Genetics and genomics knowledge and competency of Chinese nurses based on the theory of diffusion of innovation: A descriptive cross-sectional study.

ABSTRACT:

Background

Nurses are responsible for providing genetics and genomics health care services, including disease risk assessment, medical referrals, and advocating for communities and individuals who probably benefit from genomics and genetics services.

Objective

This study aimed to investigate Chinese nurses' understanding and previous training in genomics and genetics. Additionally, we aimed to understand the degree to which Chinese nurses use genomics and genetics knowledge in their daily practice and to assess the learning needs regarding genomics and genetics.

Method

This study utilized the design of a descriptive cross-sectional study. From January 18 to March 15, 2022, 406 registered nurses from the Shandong province of China working in clinical institutions and with good English proficiency were recruited as study subjects. Nurses' demographic data and data related to genetic knowledge and competency were collected using the Genetics and Genomics Nursing Practice Survey (GGNPS) questionnaire. Kruskal Wallis and Mann-Whitney-U tests were utilized to investigate the descriptive statistics and non-normally distributed data.

Results

There are 406 nurses from Shandong Province, China participated in the study. Among them, BSN degrees accounted for 83.7%. Female nurses accounted for 96.8%, and 40% are clinical nurses. Among such nurses, 65.5% reported that genetics was involved in their nursing courses, and 56.2% planned to study more knowledge associated with genetics. The mean knowledge score was 7.35 (out of 12). Nurses' gender, primary role, genomics education, and experience caring for patients with common diseases influenced nurses' scores on genetic and genomics knowledge.

Conclusion

The genomics knowledge and competency of Chinese nurses need to be further improved. Incorporating genomics into the standard nursing curriculum can effectively reduce the knowledge gap. Simultaneously, it is also necessary to improve the attention of senior nurses and nursing managers to genomics nursing and improve the clinical practice environment.

Project description:BackgroundNurses' competency in pain management is essential for effectively alleviating patients' acute pain, controlling chronic pain, and promoting patient recovery. However, reliable tools for evaluating these competencies across different clinical specialties and healthcare settings are lacking. This study aimed to develop and validate a Pain Management Competency Scale for Nurses (PMCSN) and to assess the pain management competencies of nurses in China through a nationwide survey.MethodsAn item pool and a draft scale were developed through literature reviews, semi-structured interviews, and expert panel discussions. This was followed by refinement of the scale via Delphi expert consultations and a pilot test. To validate the scale, 342 nurses were conveniently sampled from six hospitals in Eastern and Central China. The validation process included item analysis, internal consistency reliability assessment, test-retest reliability (with 40 nurses retaking the questionnaire after a 14-day interval), content validity (evaluated by six experts using a 4-point Likert scale), and structural validity (assessed through exploratory and confirmatory factor analyses). The validated scale was then applied in a survey of 1,500 nurses from 15 hospitals across Eastern, Central, and Western China. Statistical analyses included descriptive statistics, analysis of variance (ANOVA), and t-tests.ResultsThe PMCSN comprised six primary dimensions-Pain Assessment and Monitoring, Pharmacological Pain Management, Non-Pharmacological Pain Management, Management of Analgesic Adverse Effects, Patient/Family Education, and Professional Development-and includes 52 tertiary items. The PMCSN scores ranged from 6 to 120, calculated by summing the standardized scores across the six dimensions, with higher scores indicating greater competency in pain management. The scale's Cronbach's α was 0.974 (dimension-specific values ranging from 0.863 to 0.935) and a test-retest reliability of 0.871. The content validity index (CVI) of the scale was 0.965. Exploratory factor analysis (EFA) showed that the six-factor model explained 67.50% of the variance. Confirmatory factor analysis (CFA) indicated good model fit, with average variance extracted (AVE) values ranging from 0.659 to 0.811 and composite reliability (CR) between 0.909 and 0.973, confirming good convergent validity. The square roots of the AVE values exceeded the inter-factor correlations, indicating good discriminant validity. In the nationwide survey, the average PMCSN score among 1,500 nurses was 101.27 ± 20.97. Nurses with higher education levels scored higher (F = 14.173, p < 0.01), as did those working in Eastern regions (F = 24.632, p < 0.01) and tertiary hospitals (T = -5.476, p < 0.01).ConclusionsThe PMCSN is a valid and reliable tool for assessing nurses' pain management competencies. It provides a standardized approach for evaluation and guides targeted interventions to improve competency. Regional and hospital-level disparities highlight the need for enhanced training in underdeveloped areas and collaboration between hospitals to promote balanced healthcare resources.

