Effectiveness of a Preventative Program for Groin Pain Syndrome in Elite Youth Soccer Players: A Prospective, Randomized, Controlled, Single-Blind Study.

ABSTRACT: Groin pain syndrome (GPS) is a prevalent issue in soccer. This study assessed the effectiveness of a new preventive protocol on GPS for youth soccer players. The protocol included targeted stretching and strengthening exercises for the adductor and core muscles from preseason to midseason. A questionnaire and two pain provocation tests were used for the evaluation. Mild GPS required positive results in at least two evaluations, while severe GPS was associated with pain incompatible with engagement in any activity confirmed by diagnostic ultrasound. Forty-two elite male athletes (aged 16.9 ± 0.7 years) participated in the study, with half of them assigned to the usual training (control group) and the remaining athletes undergoing the preventive protocol (treatment group) for 24 weeks. GPS rates were 14.3% (three diagnoses: two mild, one severe) in the treatment group and 28.6% (six diagnoses: three mild, three severe) in the control group. Toward the end of the season, three players, one from the treatment group and two from the control group had to stop playing due to severe GPS problems. In addition, one player in the control group stopped midseason. Even though the reduction in the risk of developing GPS was not significant (relative risk of 0.50 ([95%CI 0.14 to 1.74], p = 0.2759), the halved incidence of severe GPS and the increased muscle strength related to the treatment (p = 0.0277) are encouraging data for future studies.

Project description:Soccer players are required to anticipate and react continuously in a changing, relatively unpredictable situation in the field. Cognitive functions might be important to be successful in soccer. The current study investigated the relationship between cognitive functions and performance level in elite and sub-elite youth soccer players aged 13-17 years. A total of 47 elite youth soccer players (mean age 15.5 years, SD = 0.9) and 41 sub-elite youth soccer players (mean age 15.2 years, SD = 1.2) performed tasks for "higher-level" cognitive functions measuring working memory (i.e., Visual Memory Span), inhibitory control (i.e., Stop-Signal Task), cognitive flexibility (i.e., Trail Making Test), and metacognition (i.e., Delis-Kaplan Executive Function System Design Fluency Test). "Lower-level" cognitive processes, i.e., reaction time and visuo-perceptual abilities, were also measured with the previous tasks. ANOVA's showed that elite players outscored sub-elite players at the "higher-level" cognitive tasks only, especially on metacognition (p < .05). Using stepwise discriminant analysis, 62.5% of subjects was correctly assigned to one of the groups based on their metacognition, inhibitory control and cognitive flexibility performance. Controlling for training hours and academic level, MANCOVA's showed differences in favor of the elite youth soccer players on inhibitory control (p = .001), and cognitive flexibility (p = .042), but not on metacognition (p = .27). No differences were found concerning working memory nor the "lower-level" cognitive processes (p > .05). In conclusion, elite youth soccer players have better inhibitory control, cognitive flexibility, and especially metacognition than their sub-elite counterparts. However, when training hours are taken into account, differences between elite and sub-elite youth soccer players remain apparent on inhibitory control and cognitive flexibility in contrast to metacognition. This highlights the need for longitudinal studies to further investigate the importance of "higher-level" cognitive functions for talent identification, talent development and performance in soccer.

Project description:IntroductionExecutive functions (EFs) are relevant for game performance in soccer and have been investigated in previous research. However, emotions are a well-known performance factor in sport competitions, which may affect performance by means of EFs. The diagnostic of EFs has mainly been performed disregarding the potential impact of emotional states. Thus, we aimed to initially analyze interaction between emotional states and EFs in two studies with male youth elite soccer players.MethodsIn the first study, 105 players (Mage = 14.97) completed computerized tasks assessing inhibition, cognitive flexibility and working memory. In the second study, 92 players (Mage = 15.17) performed adapted and validated tests of inhibition (n = 45) or cognitive flexibility (n = 47) in a soccer-specific setting (SoccerBot360). Emotional and affective states were assessed using the German Sport Emotion Questionnaire and self-assessment manikins.ResultsFor the computerized tasks, results showed a significant negative correlation between switch costs accuracy and tension, r = 0.28, indicating lower error rates with higher levels of tension. In contrast, in the SoccerBot360 we found significant positive correlations for response time and tension (no-switch: r = 0.38; switch: r = 0.39) representing prolonged response times related to tension. Further, for soccer-specific inhibition, positive emotions were significantly positively correlated with response time (congruent: r = 0.32; incongruent: r = 0.32). Subsequent regression analyses also demonstrate that valence and arousal effectively explain variance in cognitive performance parameters under neutral conditions.DiscussionAccordingly, the ambiguity of the results suggests high variability in EF performance, affective and emotional states as well as a potentially moderating influence of other variables such as context and task difficulty. Thus, future cognitive diagnostic research should integrate assessments of emotional and affective states as these may contribute to situational fluctuations in EF performance.
