Project description:BackgroundSome attempts have been made to detect atrial fibrillation (AF) with a wearable device equipped with photoelectric volumetric pulse wave technology, and it is expected to be applied under real clinical conditions.ObjectiveThis study is the second part of a 2-phase study aimed at developing a method for immediate detection of paroxysmal AF, using a wearable device with built-in photoplethysmography (PPG). The objective of this study is to develop an algorithm to immediately diagnose AF by an Apple Watch equipped with a PPG sensor that is worn by patients undergoing cardiac surgery and to use machine learning on the pulse data output from the device.MethodsA total of 80 patients who underwent cardiac surgery at a single institution between June 2020 and March 2021 were monitored for postoperative AF, using a telemetry-monitored electrocardiogram (ECG) and an Apple Watch. AF was diagnosed by qualified physicians from telemetry-monitored ECGs and 12-lead ECGs; a diagnostic algorithm was developed using machine learning on the pulse rate data output from the Apple Watch.ResultsOne of the 80 patients was excluded from the analysis due to redness caused by wearing the Apple Watch. Of 79 patients, 27 (34.2%) developed AF, and 199 events of AF including brief AF were observed. Of them, 18 events of AF lasting longer than 1 hour were observed, and cross-correlation analysis showed that pulse rate measured by Apple Watch was strongly correlated (cross-correlation functions [CCF]: 0.6-0.8) with 8 events and very strongly correlated (CCF>0.8) with 3 events. The diagnostic accuracy by machine learning was 0.9416 (sensitivity 0.909 and specificity 0.838 at the point closest to the top left) for the area under the receiver operating characteristic curve.ConclusionsWe were able to safely monitor pulse rate in patients who wore an Apple Watch after cardiac surgery. Although the pulse rate measured by the PPG sensor does not follow the heart rate recorded by telemetry-monitored ECGs in some parts, which may reduce the accuracy of AF diagnosis by machine learning, we have shown the possibility of clinical application of using only the pulse rate collected by the PPG sensor for the early detection of AF.

Project description:Digital technologies empower users to manage their health and reduce the burden on the public health system. The mass adoption of wearable medical devices (WMDs) promotes the ageing population's confidence besides facilitating users. Thus, the current study aims to empirically evaluate the formation of perceived product value (PPV) with the WMDs' computability, usefulness, cost, and accuracy, the intention to use WMDs influenced by health consciousness (HCS), health anxiety (HAY), product value, and perceived critical mass (PCM), and later the adoption of WMDs among Chinese adults. The study examined the mediating effect of PPV on the relationship between the intention to use WMDs and perceived compatibility (PCT), perceived cost (PCO), perceived usefulness (PUS), and perceived technology accuracy (PTA). This study adopted a cross-sectional approach and used an online survey to collect quantitative data from 1,160 Chinese adults. Data analysis was performed using the partial least squares structural equation modelling (PLS-SEM). Results showed that PCT, PUS, and PTA significant positive effect on PPV. Meanwhile, HCS, PCM, and PPV has a significant positive effect on intention to use WMDs, and the intention to use WMDs and PCM influenced the adoption of WMDs. Consequently, the analysis confirmed that PPV mediated the relationships between the intention to use WMD and PCT, PUS, and PTA. The WMD cost must be reduced to enhance the value of WMDs. Finally, the study's implications, limitations, and suggestions for future studies are discussed.

Project description:ObjectiveThe current study explores the formation of an intention to use wearable fitness devices (WFDs) with wearable fitness attributes and health consciousness (HCS). Moreover, the research examines the use of WFDs with the health motivation (HMT) and intention to use WFDs. The study also exposes the moderating effect of HMT between the intention to use WFDs and the use of WFDs.MethodsFive hundred and twenty-five adults participated in the current study, and data were collected from January 2021 to March 2021 through the online survey of Malaysian respondents. The cross-sectional data was analyzed using the second-generation statistical method of partial least square structural equation modeling.ResultsHCS is insignificantly associated with the intention to use WFDs. Perceived compatibility, perceived product value, perceived usefulness, and perceived technology accuracy significantly influence the intention to use WFDs. HMT significantly impacts the adoption of WFDs; however, the intention to use WFDs negatively but significantly influences the use of WFDs. Lastly, the association between the intention to use WFDs and the adoption of WFDs is significantly moderated by HMT.ConclusionsOur study findings illuminate the significant impact of technology-level attributes of WFDs on the intention to use WFDs. However, an insignificant impact of HCS on the intention to use WFDs was reported. Our result confirms that HMT plays a significant role in the use of WFDs. Such as the moderating role of HMT is vital to transform the intention to use WFDs into the adoption of WFDs.

