate when performed with appropriate technique and indications. The purpose of this article is to describe an approach to an in-office diagnostic procedure using a needle arthroscopy system. Technique Video Video 1 Standard arthroscopic approach to the left knee via anterolateral and anteromedial portals. Place the patient in a supine or sitting position, with the left knee at ∼90°, and sterilely prep the left knee. Anesthetize the anterolateral and anteromedial standard portal sites with ∼15 cc of local anesthetic (1% lidocaine with epinephrine and 0.5% Marcaine without epinephrine) on a 25-gauge needle, creating a wheel. Next, insufflate the joint via the anterolateral portal using 20 cc of .25% Marcaine without epinephrine on a 20-gauge needle. After an additional re-prep of the knee with chlorohexidine, wait about 5 minutes, and then place sterile blue drapes at the proximal tibia and distal femur to create a sterile field. Introduce the 2.2-mm inflow NanoScope™ (Arthrex) sheath with the sharp trocar into the anterolateral portal, which is the primary viewing portal, and withdraw the trocar, and then attach the one-way stop valve to the sheath. Insert the 1.9-mm NanoScope™ (Arthrex) needle arthroscopy system, which is approximately one-third of the size of a standard arthroscopy camera. Connect a 30-cc syringe of sterile 0.9% normal saline to the NanoScope™ (Arthrex) via the one-way stop valve, open the valve, and use all 30 cc to insufflate the joint. This is followed by connecting another 30 cc of sterile saline to the one-way stop valve to inject as needed, while performing a standard diagnostic left knee arthroscopy to identify and assess any intraarticular pathology. In this particular patient, a medial femoral condyle defect is identified. If needed, a 2.0-mm NanoProbe (Arthrex) or other NanoSope™ (Arthrex) instrumentation can be used for both diagnostic and therapeutic treatment. This is inserted into the anteromedial portal, which is the accessory portal. If possible, have an assistant available to provide valgus stress or assist in patient knee positioning, as the arthroscopy is being performed. At the conclusion of the standard diagnostic needle arthroscopy, aspirate as much fluid off the knee as possible through the NanoSope™ (Arthrex), while it remains inserted in the anterolateral portal. Remove the scope and resolve any bleeding by applying pressure with sterile gauze. Apply Steri-Strips to the percutaneous portal sites, followed by gauze and an ACE wrap. The patient may ambulate immediately afterward. Post-procedure antibiotics may be considered

Project description:Shoulder arthroscopy is the second most common orthopaedic procedure. Diagnostic arthroscopy of the shoulder requires an efficient and reproducible technique. In this Technical Note, we describe a step-wise approach to diagnostic arthroscopy of the shoulder. This technique is performed using a posterior viewing portal. It can be performed from the beach chair or the lateral decubitus position. This technique uses a 2-circle approach: the surgeon first evaluates the glenoid aspect of the joint space, followed by the humeral aspect of the joint space. This method ensures a complete and consistent evaluation of the glenohumeral joint.

Project description:A technique to perform glenohumeral arthroscopy without using arthroscopic fluid for visualization or instrumentation is presented. The patient is placed in the beach-chair position. No traction is applied. Portal placement is performed as usual. Water flow is used only sporadically to wash the joint cavity, and then it is immediately aspirated with a synoviotome suction pump. The entire exploration and instrumentation occur within a pressure-free dry joint. We have applied this technique in more than 50 patients, and in none of these cases was there a need to switch from the dry to the wet technique. By use of this surgical technique, intra-articular shoulder pathology treatment can be performed in a more accurate biomechanical and physiological scenario. It avoids many visualization and instrumentation inconveniences, as well as the risk of some major complications. Joint distension with fluid becomes unnecessary, making it possible to use medium-sized incisions to perform and simplify complex procedures.

Project description:Critical shoulder angle (CSA) is the angle between the superior and inferior bone margins of the glenoid and the most lateral border of the acromion and is potentially affected during a rotator cuff tear (RCT). Acromioplasty is generally performed to rectify the anatomy of the acromion during RCT repair surgery. However, limited information is available regarding the changes in the CSA after anterolateral acromioplasty. We hypothesized that CSA can be decreased after anterolateral acromioplasty. Data were retrospectively collected from 712 patients with RCTs and underwent arthroscopic rotator cuff repair between January 2012 and December 2018, of which 337 patients were included in the study. The presurgical and postsurgical CSA were then determined and compared using a paired samples t test. Because previous study mentioned CSA more than 38 degrees were at risk of rotator cuff re-tear, patients were segregated into two groups: CSA < 38° and CSA ≥ 38°; these groups were compared using an independent-samples t test. These 337 participants (160 male and 177 female) presented a CSA of 38.4° ± 6.0° before anterolateral acromioplasty, which significantly decreased to 35.8° ± 5.9° after surgery (P < .05). Before surgery, 172 patients were present in the CSA ≥ 38° group and 57 were preset in the CSA < 38° group after surgery. The CSA decreased significantly in the CSA ≥ 38° group rather than in the CSA < 38° group (P < .05). In conclusion, the CSA can be effectively decreased through anterolateral acromioplasty, and this reduction in the CSA is more significant among individuals with CSA ≥ 38° than among those with CSA < 38°, indicating that acromioplasty is recommended along with RCT repair especially among individuals with a wide presurgical CSA.

