Voice of incarcerated male youths in Malawi's semi-urban prisons: a qualitative study.

ABSTRACT:

Objective

Incarcerated male youths in Malawi's detention facilities face multiple threats to their health and well-being. The most common challenges are overcrowding, poor sanitation and a lack of access to basic needs such as adequate food, bedding and healthcare. In addition, youth are vulnerable to sexual abuse.

Design

A qualitative approach using an exploratory study design was used to explore the experiences of incarcerated male youths in semi-urban prisons.

Setting

Two semi-urban prisons in Malawi.

Participants

Three focus group discussions (each with six participants) with incarcerated youths aged 18 were conducted.

Results

The following four themes emerged from the data: sexual violence perpetuated by other incarcerated persons; the need for sexual violence communication and prevention; trauma associated with incarceration and youth surviving prison life. The incarcerated male youths reported being given incentives such as extra food and adequate sleeping space in exchange for sexual favours with adult incarcerated people or incarcerated individuals with leadership positions. The victims of sexual abuse have had some difficulties reporting the incidences due to a non-existent reporting system for such matters to prison authorities.

Conclusions

This study reveals that the prison environment in Malawi is unsafe, particularly for incarcerated male youths who are at an increased risk of being victims of sexual violence. We recommend that the youths be separated from adults according to the Prison Rape Elimination Act standards. The prison officers should prevent incarcerated youth from having sight or physical contact with adult incarcerated people while in housing units. Policy-makers, prison authorities and researchers must safeguard and empower survivors of sexual violence in prisons.

Project description:IntroductionCanadian correctional institutions have been prioritized for COVID-19 vaccination given the multiple outbreaks that have occurred since the start of the pandemic. Given historically low vaccine uptake, we aimed to explore barriers and facilitators to COVID-19 vaccination acceptability among people incarcerated in federal prisons.MethodsThree federal prisons in Quebec, Ontario, and British Columbia (Canada) were chosen based on previously low influenza vaccine uptake among those incarcerated. Using a qualitative design, semi-structured interviews were conducted with a diverse sample (gender, age, and ethnicity) of incarcerated people. An inductive-deductive analysis of audio-recorded interview transcripts was conducted to identify and categorize barriers and facilitators within the Theoretical Domains Framework (TDF).ResultsFrom March 22-29, 2021, a total of 15 participants (n = 5 per site; n = 5 women; median age = 43 years) were interviewed, including five First Nations people and six people from other minority groups. Eleven (73%) expressed a desire to receive a COVID-19 vaccine, including two who previously refused influenza vaccination. We identified five thematic barriers across three TDF domains: social influences (receiving strict recommendations, believing in conspiracies to harm), beliefs about consequences (believing that infection control measures will not be fully lifted, concerns with vaccine-related side effects), and knowledge (lack of vaccine-specific information), and eight thematic facilitators across five TDF domains: environmental context and resources (perceiving correctional employees as sources of outbreaks, perceiving challenges to prevention measures), social influences (receiving recommendations from trusted individuals), beliefs about consequences (seeking individual and collective protection, believing in a collective "return to normal", believing in individual privileges), knowledge (reassurance about vaccine outcomes), and emotions (having experienced COVID-19-related stress).ConclusionsLack of information and misinformation were important barriers to COVID-19 vaccine acceptability among people incarcerated in Canadian federal prisons. This suggests that educational interventions, delivered by trusted health care providers, may improve COVID-19 vaccine uptake going forward.

Project description:BackgroundSince the novel coronavirus SARS-COV-2 was first identified to be circulating in the US on January 20, 2020, some of the worst outbreaks have occurred within state and federal prisons. The vulnerability of incarcerated populations, and the additional threats posed to the health of prison staff and the people they contact in surrounding communities underline the need to better understand the dynamics of transmission in the inter-linked incarcerated population/staff/community sub-populations to better inform optimal control of SARS-COV-2.MethodsWe examined SARS-CoV-2 case data from 101 non-administrative federal prisons between 5/18/2020 to 01/31/2021 and examined the per capita size of outbreaks in staff and the incarcerated population compared to outbreaks in the communities in the counties surrounding the prisons during the summer and winter waves of the SARS-COV-2 pandemic. We also examined the impact of decarceration on per capita rates in the staff/incarcerated/community populations.ResultsFor both the summer and winter waves we found significant inter-correlations between per capita rates in the outbreaks among the incarcerated population, staff, and the community. Over-all during the pandemic, per capita rates were significantly higher in the incarcerated population than in both the staff and community (paired Student's t-test p = 0.03 and p < 0.001, respectively). Average per capita rates of incarcerated population outbreaks were significantly associated with prison security level, ranked from lowest per capita rate to highest: High, Minimum, Medium, and Low security. Federal prisons decreased the incarcerated population by a relative factor of 96% comparing the winter to summer wave (one SD range [90%,102%]). We found no significant impact of decarceration on per capita rates of SARS-COV-2 infection in the staff community populations, but decarceration was significantly associated with a decrease in incarcerated per capita rates during the winter wave (Negative Binomial regression p = 0.015).ConclusionsWe found significant evidence of community/staff/incarcerated population inter-linkage of SARS-COV-2 transmission. Further study is warranted to determine which control measures aimed at the incarcerated population and/or staff are most efficacious at preventing or controlling outbreaks.
