Assessing the impact of recovery housing on healthcare utilization in Portland, Oregon.

ABSTRACT:

Introduction

Central City Concern (CCC) operates several recovery housing sites in the Portland, Oregon metropolitan region, including the Blackburn Center (Blackburn) and the Richard L. Harris Building (Harris). This retrospective, observational study was designed to assess recovery housing's impact on inpatient detoxification readmission rates and healthcare utilization patterns.

Methods

Our study population consisted of individuals discharged from CCC's Hooper Detox Stabilization Center from June 2019 to September 2020. A total of 75 clients housed at Blackburn, 63 clients housed at Harris, and 57 clients discharged as unhoused were included in the study sample. Using logistic regression for each of the two recovery housing groups relative to the unhoused group, we examined differences in readmissions to inpatient detoxification after their qualifying discharge. We then used Difference-In-Difference model to compare the per member per year (PMPY) use of different domains of health care before and after their qualifying discharge.

Results

Compared to clients discharged as unhoused, Blackburn and Harris residents had lower risk of readmissions to inpatient detoxification treatment at 90- and 180-days post-discharge. Additionally, while the mean number of PMPY emergency department visits increased for clients discharged as unhoused in the post period, the average number of emergency department visits decreased for clients who obtained recovery housing at Blackburn (DiD=-3.65 PMPY, p-value=0.02) and at Harris (DiD=-3.87 PMPY, p-value=0.01).

Conclusion

Findings highlight the impact and importance of recovery housing for individuals managing a substance use disorder and the value of healthcare system and public sector investment housing like Blackburn and Harris.

Project description:BackgroundPeople who are houseless (also referred to as homeless) perceive high stigma in healthcare settings, and face disproportionate disparities in morbidity and mortality versus people who are housed. Medical students and the training institutions they are a part of play important roles in advocating for the needs of this community. The objective of this study was to understand perceptions of how medical students and institutions can meet needs of the self-identified needs of the houseless community.MethodsBetween February and May 2018, medical students conducted mixed-methods surveys with semi-structured qualitative interview guides at two community-based organizations that serve people who are houseless in Portland, Oregon. Medical students approach guests at both locations to ascertain interest in participating in the study. Qualitative data were analyzed using thematic analysis rooted in an inductive process.ResultsWe enrolled 38 participants in this study. Most participants were male (73.7%), white (78.9%), and had been houseless for over a year at the time of interview (65.8%). Qualitative themes describe care experiences among people with mental health and substance use disorders, and roles for medical students and health-care institutions. Specifically, people who are houseless want medical students to 1) listen to and believe them, 2) work to destigmatize houselessness, 3) engage in diverse clinical experiences, and 4) advocate for change at the institutional level. Participants asked healthcare institutions to use their power to change laws that criminalize substance use and houselessness, and build healthcare systems that take better care of people with addiction and mental health conditions.ConclusionsMedical students, and the institutions they are a part of, should seek to reduce stigma against people who are houseless in medical systems. Additionally, institutions should change their approaches to healthcare delivery and advocacy to better support the health of people who are houseless.

Project description:Urban park managers are tasked with maintaining ecological function and quality of parks while also meeting visitor preferences. The purpose of this study was to better understand how managers currently manage vegetation in parks of Portland, Oregon. Twenty-one urban park manager interviews were completed regarding 15 parks, which included natural-passive use, recreational-active use, and multi-use park types. Responses were coded for themes and patterns of meaning. Mixed methods were used to evaluate the urban park manager interview data in the context of visitor interview and plant community composition data collected at the same parks. Nonmetric multidimensional scaling ordinations were used to identify urban park manager and visitor perspectives correlated with different park types and their vegetation. Across park types, managers discussed maintenance as a favorite aspect of plant management, while ecosystem management was often described by managers of natural-passive use parks. Some managers indicated that they would make no changes to plant management, but the majority provided detailed recommendations such as enhancing maintenance, increasing staffing, adding plants, updating infrastructure, and improving plant species selection. There are opportunities to better meet the preferences of both managers and visitors by continuing to maintain large trees and trail/path vegetation for accessibility, removing invasive/harmful plants, and improving plant selection to include those that are heartier, more colorful, produce flowers, and are disease resistant, climate adapted, and provide habitat for a variety of species. While urban park managers discussed how they incorporated visitor preferences and accessibility in plant management, they also described limitations such as funding, staff resources, and undesirable visitor behaviors. Increased communication and collaboration among governmental agencies, non-profit organizations, and community members, as well as continued investment in park management and interdisciplinary mixed methods research have the potential to enhance the many ecological and social benefits of urban parks.