Project description:BackgroundThe purpose of this analysis was to determine whether in office diagnostic needle arthroscopy (Visionscope Imaging System [VSI]) can provide for improved diagnostic assessment and; more cost effective care.MethodsData on arthroscopy procedures in the US for deep seated pathology in the knee and shoulder were used (Calendar Year 2012). These procedures represent approximately 25-30% of all arthroscopic procedures performed annually. Sensitivities, specificities, positive predictive, and negative predictive values for MRI analysis of this deep seated pathology from systematic reviews and meta-analyses were used in assessing for false positive and false negative MRI findings. The costs of performing diagnostic and surgical arthroscopy procedures (using 2013 Medicare reimbursement amounts); costs associated with false negative findings; and the costs for treating associated complications arising from diagnostic and therapeutic arthroscopy procedures were then assessed.ResultsIn patients presenting with medial meniscal pathology (ICD9CM diagnosis 836.0 over 540,000 procedures in CY 2012); use of the VSI system in place of MRI assessment (standard of care) resulted in a net cost savings to the system of $151 million. In patients presenting with rotator cuff pathology (ICD9CM 840.4 over 165,000 procedures in CY2012); use of VSI in place of MRI similarly saved $59 million. These savings were realized along with more appropriate care as; fewer patients were exposed to higher risk surgical arthroscopic procedures.ConclusionsThe use of an in-office arthroscopy system can: possibly save the US healthcare system money; shorten the diagnostic odyssey for patients; potentially better prepare clinicians for arthroscopic surgery (when needed) and; eliminate unnecessary outpatient arthroscopy procedures, which commonly result in surgical intervention.

Project description:Shoulder arthroscopy is traditionally performed with the patient in either the beach chair position or the lateral decubitus position. Each position has its advantages and disadvantages. The main topics for consideration include ease of surgery, view into the surgical field, risks to the patient, and economics of the setup. In the lateral decubitus position, it is inconvenient to work through the anterior portal and it is difficult to convert to an open procedure. In the beach chair position, it is difficult to manage the airway and cerebral oxygenation and the patient's head and the beach chair frame obstruct the insertion of a scope into the superior and posterior portals. This technical note presents the supine position for shoulder arthroscopic surgery. The supine position does not have the disadvantages of the traditional positions. In addition, it is comparatively easy to set up and comfortable for the patient.

Project description:PurposeTo evaluate the effect of acromioplasty using a cutting block technique on bony coverage as measured by the critical shoulder angle (CSA) and acromial index (AI).MethodsThis study is a retrospective radiographic review using data from a previous prospective randomized clinical trial that offered enrollment to patients aged 18 years or older with a full-thickness tear of the superior rotator cuff between October 2007 and January 2011. Each patient was allocated to repair with either acromioplasty using a cutting block technique or non-acromioplasty treatment arms in a blinded fashion. Medical and demographic information was recorded for each patient. Between January 2017 and December 2017, patients were contacted for repeat follow-up clinical evaluation and radiographs. Measurements of acromial index and critical shoulder angle were performed on pre- and postoperative radiographs by a single reviewer.ResultsSeventy-one (75%) patients were available for follow up. The 2 groups were similar in terms of baseline demographics and acromial type. When compared with preoperative measures, acromioplasty did not result in significant reductions in mean CSA (34.5° vs 35.5°; P = .293) or AI (0.68 vs 0.66; P = .283). Furthermore, postoperative CSA (34.5° vs 36.2°, P = .052) and AI (0.66 vs 0.67, P = .535) demonstrated no statistically significant differences between patients with and without acromioplasty, respectively.ConclusionsThere was no statistically significant change in either the CSA or AI following acromioplasty, nor was there a significant postoperative difference in CSA or AI between the group that underwent acromioplasty and the group that did not.Clinical relevanceSome studies suggest a greater postoperative CSA may result in greater risk of retear after arthroscopic rotator cuff repair. The CSA and AI may not be modifiable with acromioplasty.