Project description:BackgroundDepression is a prevalent mental disorder that is undiagnosed and untreated in half of all cases. Wearable activity trackers collect fine-grained sensor data characterizing the behavior and physiology of users (ie, digital biomarkers), which could be used for timely, unobtrusive, and scalable depression screening.ObjectiveThe aim of this study was to examine the predictive ability of digital biomarkers, based on sensor data from consumer-grade wearables, to detect risk of depression in a working population.MethodsThis was a cross-sectional study of 290 healthy working adults. Participants wore Fitbit Charge 2 devices for 14 consecutive days and completed a health survey, including screening for depressive symptoms using the 9-item Patient Health Questionnaire (PHQ-9), at baseline and 2 weeks later. We extracted a range of known and novel digital biomarkers characterizing physical activity, sleep patterns, and circadian rhythms from wearables using steps, heart rate, energy expenditure, and sleep data. Associations between severity of depressive symptoms and digital biomarkers were examined with Spearman correlation and multiple regression analyses adjusted for potential confounders, including sociodemographic characteristics, alcohol consumption, smoking, self-rated health, subjective sleep characteristics, and loneliness. Supervised machine learning with statistically selected digital biomarkers was used to predict risk of depression (ie, symptom severity and screening status). We used varying cutoff scores from an acceptable PHQ-9 score range to define the depression group and different subsamples for classification, while the set of statistically selected digital biomarkers remained the same. For the performance evaluation, we used k-fold cross-validation and obtained accuracy measures from the holdout folds.ResultsA total of 267 participants were included in the analysis. The mean age of the participants was 33 (SD 8.6, range 21-64) years. Out of 267 participants, there was a mild female bias displayed (n=170, 63.7%). The majority of the participants were Chinese (n=211, 79.0%), single (n=163, 61.0%), and had a university degree (n=238, 89.1%). We found that a greater severity of depressive symptoms was robustly associated with greater variation of nighttime heart rate between 2 AM and 4 AM and between 4 AM and 6 AM; it was also associated with lower regularity of weekday circadian rhythms based on steps and estimated with nonparametric measures of interdaily stability and autocorrelation as well as fewer steps-based daily peaks. Despite several reliable associations, our evidence showed limited ability of digital biomarkers to detect depression in the whole sample of working adults. However, in balanced and contrasted subsamples comprised of depressed and healthy participants with no risk of depression (ie, no or minimal depressive symptoms), the model achieved an accuracy of 80%, a sensitivity of 82%, and a specificity of 78% in detecting subjects at high risk of depression.ConclusionsDigital biomarkers that have been discovered and are based on behavioral and physiological data from consumer wearables could detect increased risk of depression and have the potential to assist in depression screening, yet current evidence shows limited predictive ability. Machine learning models combining these digital biomarkers could discriminate between individuals with a high risk of depression and individuals with no risk.