Project description:IntroductionEngaging with minority communities, such as immigrants and ethnic minorities, often involves adopting top-down approaches, wherein researchers and policymakers provide solutions based on their perspective. However, these approaches may not adequately address the needs and preferences of the community members, who have valuable insights and experiences to share. Therefore, community-engaged approaches, which involve collaborative partnerships between community members and researchers to identify issues, co-create solutions, and recommend policy changes, are becoming more recognized for their effectiveness and relevance. Yet, prevailing community engagement efforts often focus on easily reachable and already engaged segments of the community, sometimes overlooking the broader population.MethodsWhen working with immigrant and racialized communities, we encountered difficulties in engaging the wider community through traditional researcher-led approaches. We realized that overcoming these challenges required innovative strategies rooted in community-based participatory research principles and the diffusion of innovation theory. We recognized that a nuanced understanding of the community's dynamics and preferences was crucial in shaping our approach and building trust and rapport with the community members.ResultsThe need to bridge the gap between researcher-led initiatives and community-driven involvement has never been more pronounced. Our experience, chronicled in this article, highlights the journey of our research program with an immigrant/racialized community. This reflection enhances our comprehension of community engagement that deliberately strives to reach a larger cross-section of the community. Project description:BackgroundUnderstanding factors affecting adoption of an innovation is critical to its long-term success. Maternity waiting homes (MWHs) increase access to facility-based delivery in low-resourced settings; yet, quality issues deter utilization of this innovative approach. We sought to understand how attributes that are thought to promote diffusion of innovations (e.g., relative advantage, compatibility, observability, complexity, etc.) affected MWH use after implementation of an improved quality MWH model in rural Zambia compared to standard of care.MethodsWe conducted 158 in-depth interviews (IDIs) with randomly selected rural-living women who had delivered a baby in the prior 12 months. Half lived in catchment areas where new quality MWHs were constructed, half in catchment areas with standard of care (ranging from low quality community structures to no MWH). We applied content analysis to identify themes.ResultsUtilization of MWHs was higher among intervention (65.4%) than control women (42.5%). Respondents in both study arms perceived relative advantages to pregnant women staying at MWHs compared to going directly to health facilities when labor begins. MWH stays allowed for clinical staff to routinely check on and educate women, and address complications immediately. Compatibility of the homes with cultural values and needs depended on implementation. While some women from intervention sites complained about overcrowding, women in control sites more often perceived the lack of cleanliness, amenities, and safety as deterrents to utilization. Women at intervention sites received sensitization about MWHs from a wider range of sources, including traditional leaders. Required preparations needed to stay at MWHs (e.g. delivery supplies, food, and childcare) made adoption complex and may have deterred utilization.ConclusionsThe improved MWH model addressed most community concerns around quality. Having opinion leaders who communicate the relative advantage of MWHs to pregnant women and their social networks may facilitate MWH utilization. The complexity of decisions and resources needed to stay at MWHs remains a critical barrier to use. Project description:BackgroundSince Canadian drug regulatory approval of mifepristone for medical abortion in 2015 and its market availability in January 2017, the role of pharmacists in abortion provision has changed rapidly. We sought to identify the factors that influenced the initiation and provision of medical abortion from the perspectives of Canadian pharmacists, bridging two frameworks - Diffusion of Innovation in Health Service Organizations and integrated knowledge translation.MethodsWe conducted one-on-one semi-structured interviews with pharmacists residing in Canada who intended to stock and dispense mifepristone within the first year of availability. Our data collection, analysis, and interpretation were guided by reflexive thematic analysis and supported by an integrated knowledge translation partnership with pharmacy stakeholders.ResultsWe completed interviews with 24 participants from across Canada: 33% had stocked and 21% had dispensed mifepristone. We found that pharmacists were willing and able to integrate medical abortion care into their practice and that those who had initiated practice were satisfied with their dispensing experience. Our analysis indicated that several key Diffusion of Innovation constructs impacted the uptake of mifepristone, including: innovation (relative advantage, complexity and compatibility, technical support), system readiness (innovation-system fit, dedicated time, resources), diffusion and dissemination (expert opinion, boundary spanners, champions, social networks, peer opinions), implementation (external collaboration), and linkage. Participants' experiences suggest that integrated knowledge translation facilitated evidence-based changes to mifepristone dispensing restrictions, and communication of those changes to front line pharmacists.ConclusionsWe illustrate how Diffusion of Innovation and integrated knowledge translation may work together as complimentary frameworks for implementation science research. Unlike in the USA, UK, and other highly regulated settings globally, pharmacists in Canada are permitted to dispense mifepristone for medical abortion. We contribute to literature that shows that mifepristone dispensed outside of hospitals, clinics, and medical offices is safe and acceptable to both patients and prescribers. This finding is of particular importance to the current COVID-19 pandemic response and calls for continued and equitable access to abortion care in primary practice.
