Predictors of obesity among school-age children in Debre Berhan City, Ethiopia.

ABSTRACT:

Background

Obesity causes a serious diet-related chronic disease, including type-2 diabetes, cardiovascular disease, hypertension, osteoarthritis, and certain forms of cancer. In Sub- Saharan Africa including Ethiopia, most nutritional interventions mainly focused on a child undernutrition and ignored the impacts of obesity among children. In Ethiopia, the magnitude and associated factors of obesity among school-age children were not clearly described. Therefore this study assesses the predictors of obesity among school- age children in Debre Berhan City, Ethiopia, 2022.

Methods

A cross-sectional study design was conducted from June to July, 2022. Participants were selected by using multistage sampling method. Data were collected using pre-tested and structured questions. Data were coded and entered in Epi-data version 4.6 and exported and analyzed using SPSS version 25.

Result

A total of 600 children were participating in the study. The prevalence of obesity was 10.7% (95% CI: 8.3, 13.2). In this study, attending at private school (AOR = 4.24, 95% CI: 1.58, 11.32), children aged between 10-12years (AOR = 2.67, 95% CI: 1.30, 5.48), soft drink available in home (AOR = 2.27, 95% CI: 1.25,18.13), Loneliness (AOR = 1.67 95% CI: 1.12, 3.15) and mothers with occupational status of daily labour (AOR = 8.54 95% CI: 1.12, 65.39) were significantly associated with childhood obesity.

Conclusion

In this study, the overall magnitude of childhood obesity was (10.7%) which means one in eleven children and relatively high as compare to the EDHS survey. Therefore, more attention should be given to strengthening physical activities, providing nutritional education, and creating community awareness about healthy diets as well as other preventive measures.

Project description:BackgroundThe COVID-19 pandemic has spread over the world. The ability to achieve sufficient immunization coverage to end the global pandemic depends on the acceptance of the COVID-19 vaccine, but it has faced a major challenge around the world. In low-income and developing countries, 22.7% of the population has received at least one dose of the Covid-19 vaccine, which means that a large percentage of the population are unvaccinated, even though they have access to the Covid-19 vaccine so many countries do not accept the vaccine. The aim of this study was to assess COVID-19 vaccine acceptance and its associated factors in Debre Berhan City, Ethiopia, 2022.MethodsA mixed-methods approach comprising both qualitative interviews and a quantitative survey was used among participants in Debre Berhan City. A multi-stage sampling technique was used to recruit the study participants. An in-depth interview was used for the qualitative data. Data was collected by a face-to-face interview questionnaire from June 08 to July 08, 2022. The collected data was entered using Epi Data version 4.6 and analyzed using SPSS version 25. Variables with a p-value less than 0.25 at Bivariable logistic regression analysis were entered into multivariable logistic regression analysis. Logistics regression was employed, and a p-value <0.05 was considered statistically significant.ResultA total of 765 participants were included in the study, with a response rate of 97.08%. More than half (52.9%) of the respondents had the willingness to accept the COVID-19 vaccine. Participants who had Contact with COVID-19 patient (AOR = 3.98; 95% CI: (1.30-12.14), having good knowledge of COVID-19 vaccine (AOR = 4.63; 95% CI: (1.84-11.63), and positive attitude toward the COVID-19 vaccine (AOR = 3.41; 95% CI: (1.34-8.69) were statistically significantly associated variables with COVID-19 vaccine acceptance.Conclusion and recommendationThe present study revealed that the acceptance COVID-19 vaccine was 52.9, and a significant proportion of participants were hesitant to receive the vaccine and refused to get vaccinated. Significantly associated Variables for COVID-19 vaccine acceptance were Contact with COVID-19 patient, having good knowledge of the COVID-19 vaccine, and having a positive attitude towards the COVID-19 vaccine. Various stakeholders to apprise the public about the cause of the disease and the scientific development of vaccine in order to enhance acceptance of the vaccine.

