Circular wood use can accelerate global decarbonisation but requires cross-sectoral coordination.

ABSTRACT: Predominantly linear use of wood curtails the potential climate-change mitigation contribution of forestry value-chains. Using lifecycle assessment, we show that more cascading and especially circular uses of wood can provide immediate and sustained mitigation by reducing demand for virgin wood, which increases forest carbon sequestration and storage, and benefits from substitution for fossil-fuel derived products, reducing net greenhouse gas emissions. By United Kingdom example, the circular approach of recycling medium-density fibreboard delivers 75% more cumulative climate-change mitigation by 2050, compared with business-as-usual. Early mitigation achieved by circular and cascading wood use complements lagged mitigation achieved by afforestation; and in combination these measures could cumulatively mitigate 258.8 million tonnes CO₂e by 2050. Despite the clear benefits of implementing circular economy principles, we identify many functional barriers impeding the structural reorganisation needed for such complex system change, and propose enablers to transform the forestry value-chain into an effective societal change system and lead to coherent action.

Project description:BackgroundPatients with cancer need coordinated care for both treatment and concurrent health conditions. This requires collaboration among specialists when using telemedicine services, emphasizing the importance of care continuity.ObjectiveThis study aimed to explore the effects of cross-sectorial video consultation involving oncologists, general practitioners, and patients with cancer on patients' perceived coordination of care, compared with usual care.MethodsThis study describes the primary outcomes from a 7-month follow-up of patients in the Partnership Project, a randomized clinical trial. Patients in the intervention group were randomized to receive a "partnership consultation," a shared video consultation with an oncologist, general practitioners, and the patient, in addition to their usual care. Questionnaires were completed for both groups at baseline and 7 months to assess the primary outcome, "global assessment of inter-sectorial cooperation," from the Danish questionnaire "Patients' attitude to the health care service." The questionnaire also included 2 single items and 5 index scales, examining patients' attitude toward cooperation in the health care system. Change in perceived global coordination from baseline to 7 months was compared between intention-to-treat groups using generalized estimating equations in a linear regression model.ResultsA total of 278 participants were randomized with 1:1 allocation, with 80 patients receiving the intervention. Further, 210 patients completed the questionnaire at baseline, while 118 responded at 7-month follow-up. The estimated difference in the primary outcome between usual care (-0.13, 95% CI -0.38 to 0.12) and intervention (0.11, 95% CI -0.11 to 0.34) was 0.24 (95% CI -0.09 to 0.58) and not statistically significant (P=.15).ConclusionsLow rates of intervention completion and high levels of missing data compromised the interpretability of our study. While we observed a high level of global assessment of coordination, the estimated intervention effect was smaller than anticipated, with no significant difference in perceived coordination between control and intervention groups. Future studies should explore strategies like patient incentives to increase response rate and improve the evaluation of this innovative health care model.

Project description:IntroductionSeveral Proficiency Test (PT) or External Quality Assessment (EQA) schemes are currently available for assessing the ability of laboratories to detect and characterize enteropathogenic bacteria, but they are usually targeting one sector, covering either public health, food safety or animal health. In addition to sector-specific PTs/EQAs for detection, cross-sectoral panels would be useful for assessment of the capacity to detect and characterize foodborne pathogens in a One Health (OH) perspective and further improving food safety and interpretation of cross-sectoral surveillance data. The aims of the study were to assess the cross-sectoral capability of European public health, animal health and food safety laboratories to detect, characterize and notify findings of the foodborne pathogens Campylobacter spp., Salmonella spp. and Yersinia enterocolitica, and to develop recommendations for future cross-sectoral PTs and EQAs within OH. The PT/EQA scheme developed within this study consisted of a test panel of five samples, designed to represent a theoretical outbreak scenario.MethodsA total of 15 laboratories from animal health, public health and food safety sectors were enrolled in eight countries: Denmark, France, Italy, the Netherlands, Poland, Spain, Sweden, and the United Kingdom. The laboratories analyzed the samples according to the methods used in the laboratory and reported the target organisms at species level, and if applicable, serovar for Salmonella and bioserotype for Yersinia.ResultsAll 15 laboratories analyzed the samples for Salmonella, 13 for Campylobacter and 11 for Yersinia. Analytical errors were predominately false negative results. One sample (S. Stockholm and Y. enterocolitica O:3/BT4) with lower concentrations of target organisms was especially challenging, resulting in six out of seven false negative results. These findings were associated with laboratories using smaller sample sizes and not using enrichment methods. Detection of Salmonella was most commonly mandatory to notify within the three sectors in the eight countries participating in the pilot whereas findings of Campylobacter and Y. enterocolitica were notifiable from human samples, but less commonly from animal and food samples.DiscussionThe results of the pilot PT/EQA conducted in this study confirmed the possibility to apply a cross-sectoral approach for assessment of the joint OH capacity to detect and characterize foodborne pathogens.
