ABSTRACT:

Background

Patients with inflammatory bowel disease (IBD) require accessible, timely, and noninvasive strategies to monitor disease. The aim was to assess the integration of intestinal ultrasound (IUS) on decision-making and endoscopy utilization in a standardized care pathway.

Methods

This prospective, multicenter, international, observational cohort study included patients seen within a centralized model for IBD care was conducted during the COVID pandemic. Patients were evaluated with IUS alone or in combination with an in-clinic, unsedated sigmoidoscopy. Demographic, clinical, laboratory, and imaging data, clinical decisions, and need for urgent endoscopy, hospitalization, and surgeries were recorded.

Results

Of the 158 patients included, the majority had an established diagnosis of Crohn's disease (n = 123, 78%), and 47% (n = 75) of patients were on biologic therapy. IUS identified active inflammation in 65% (n = 102) of patients, and strictures in 14% (n = 22). Fecal calprotectin levels correlated with inflammation detected on IUS (median of 50 μg/g [Q1-Q3: 26-107 μg/g] without inflammation and 270 μg/g [Q1-Q3: 61-556 μg/g] with inflammation; p = 0.0271). In the majority of patients, clinical assessment with IUS led to an acute change in IBD-specific medications (57%, n = 90) and avoided or delayed the need for urgent endoscopy (85%, n = 134). Four patients were referred for urgent surgical consultation.

Conclusions

Point-of-care IUS used in a flare clinic pathway is a useful strategy to improve effective IBD care delivery and to assist in therapeutic management decisions, in many cases avoiding the acute need for endoscopy.

Project description:ObjectiveTo conceptualise the cognitive processes of early expert decision-making in urgent care.BackgroundExpert clinicians in the UK frequently determine suitable urgent care patient pathways via telephone triage. This strategy is promoted by policymakers but how it is performed, and its effectiveness has not been evaluated. Evaluation of early senior decision-making requires knowledge of decision-processes, influences, and goals. Previous research has focused on diagnostic decision-making and rarely studied clinicians in the field.MethodWe triangulated analytic autoethnography of early expert decision-making with focused ethnography of experts and trainee doctors performing the task. The study took place in a medium-sized Acute Medical Unit which provided internal medical emergency care for a mixed urban and rural population in the UK. A grounded theoretical model of expert decision-making was created via Gioia Methodology. Decision types were categorised to identify differences in solutions as well as decision processes.ResultsThe hallmarks of intuitive decision-making were found in most expert decisions. Experts made intuitive use of pattern-matching to extract key data from large volumes of information which triggered the spontaneous manifestation of solutions. Solutions were holistic and usually solitary. Upon manifestation solutions were consciously tested for viability with emotional affect playing a key role. Expert solutions were previously applied ones but were frequently entirely novel. Novel solution generation was not a feature of trainee decisions but moments of intuition were. Expert goals varied between optimal care for individual patients, system-wide efficiency, and equity of care. The decision environment had a large influence upon experts.ConclusionExpert clinicians employ intuitive decision-making supported by rational analysis in early urgent care decision-making. Expert solutions generated in this manner are pragmatic rather than optimal, context dependent, and seek to achieve goals which vary from moment-moment. Findings are crucial to inform research evaluating the effectiveness of early expert decision-making in urgent care as it is a high cost strategy. They also have implications for methodological approaches in future studies of expert clinical decision-making, developing artificial expert systems, and clinician training.

Project description:Adaptive management involves learning-oriented decision making in the presence of uncertainty about the responses of a resource system to management. It is implemented through an iterative sequence of decision making, monitoring and assessment of system responses, and incorporating what is learned into future decision making. Decision making at each point is informed by a value or objective function, for example total harvest anticipated over some time frame. The value function expresses the value associated with decisions, and it is influenced by system status as updated through monitoring. Often, decision making follows shortly after a monitoring event. However, it is certainly possible for the cadence of decision making to differ from that of monitoring. In this paper we consider different combinations of annual and biennial decision making, along with annual and biennial monitoring. With biennial decision making decisions are changed only every other year; with biennial monitoring field data are collected only every other year. Different cadences of decision making combine with annual and biennial monitoring to define 4 scenarios. Under each scenario we describe optimal valuations for active and passive adaptive decision making. We highlight patterns in valuation among scenarios, depending on the occurrence of monitoring and decision making events. Differences between years are tied to the fact that every other year a new decision can be made no matter what the scenario, and state information is available to inform that decision. In the subsequent year, however, in 3 of the 4 scenarios either a decision is repeated or monitoring does not occur (or both). There are substantive differences in optimal values among the scenarios, as well as the optimal policies producing those values. Especially noteworthy is the influence of monitoring cadence on valuation in some years. We highlight patterns in policy and valuation among the scenarios, and discuss management implications and extensions.

Project description:BackgroundPatients may prefer different levels of involvement in decision-making regarding their medical care which may influence their medical knowledge.ObjectiveWe investigated associations of patients' decisional control preference (DCP) with their medical knowledge, ward round performance measures (e.g., duration, occurrence of sensitive topics), and perceived quality of care measures (e.g., trust in the healthcare team, satisfaction with hospital stay).DesignThis is a secondary analysis of a randomized controlled multicenter trial conducted between 2017 and 2019 at 3 Swiss teaching hospitals.ParticipantsAdult patients that were hospitalized for inpatient care.Main measuresThe primary outcome was patients' subjective average knowledge of their medical care (rated on a visual analog scale from 0 to 100). We classified patients as active, collaborative, and passive according to the Control Preference Scale. Data collection was performed before, during, and after the ward round.Key resultsAmong the 761 included patients, those with a passive DCP had a similar subjective average (mean ± SD) knowledge (81.3 ± 19.4 points) compared to patients with a collaborative DCP (78.7 ± 20.3 points) and active DCP (81.3 ± 21.5 points), p = 0.25. Regarding patients' trust in physicians and nurses, we found that patients with an active vs. passive DCP reported significantly less trust in physicians (adjusted difference, - 5.08 [95% CI, - 8.69 to - 1.48 points], p = 0.006) and in nurses (adjusted difference, - 3.41 [95% CI, - 6.51 to - 0.31 points], p = 0.031). Also, patients with an active vs. passive DCP were significantly less satisfied with their hospital stay (adjusted difference, - 7.17 [95% CI, - 11.01 to - 3.34 points], p < 0.001).ConclusionPatients with active DCP have lower trust in the healthcare team and lower overall satisfaction despite similar perceived medical knowledge. The knowledge of a patient's DCP may help to individualize patient-centered care. A personalized approach may improve the patient-physician relationship and increase patients' satisfaction with medical care.Trial registrationClinicalTrials.gov (NCT03210987).
