Predicting Cardiovascular Disease Mortality: Leveraging Machine Learning for Comprehensive Assessment of Health and Nutrition Variables.

ABSTRACT: Cardiovascular disease (CVD) is one of the primary causes of death around the world. This study aimed to identify risk factors associated with CVD mortality using data from the National Health and Nutrition Examination Survey (NHANES). We created three models focusing on dietary data, non-diet-related health data, and a combination of both. Machine learning (ML) models, particularly the random forest algorithm, demonstrated robust consistency across health, nutrition, and mixed categories in predicting death from CVD. Shapley additive explanation (SHAP) values showed age, systolic blood pressure, and several other health factors as crucial variables, while fiber, calcium, and vitamin E, among others, were significant nutritional variables. Our research emphasizes the importance of comprehensive health evaluation and dietary intake in predicting CVD mortality. The inclusion of nutrition variables improved the performance of our models, underscoring the utility of dietary intake in ML-based data analysis. Further investigation using large datasets with recurring dietary recalls is necessary to enhance the effectiveness and interpretability of such models.

Project description:Hyperkalemia is associated with increased risks of mortality and adverse clinical outcomes. The treatment of hyperkalemia often leads to the discontinuation or restriction of beneficial but potassium-increasing therapy such as renin-angiotensin-aldosterone inhibitors (RAASi) and high-potassium diet including fruits and vegetables. To date, limited evidence is available for personalized risk evaluation in this heterogeneous and multifactorial pathophysiological condition. We developed risk prediction models using extreme gradient boosting (XGB), multiple logistic regression (LR), and deep neural network. Models were derived from a retrospective cohort of hyperkalemic patients with either heart failure or chronic kidney disease stage ≥3a from a Japanese nationwide database (1 April 2008-30 September 2018). Studied outcomes included all-cause death, renal replacement therapy introduction (RRT), hospitalization for heart failure (HHF), and cardiovascular events within three years after hyperkalemic episodes. The best performing model was further validated using an external cohort. A total of 24,949 adult hyperkalemic patients were selected for model derivation and internal validation. A total of 1452 deaths (16.6%), 887 RRT (10.1%), 1,345 HHF (15.4%), and 621 cardiovascular events (7.1%) were observed. XGB outperformed other models. The area under receiver operator characteristic curves (AUROCs) of XGB vs. LR (95% CIs) for death, RRT, HHF, and cardiovascular events were 0.823 (0.805-0.841) vs. 0.809 (0.791-0.828), 0.957 (0.947-0.967) vs. 0.947 (0.936-0.959), 0.863 (0.846-0.880) vs. 0.838 (0.820-0.856), and 0.809 (0.784-0.834) vs. 0.798 (0.772-0.823), respectively. In the external dataset including 86,279 patients, AUROCs (95% CIs) for XGB were: death, 0.747 (0.742-0.753); RRT, 0.888 (0.882-0.894); HHF, 0.673 (0.666-0.679); and cardiovascular events, 0.585 (0.578-0.591). Kaplan-Meier curves of the high-risk predicted group showed a statistically significant difference from that of the low-risk predicted groups for all outcomes (p < 0.005; log-rank test). These findings suggest possible use of machine learning models for real-world risk assessment as a guide for observation and/or treatment decision making that may potentially lead to improved outcomes in hyperkalemic patients while retaining the benefit of life-saving therapies.

Project description:IntroductionApproximately 7,000 newborns die every day, accounting for almost half of child deaths under 5 years of age. Deciphering which neonates are at increased risk for mortality can have an important global impact. As such, integrating high computational technology (e.g., artificial intelligence [AI]) may help identify the early and potentially modifiable predictors of neonatal mortality. Therefore, the objective of this study was to collate, critically appraise, and analyze neonatal prediction studies that included AI.MethodsA literature search was performed in PubMed, Cochrane, OVID, and Google Scholar. We included studies that used AI (e.g., machine learning (ML) and deep learning) to formulate prediction models for neonatal death. We excluded small studies (n < 500 individuals) and studies using only antenatal factors to predict mortality. Two independent investigators screened all articles for inclusion. The data collection consisted of study design, number of models, features used per model, feature importance, internal and/or external validation, and calibration analysis. Our primary outcome was the average area under the receiving characteristic curve (AUC) or sensitivity and specificity for all models included in each study.ResultsOf 434 articles, 11 studies were included. The total number of participants was 1.26 M with gestational ages ranging from 22 weeks to term. Number of features ranged from 3 to 66 with timing of prediction as early as 5 min of life to a maximum of 7 days of age. The average number of models per study was 4, with neural network, random forest, and logistic regression comprising the most used models (58.3%). Five studies (45.5%) reported calibration plots and 2 (18.2%) conducted external validation. Eight studies reported results by AUC and 5 studies reported the sensitivity and specificity. The AUC varied from 58.3% to 97.0%. The mean sensitivities ranged from 63% to 80% and specificities from 78% to 99%. The best overall model was linear discriminant analysis, but it also had a high number of features (n = 17).Discussion/conclusionML models can accurately predict death in neonates. This analysis demonstrates the most commonly used predictors and metrics for AI prediction models for neonatal mortality. Future studies should focus on external validation, calibration, as well as deployment of applications that can be readily accessible to health-care providers.

Project description:Background: Individuals with major depressive disorder (MDD) vary in their response to antidepressants. However, identifying objective biomarkers, prior to or early in the course of treatment that can predict antidepressant efficacy, remains a challenge. Methods: Individuals with MDD participated in a 12-week antidepressant pharmacotherapy trial. Electroencephalographic (EEG) data was collected before and 1 week post-treatment initiation in 51 patients. Response status at week 12 was established with the Montgomery-Asberg Depression Scale (MADRS), with a ≥50% decrease characterizing responders (N = 27/24 responders/non-responders). We used a machine learning (ML)-approach for predicting response status. We focused on Random Forests, though other ML methods were compared. First, we used a tree-based estimator to select a relatively small number of significant features from: (a) demographic/clinical data (age, sex, individual item/total MADRS scores at baseline, week 1, change scores); (b) scalp-level EEG power; (c) source-localized current density (via exact low-resolution electromagnetic tomography [eLORETA] software). Second, we applied kernel principal component analysis to reduce and map important features. Third, a set of ML models were constructed to classify response outcome based on mapped features. For each dataset, predictive features were extracted, followed by a model of all predictive features, and finally by a model of the most predictive features. Results: Fifty eLORETA features were predictive of response (across bands, both time-points); alpha1/theta eLORETA features showed the highest predictive value. Eighty-eight scalp EEG features were predictive of response (across bands, both time-points), with theta/alpha2 being most predictive. Clinical/demographic data consisted of 31 features, with the most important being week 1 "concentration difficulty" scores. When all features were included into one model, its predictive utility was high (88% accuracy). When the most important features were extracted in the final model, 12 predictive features emerged (78% accuracy), including baseline scalp-EEG frontopolar theta, parietal alpha2 and frontopolar alpha1. Conclusions: These findings suggest that ML models of pre- and early treatment-emergent EEG profiles and clinical features can serve as tools for predicting antidepressant response. While this must be replicated using large independent samples, it lays the groundwork for research on personalized, "biomarker"-based treatment approaches.