Project description:BackgroundThe double burden of malnutrition (DBM), contained both undernutrition and overnutrition, is a growing public health concern that presents a significant challenge to the food and nutrition policies of developing nations such as Ethiopia. However, the prevalence and contributing factors of DBM among adolescents in the study area have not been adequately investigated by Ethiopian researchers. Therefore, this study aims to determine the prevalence of DBM and contributing factors among secondary school students in Debre Berhan City, Ethiopia.MethodsA school-based cross-sectional study was conducted among 742 adolescents aged 10-19 years from October 13, 2022, to November 14, 2022, using a multi-stage sampling method. Data were collected using the online Kobo toolbox tool. A multinomial logistic regression model was used to analyze the data. The data were cleaned and analyzed in R software 4.2.2. Adolescents who had body mass index for age Z score (BAZ) < -2 SD, > +1 SD, and > +2 from the median value were considered thin, overweight, and obese, respectively.ResultsThe overall prevalence of DBM was 21.5% (14.8% thinness and 6.7% overweight/obesity). In the multivariable multinomial logistic regression analysis models factors such as age [AOR = 0.79, 95% CL: (0.67, 0.93)], sex [AOR = 3.86, 95% CL: (2.35, 6.32)], school type [AOR 5.03, 95% CL: (2.30, 10.99)], minimum dietary diversity score [AOR = 2.29, 95% CL: (1.27, 4.14)], frequency of meals [AOR = 2.09, 95% CL: (1.13, 3.89)], home gardening practice [AOR = 2.31, 95% CL: (1.44, 3.67)], history of illness [AOR = 0.57, 95% CL: (0.36, 0.93)], and knowledge of nutrition [AOR = 4.96, 95% CL: (1.61, 15.33)] were the significant predictors of either thinness or overweight/obesity (DBM).ConclusionMore than one-fifth of adolescents were affected by DBM in the study area. This prevalence is higher compared with the national and regional prevalence that found to be a public health concern. Thus, interventions like double-duty interventions should consider the age, sex, school type, minimum dietary diversity score, frequency of meals, home gardening practice, history of illness, and nutritional knowledge of adolescents.Clinical trial registrationclinicaltrial.gov, identifier NCT05574842.

Project description:BackgroundMorbidity is an immediate predictor of malnutrition. However, nutritional interventions to reduce frequent morbidities in adolescents were not conducted well based on behavioral models in low-income countries like Ethiopia. Hence, the aim of this study was to examine the effect of selected double-duty interventions on frequency of morbidities among adolescents based on health belief model in Debre Berhan Regiopolitan City, Central Ethiopia.MethodsA two-arm parallel cluster randomized controlled trial was conducted from October 13, 2022, to June 30, 2023, involving 356 adolescents in the intervention group (IG) and 352 adolescents in the control group (CG). The frequency of morbidity (FoM) was assessed through self-reported responses provided by the adolescents. To evaluate the impact of the intervention, a difference-in-differences analysis was employed along with a Generalized Estimating Equation (GEE) model using a Negative Binomial regression approach, which is suitable for analyzing count data with overdispersion. The statistical significance of the study was determined by setting a threshold of p-value less than 0.05.ResultsThe mean (± SD) score of FoM among IG and CG was - 4.5 (-1.82) and - 1.0 (-0.12), respectively. After the intervention had implemented, the IG had better reduction of mean score of FoM than CG. Moreover, adolescents in the endline measurements had 68% less likely to have higher FoM compared to the baseline measurements [IRR = 0.32, 95% CI (0.27, 0.37)]. Similarly, adolescents in the IG had 82.6% less likely to have higher FoM than CG [IRR = 0.72, 95% CI (0.12, 0.14)]. Likewise, adolescents in the time and treatment interaction category had 24.6% less likely to have higher FoM than adolescents who were not in the interaction category [IRR = 0.75, 95% CI (0.73, 0.78)].ConclusionThe results showed that selected double-duty intervention packages based on the health belief model are effective in reducing FoM among adolescents. The findings suggest the need for integrating the double-duty intervention packages accompanied with behavioral models to the adolescent nutrition guideline and food and nutrition policy and strategy.
