Tinnitus, Suicide, and Suicidal Ideation: A Scoping Review of Primary Research.

ABSTRACT: Tinnitus (the perception of sound in the absence of any corresponding external source) is highly prevalent and can be distressing. There are unanswered questions about how tinnitus, suicidal thoughts, and suicidal behaviours co-occur and interact. To establish the extent of scientific literature, this scoping review catalogued primary reports addressing the associations between tinnitus, suicidal ideation, attempted suicide, and death by suicide. We searched OvidSP, Medline, EMBASE, PsycINFO, CINAHL, Google Scholar, EThoS, and ProQuest for all studies and case reports on ideation and/or attempted and/or completed suicide in the context of tinnitus. Twenty-three studies were included, and data were charted according to study type. Several epidemiological and other observational studies gave evidence of risk factors and an association between suicidal ideation, suicidal behaviour, and tinnitus. However, there was no evidence of the direction of causality. Qualitative studies are indicated to explore the patient's experience and understand the dynamics of any interaction between tinnitus and suicidal thoughts and behaviours. A theory-informed model of tinnitus and suicide needs to be developed to inform the development of interventions and how tinnitus patients are supported clinically.

Project description:IntroductionA core facilitator of the transition from suicidal thoughts to suicide attempt is the individual's capacity for suicide. Suicide capacity is a theoretically universal concept adaptable for specific groups that is hypothesised to comprise three contributing factors: acquired capability, for example, previous self-harm; dispositional, such as genetic influences and practical, knowledge of and access to lethal means. Given that suicide capacity as a concept is continuing to develop, a review and synthesis of the current literature is timely to ensure future research and development of suicide prevention strategies are based on evidential knowledge. The aim of this review is to map the available evidence to provide an overview of factors that contribute to an adult's capacity for suicide.Methods and analysisThis review will encompass five stages. Studies will be identified through broad search strings applied to 11 academic databases: Academic Search Ultimate, APA PsycArticles, APA PsycINFO, CINAHL, Psychology & Behavioural Sciences, & Sociology Source Ultimate via EBSCOHost Megafile Ultimate; PubMed; Science Direct; Wiley Online; Taylor & Francis and ProQuest dissertations and theses. Grey literature databases and key suicide organisations will also be searched for relevant literature. Two reviewers will independently screen titles and abstracts then review full texts to identify articles meeting inclusion criteria. Articles will be assessed for eligibility based on suicide attempt history, primary research study design, language and publication date. Data from eligible full texts will be extracted using a predesigned template for analysis. The synthesisation method will be textual narrative synthesis with an incorporated quality appraisal checklist tool.Ethics and disseminationEthics approval is not required for this scoping review as no human participants are involved. Study findings will be shared with key suicide organisations, through peer-reviewed publications, and conference presentations.

Project description:ImportanceReliable estimates of the prevalence and risk of suicide and suicidal ideation (SI) among childhood cancer survivors (CCSs) are crucial in informing efforts to address this issue.ObjectiveTo systematically evaluate the prevalence and risk of suicide and SI among CCSs.Data sourcesA literature search was conducted of PubMed, Embase, Cochrane Library, and PsycINFO from January 1, 2000, to November 17, 2024.Study selectionAll noncontrolled and controlled studies that evaluated the prevalence and risk of suicide and SI and drew comparisons between CCSs and their counterparts. Studies including participants older than 25 years, no reported outcomes, case studies, case reviews, and review articles were excluded.Data extraction and synthesisThe Preferred Reporting Items for Systematic Reviews and Meta-analyses reporting guideline was followed with prospective PROSPERO registration. Prevalence was pooled for single-arm studies. Relative risk ratio (RR) was pooled for double-arm studies. Metaprop was used to meta-analyze the prevalence of suicide and SI under a generalized linear mixed model. For dichotomous outcomes, meta-analyses were performed to compute the RR of the psychological outcome compared with controls.Main outcomes and measuresPrevalence of suicide and SI, the RR of suicide and SI among CCSs compared with controls, and risk factors associated with suicidality outcomes.ResultsFrom 531 records, 16 studies were included. Studies on suicide included 148 869 participants and studies on SI included 20 140 participants. The prevalence of suicide among CCSs was 0.30% (0.13%-0.69%) and of SI was 9% (95% CI, 7%-11%). There was a significantly increased risk of SI among CCSs compared with controls (RR, 1.67 [95% Cl, 1.39-2.01]), but not of suicide (RR, 1.50 [95% CI, 0.63-3.62]). Subgroup analyses suggested that prevalence of SI is much higher during the active cancer period than years after (proportion, 0.14 [95% CI, 0.10-0.19] vs 0.08 [95% CI, 0.06-0.09]). The systematic review identified preexisting mental illness, poor physical health, older age, male sex, and being single as risk factors associated with suicidality outcomes.Conclusions and relevanceIn this meta-analysis of suicide and SI among CCSs, the risk of suicide was no different among CCSs compared with controls, but CCSs had a higher risk of SI. Future studies are needed to explore suicidality outcomes among CCSs to guide targeted support policies.