Project description:IntroductionThe clinical assessment of Parkinson's disease (PD) symptoms can present reliability issues and, with visits typically spaced apart 6 months, can hardly capture their frequent variability. Smartphones and smartwatches along with signal processing and machine learning can facilitate frequent, remote, reliable and objective assessments of PD from patients' homes.AimTo investigate the feasibility, compliance and user experience of passively and actively measuring symptoms from home environments using data from sensors embedded in smartphones and a wrist-wearable device.Methods and analysisIn an ongoing clinical feasibility study, participants with a confirmed PD diagnosis are being recruited. Participants perform activity tests, including Timed Up and Go (TUG), tremor, finger tapping, drawing and vocalisation, once a week for 2 months using the Mobistudy smartphone app in their homes. Concurrently, participants wear the GENEActiv wrist device for 28 days to measure actigraphy continuously. In addition to using sensors, participants complete the Beck's Depression Inventory, Non-Motor Symptoms Questionnaire (NMSQuest) and Parkinson's Disease Questionnaire (PDQ-8) questionnaires at baseline, at 1 month and at the end of the study. Sleep disorders are assessed through the Parkinson's Disease Sleep Scale-2 questionnaire (weekly) and a custom sleep quality daily questionnaire. User experience questionnaires, Technology Acceptance Model and User Version of the Mobile Application Rating Scale, are delivered at 1 month. Clinical assessment (Movement Disorder Society-Unified Parkinson Disease Rating Scale (MDS-UPDRS)) is performed at enrollment and the 2-month follow-up visit. During visits, a TUG test is performed using the smartphone and the G-Walk motion sensor as reference device. Signal processing and machine learning techniques will be employed to analyse the data collected from Mobistudy app and the GENEActiv and correlate them with the MDS-UPDRS. Compliance and user aspects will be informing the long-term feasibility.Ethics and disseminationThe study received ethical approval by the Swedish Ethical Review Authority (Etikprövningsmyndigheten), with application number 2022-02885-01. Results will be reported in peer-reviewed journals and conferences. Results will be shared with the study participants.

Project description:ObjectiveHealth self-monitoring technologies are gaining popularity worldwide, but they face low adoption rates in emerging countries. There is a deficiency in studies that have applied the value-belief-norm (VBN) model to understand the adoption of IoT-enabled wearable healthcare devices (WHDs). This study investigates the adoption of IoT-enabled WHDs among older adults in China, using the VBN model as a theoretical framework.MethodsUsing a convenience sampling method and a web-based cross-sectional survey method, we collected data from 476 respondents, which we analyzed using partial least squares structural equation modeling using Smart PLS version 3.3.5.ResultsThe findings highlight the significance of health values and motivation in shaping personal health beliefs, which, in turn, influence personal norms and awareness of consequences. Particularly, awareness of consequences and attributions of responsibility significantly impact personal norms. Personal and social norms, in turn, strongly affect the intention to adopt IoT-enabled WHDs, ultimately driving their actual adoption.ConclusionThis research contributes novel insights into the behavioral dynamics surrounding the adoption of IoT-enabled WHDs, providing valuable guidance for marketers and policymakers. Marketers can leverage these insights to develop tailored marketing strategies within the IoT-enabled WHD industry. Additionally, policymakers are urged to prioritize campaigns aimed at enhancing awareness and understanding of self-healthcare monitoring, with a focus on promoting the unique health benefits of IoT-enabled WHDs.

Project description:BackgroundKnee osteoarthritis (KOA) is an irreversible degenerative disease that characterized by pain and abnormal gait. Radiography is typically used to detect KOA but has limitations. This study aimed to identify changes in plantar pressure that are associated with radiological knee osteoarthritis (ROA) and to validate them using machine learning algorithms.MethodsThis study included 92 participants with variable degrees of KOA. A modified Kellgren-Lawrence scale was used to classify participants into non-ROA and ROA groups. The total feature set included 210 dynamic plantar pressure features captured by a wearable in-shoe system as well as age, gender, height, weight, and body mass index. Filter and wrapper methods identified the optimal features, which were used to train five types of machine learning classification models for further validation: k-nearest neighbors (KNN), support vector machine (SVM), random forest (RF), AdaBoost, and eXtreme gradient boosting (XGBoost).ResultsAge, the standard deviation (SD) of the peak plantar pressure under the left lateral heel (f_L8PPP_std), the SD of the right second peak pressure (f_Rpeak2_std), and the SD of the variation in the anteroposterior displacement of center of pressure (COP) in the right foot (f_RYcopstd_std) were most associated with ROA. The RF model with an accuracy of 82.61% and F1 score of 0.8000 had the best generalization ability.ConclusionChanges in dynamic plantar pressure are promising mechanical biomarkers that distinguish between non-ROA and ROA. Combining a wearable in-shoe system with machine learning enables dynamic monitoring of KOA, which could help guide treatment plans.

