Cross-cultural translation and modification of the revised oral assessment guide for oral health assessment by non-dentists.

ABSTRACT:

Objectives

This study aimed to conduct a cross-cultural translation of the revised oral assessment guide (ROAG) into Thai language and to modify the tool to increase its validity and reliability.

Materials and methods

The present study was a cross-sectional design conducted in dental and hospitalized patients, and community-dwelling people. The original English-version of the ROAG was translated into Thai, which was evaluated for validity and reliability. The tool was then revised to develop the modified ROAG for non-dentist (ndROAG) comprising 9 oral assessment categories with a three-level response; healthy, mild, and severe alteration. The criterion validity of the ndROAG was tested in 82 adult and older participants, and 46 non-dentists comprising dental assistants, dental hygienists, community health volunteers, and nurses, using a calibrated dentist as the reference standard. The ndROAG was translated back into an English version. The criterion validity was evaluated using weighted Kappa (K_w) and intraclass correlation coefficient (ICC). Internal consistency was determined using Cronbach alpha. The three-level response was dichotomized into healthy and changed to determine the sensitivity and specificity.

Results

The K_w values, ICC, and Cronbach alpha values of the ndROAG were higher than those of the pre-test ROAG. The sensitivity of the ndROAG in identifying the healthy and changed state ranged from 57.1 to 100.0% with the lowest value in the saliva category, whereas the specificity ranged from 90.9-100.0%.

Conclusion

The original ROAG was translated and revised into the ndROAG with improved validity and reliability. The ndROAG can be used by non-dentists to assess the oral health of adult and older individuals to detect oral changes, which includes self-care instructions and patient referral guidance.

Project description:BackgroundThe Oral Health Impact Profile-14 (OHIP-14) questionnaire assesses quality of life related to people's perception of oral disorders on their well-being. However, a translated and validated Danish version of OHIP-14 is not yet available. The purpose of this pilot study was to translate and cross-culturally adapt the English version of the OHIP-14 into Danish (OHIP-14-DK). In addition, to assess its content and face validity, internal consistency and test-retest reliability.MethodsThe English version of OHIP-14 was translated into Danish following a standard protocol of cross-cultural adaptation. Stages I-IV: translation phase to generate a pre-final version "OHIP-14-DK". Stage V: pre-testing phase. A random sample of 22 orthodontic patients (mean age 24.7 years, SD ±14.8; 14 females, 8 males) were selected at the Section of Orthodontics, Aarhus University, Denmark. All patients self-completed the OHIP-14-DK and were then interviewed to assess its content and face validity. Internal consistency was assessed with Cronbach's alpha coefficients. All patients completed the same questionnaire again at a one-week interval. Test-retest reliability was assessed using Spearman's correlation coefficient and intra-class correlation coefficient (ICC).ResultsThe initial and back translations were very similar: the OHIP-14-DK proved to have a good level of equivalence with no translation errors or deviations. Furthermore, the OHIP-14-DK seemed well-adapted to Danish culture and was understood by individuals down to 12 years of age. Pre-testing demonstrated good face and content validity; interviews had a response rate of 100% and confirmed that each item was understandable without inducing reluctance or hesitation. Thus, responses were related to their corresponding item. Therefore, no final adjustments were required for the pre-tested version. Cronbach's alpha for the OHIP-14-DK subscales fell in the 0.75-0.84 range, indicating an adequate-to-good internal consistency. Spearman's correlation coefficient for the OHIP-14-DK total score was 0.77. The ICC for the OHIP-14-DK total score was 0.91.ConclusionsThe OHIP-14-DK seems well adapted to Danish culture, proved to be face and content valid and also showed good internal consistency and excellent reliability. Project description:The development of the Oral Health Impact Profile-49 (OHIP-49) and its shorter version, OHIP-14, has been criticized for not adequately considering the influence of systemic health on Periodontal health. The lack of a Periodontal status domain in OHIP scales has led to the proposal of the development of a new profile, Oral and Systemic Health Impact Profile (OSHIP-Perio). The newly developed profile has not undergone translation into other languages, cross-cultural adaptation, or validation and reliability testing. The primary objective of this study was to conduct a cross-cultural adaptation, validation, and reliability assessment of the Turkish translation of OSHIP-Perio for the first time. The data set comprises of responses from 354 individuals who participated in a face-to-face survey involving a series of fourteen inquiries related to the influence of Periodontal diseases on their general quality of life. The survey employed the OHIP-14-TR gold standard questionnaire, which has undergone prior validation and reliability testing in cross-cultural adaptation studies. Confirmatory factor analysis was employed to evaluate the construct validity. Test-retest reliability was evaluated using the Spearman-Brown and Gutmann test. Cronbach's alpha was used to assess the internal consistency. The concurrent validity was assessed using Spearman's correlation coefficient. The confirmatory factor analysis revealed that the scale is characterized by a single factor. The Spearman's correlation coefficient results were found to be 0.922 for concurrent validity. The internal consistency analysis yielded a Cronbach's alpha value of 0.944 for the scale. The Spearman-Brown and Guttmann methods yielded coefficients of 0.972 value for test-retest reliability. No floor or ceiling effects were found to be present. The Turkish version of the OSHIP-Perio questionnaire has been found to be valid and reliable. The OSHIP-Perio-TR can be used to assess the relationship between general well-being, its influence on Periodontal health, and the perception of illness in individuals with Periodontal diseases.
