The Use of MartriDerm in Nail Bed Reconstruction of Thumb Injured by a Pellet.

ABSTRACT: Gunshot injuries to the hand and upper extremity can be divided into low and high-energy injuries. Nonballistic firearms such as pellet guns are generally considered low-energy guns but can be associated with serious morbidity and even mortality. Management is tailored according to the severity and type of injured structures. Here, we present the case of a 21-year-old soldier who accidently shot his thumb with an air gun while on a bird hunting trip, whose case was managed with extraction placement of bone graft and collagen matrix to prevent nail deformity. This case was written to increase the awareness about the need for safe use of such guns and to display the authors preferred way of management.

Clinical Effects of Distal Phalanges and Nail Bed Fusion in Type III Thumb Duplication.

Project description:BackgroundThis study aimed to investigate the clinical effects of bone fusion distal to the phalangeal epiphyseal plate in treating asymmetrical type III thumb polydactyly, focusing on growth and scar healing.MethodsIn patients with asymmetrical type III thumb syndactyly, the proximal phalanx of the accessory finger, distal epiphyseal plate, and some soft tissue were excised to create a pedicled tissue flap. This flap, containing part of the nail and bone distal to the epiphyseal plate, was subsequently attached to the distal segment of the main finger. The therapeutic effect was evaluated during follow-up.ResultsTen children (6 boys and 4 girls) were included, with follow-up periods ranging from 24 to 60 months. All parents expressed satisfaction with the appearance and function of reconstructed thumbs. All reconstructed thumbs utilizing pedicled composite tissue flaps survived without arteriovenous crises, and there were no instances of perioperative mortality or wound infection. Compared with the contralateral normal thumb, most reconstructed thumbs exhibited good alignment and similar length and circumference. The reconstructed nails demonstrated a smooth surface with an appropriate curvature of the nail margin. The interphalangeal (IP) joint remained stable, and no instability of the IP joint collateral ligament was observed. The active and passive ranges of motion of the IP joint were satisfactory, and the epiphyseal plate of the reconstructed thumb was well-developed.ConclusionsThis method is considerably effective in reconstructing asymmetrical type III thumb duplication through fusion of bone distal to the phalangeal epiphyseal plate.

Reconstruction of a nail bed with double-layer artificial dermis for a pincer nail: A case report and literature review.

Project description:BackgroundA pincer nail (PN) is a type of nail deformity. Although it is a minor ailment, it can cause intractable pain, affecting daily work and life. Currently, there is no standard invention for PN. The main purpose of treatment is to correct the curvature of the nail, so we apply double-layer artificial dermis (DLAD), a novel treatment, for PN according to this aim.Case presentationA 40-year-old man suffering from PN was treated with DLAD. After 1 year of follow-up, the patient's great toenail plate of the right foot was completely grown out. His pain was relieved, and the curvature of the toenail was corrected.ConclusionThe innovative method of using DLAD to fill the ingrown nail bed not only solves the problem of recurrent pincer nail deformity but also restores the original appearance of the toenail. It suggested that this simple procedure can be widely applied in clinical practice.

An experimental study on coflow diffusion combustion in a pellet-packed bed with different bed lengths.

Project description:In experimental investigations on axial symmetry, over-ventilated CH4/air diffusion combustion in a packed bed is executed to study the height, shape and stability of the flame. The combustor is a quartz tube packed with alumina pellets in which a cylindrical fuel stream is surrounded by a coflow air nozzle. The results show that the bed length and pellet diameter have a significant influence on the flame properties. In general, the flame above the pellet surface has axial symmetry, and its shape and colour are similar to those of a conventional diffusion flame when the bed length is smaller. The colour of the flame front varies with the bed length. The changed colour indicates an increased flame front temperature and that the combustion regime above the bed surface may change from non-premixed combustion to partially premixed combustion or even premixed combustion owing to the mix and dispersion effect in the packed bed. In addition, multiple flame behaviours, such as an inclined flame front, isolated reaction zone and oscillatory motion followed by a pulsating sound with a few hertz in a packed bed, are observed experimentally. The possible reasons for these phenomena are discussed.

Randomized feasibility trial of replacing or discarding the nail plate after nail-bed repair in children.

Project description:BackgroundNail-bed injuries are the most common hand injury in children. Surgical dogma is to replace the nail plate after repairing the nail bed. Recent evidence suggests this might increase infection rates and returns to clinic. The aim of this feasibility trial was to inform the design and conduct of a definitive trial comparing replacing or discarding the nail plate after nail-bed repair.MethodsThis study recruited participants from four hand units in the UK between April and July 2015. Participants were children under the age of 16 years with a nail-bed injury requiring surgery. They were randomized to either having the nail plate replaced or discarded after nail-bed repair. The follow-up method was also allocated randomly (postal versus clinic). Information was collected on complications at 2 weeks and 30 days, and on nail-plate appearance at 4 months using the Zook classification. Two possible approaches to follow-up were also piloted and compared.ResultsDuring the recruitment phase, there were 156 potentially eligible children. Sixty were randomized in just over 3 months using remote web-based allocation. By 2 weeks, there were two infections, both in children with replaced nail plates. The nail-replaced group also experienced more complications. There was no evidence of a difference in return rates between postal and clinic follow-up.ConclusionRecruitment was rapid and nail-bed repair appeared to have low complication and infection rates in this pilot trial. The findings have led to revision of the definitive trial protocol, including the mode and timing of follow-up, and modification of the Zook classification.

Data on microcirculatory perfusion dips in the resting nail bed.

Project description:This article contains the raw data from the article entitled: "The presence of synchronized perfusion dips in the microcirculation of the resting nail bed" Mirdell et al. (in press). A laser speckle contrast imager (LSCI) was used to make a total of 21 recordings of the perfusion in the resting nail bed of 10 healthy test subjects. The first 10 recordings were acquired after 5 min of acclimatization. An additional 10 recordings were acquired in the same test subjects, after 20 min of acclimatization. In the last recording, a digital nerve block was applied to the left dig III. The data show the presence of highly irregular perfusion variations, a phenomenon we like to call perfusion dips. The data also show how the perfusion dips can be abolished through a digital nerve block. An algorithm for the quantification of the perfusion dips is included in the data.

Tracheal reconstruction with nail grafts: A novel approach.

Project description:BackgroundThe replacement of tracheal defects has been a challenge for investigators worldwide. We aimed to develop autologous nail grafts for the reconstruction of anterior tracheal defects.MethodsToenail grafts were implanted to improve the structural integrity of the trachea in patients with tracheal diseases. We clinically applied these grafts for the partial replacement of the cervical tracheal cartilage. Data on graft construction details, clinical outcomes, bronchoscopy, and 3-dimensional computed tomography examinations were collected.ResultsThe nail grafts were implanted in 4 patients. The trachea was successfully reconstructed in all cases. Bronchoscopy was performed 3 times to document healing: immediately, 1 month, and 3 months after surgery. All grafts were well vascularized and incorporated into the tracheal wall and were covered with the respiratory mucosa. Three of the patients survived during the study period, but 1 patient died of progressive lung cancer.ConclusionsToenail grafts potentially may be used as an alternative strategy for the closure of small defects during tracheal reconstruction.