Project description:This study aims to examine the mediating effect of the intention to use wearable payment devices (WPD) between perceived ease of use (PE), perceived usefulness (PU), social influence (SI), perceived trust (TR), and lifestyle compatibility (CM) on the adoption of WPD. Examination was made on the moderating effect of age and gender to improve the understanding of the adoption of WPD as a new payment system. Empirical data was collected through an online survey from 1094 respondents in Malaysia. Furthermore, this study employed dual-stage data analysis through partial least squares structural equation modelling (PLS-SEM) to test the causal and moderating effects, including artificial neural network (ANN) to examine the predictive power of the selected model. As a result, it was found that PE, PU, TR, and CM had a significant positive influence on the intention to use WPD. Furthermore, facilitating conditions and the intention to use WPD exhibited strong positive impacts on the adoption of WPD among Malaysian youth. The intention to use WPD positively and significantly mediated all predictors of adoption of WPD. Following that, ANN analysis confirmed high prediction accuracy of the data fitness. Overall, the findings for ANN highlighted the importance of PE, CM, and TR on the intention to adopt WPD and the impact of facilitating conditions on the adoption of WPD among Malaysian youth. Theoretically, the study extended UTAUT with two additional determinants (e.g., perceived trust and lifestyle compatibility), which were found to have significant influences on the intention to use WPD. The study results would be able to help payment service providers and the smart wearable device industry offer an innovative spectrum of products and present effective marketing tactics to encourage the prospective consumers of Wearable Payment Devices in Malaysia.

Project description:Wearable fitness devices (WFDs) are prevalent personal technology that empowers the users' management and supervision of their personal health. The current study explored the impact of health consciousness, health motivation, perceived cost, compatibility, usefulness, and perceived technology accuracy with the intention to use the WFDs. Furthermore, the users' conspicuous consumption and intention promote the usage of WFDs. A cross-sectional and quantitative research design was utilized for the current study, followed by data collection through social media and a final analysis with 1,071 samples data. The data analysis was accomplished with the partial least square regression structural equation modeling. The findings of this study revealed that the users' level of health consciousness, perceived compatibility, usefulness, perceived cost, and technology accuracy significantly influenced the intention to use WFDs. However, the conspicuous consumption and intention indicated the support for the usage behavior of the WFDs. This behavior significantly moderated the relationship between the intention and usage behavior for the WFDs. This study contributed to the theoretical realm for prompting the intention to use the WFDs with personal protection motivation that depicts the coping strategy and technology level attributes that form the intention to use WFDs. The WFDs manufacturers should therefore focus on developing WFDs features that harness usage behavior among the adults. Developing the personal responsibility to reduce the burden of the healthcare system and taking care of personal health could promote the usage of the WFDs.

Project description:BACKGROUND:Remote monitoring of patients with heart failure (HF) using wearable devices can allow patient-specific adjustments to treatments and thereby potentially reduce hospitalizations. We aimed to assess HF state using wearable measurements of electrical and mechanical aspects of cardiac function in the context of exercise. METHODS AND RESULTS:Patients with compensated (outpatient) and decompensated (hospitalized) HF were fitted with a wearable ECG and seismocardiogram sensing patch. Patients stood at rest for an initial recording, performed a 6-minute walk test, and then stood at rest for 5 minutes of recovery. The protocol was performed at the time of outpatient visit or at 2 time points (admission and discharge) during an HF hospitalization. To assess patient state, we devised a method based on comparing the similarity of the structure of seismocardiogram signals after exercise compared with rest using graph mining (graph similarity score). We found that graph similarity score can assess HF patient state and correlates to clinical improvement in 45 patients (13 decompensated, 32 compensated). A significant difference was found between the groups in the graph similarity score metric (44.4±4.9 [decompensated HF] versus 35.2±10.5 [compensated HF]; P<0.001). In the 6 decompensated patients with longitudinal data, we found a significant change in graph similarity score from admission (decompensated) to discharge (compensated; 44±4.1 [admitted] versus 35±3.9 [discharged]; P<0.05). CONCLUSIONS:Wearable technologies recording cardiac function and machine learning algorithms can assess compensated and decompensated HF states by analyzing cardiac response to submaximal exercise. These techniques can be tested in the future to track the clinical status of outpatients with HF and their response to pharmacological interventions.
