Consumer purchase data (CPD) is a promising instrument to assess the impact of purchases on health, but is limited by the need for manual scanning, a lack of access to data from multiple retailers, and limited information on product data and health outcomes. Project description:BackgroundPrevention and management of childhood overweight involves the entire family. We aimed to investigate purchase patterns in households with at least one member with overweight in childhood by describing expenditure on different food groups.MethodsThis Danish register-based cohort study included households where at least one member donated receipts concerning consumers purchases in 2019-2021 and at least one member had their Body mass index (BMI) measured in childhood within ten years prior to first purchase. A probability index model was used to evaluate differences in proportion expenditure spent on specific food groups.ResultsWe identified 737 households that included a member who had a BMI measurement in childhood, 220 with overweight and 517 with underweight or normal weight (reference households). Adjusting for education, income, family type, and urbanization, households with a member who had a BMI classified as overweight in childhood had statistically significant higher probability of spending a larger proportion of expenditure on ready meals 56.29% (95% CI: 51.70;60.78) and sugary drinks 55.98% (95% CI: 51.63;60.23). Conversely, they had a statistically significant lower probability of spending a larger proportion expenditure on vegetables 38.44% (95% CI: 34.09;42.99), compared to the reference households.ConclusionHouseholds with a member with BMI classified as overweight in childhood spent more on unhealthy foods and less on vegetables, compared to the reference households. This study highlights the need for household/family-oriented nutrition education and intervention.

Project description:PurposeThe Health, Food, Purchases and Lifestyle (SMIL) cohort is a prospective open Danish cohort that collects electronic consumer purchase data, which can be linked to Danish nationwide administrative health and social registries. This paper provides an overview of the cohort's baseline characteristics and marginal differences in the monetary percentage spent on food groups by sex, age and hour of the day.ParticipantsAs of 31 December 2022, the cohort included 11 214 users of a smartphone-based receipt collection application who consented to share their unique identification number for linkage to registries in Denmark. In 2022, the composition of the cohort was as follows: 62% were men while 24% were aged 45-55. The cohort had a median of 63 (IQR 26-116) unique shopping trips. The cohort included participants with a range of health statuses. Notably, 21% of participants had a history of cardiovascular disease and 8% had diabetes before donating receipts.Findings to dateThe feasibility of translating consumer purchase data to operationalisable food groups and merging with registers has been demonstrated. We further demonstrated differences in marginal distributions which revealed disparities in the amount of money spent on various food groups by sex and age, as well as systematic variations by the hour of the day. For example, men under 30 spent 8.2% of their total reported expenditure on sugary drinks, while women under 30 spent 6.5%, men over 30 spent 4.3% and women over 30 spent 3.9%.Future plansThe SMIL cohort is characterised by its dynamic, continuously updated database, offering an opportunity to explore the relationship between diet and disease without the limitations of self-reported data. Currently encompassing data from 2018 to 2022, data collection is set to continue. We expect data collection to continue for many years and we are taking several initiatives to increase the cohort.

Project description:BackgroundThe development of motor skills in infancy is a vital neurodevelopmental milestone. Although previous studies have explored the neurotoxic effects of agricultural pesticides on infants' motor development, limited research has examined early postnatal household pesticide use on infants' motor development, particularly among urban communities.ObjectiveThis study examined the association between early postnatal household pesticide use and infants' gross and fine motor development at 6 months of age.MethodsQuestionnaires were administered via telephone to 296 mother-infant dyads in the Maternal and Developmental Risks from Environmental and Social Stressors (MADRES) pregnancy cohort. Early life household pesticide use was assessed via questionnaire administered when infants turned 3 months old and gross and fine motor development was assessed by the Ages and Stages Questionnaire (ASQ-3) at 6 months old. Infant gross motor scores were reverse coded so that higher scores indicated lower gross motor performance. Negative binomial regressions were performed to assess the relationship between household pesticide use and infant gross motor development.ResultsInfants were predominantly Hispanic (78.7%) and full term (gestational age at birth: 39.0 ± 1.9 weeks), with 22.3% of maternal participants reporting household use of rodent and insect pesticides. Adjusting for recruitment site, maternal age, ethnicity, household income, education, infant corrected age, infant sex, and home type, infants with maternal-reported household use of rodent and insect pesticides had 1.30 times higher expected gross motor scores (95% confiidence interval 1.05, 1.61) than infants with no reported use of household pesticides, with higher scores indicating reduced gross motor performance.ConclusionsOur results suggest household use of rodent and insect pesticides may harm infants' gross motor development in early childhood. Future research should evaluate the impact of specific household chemicals in infant biospecimens and their associations with infant motor development to confirm these findings.

Project description:When agencies such as the US Environmental Protection Agency (EPA) establish future greenhouse gas emissions standards for new vehicles, forecasting future vehicle purchases due to changes in fuel economy and prices provides insight into regulatory impacts. We compare predictions from a nested logit model independently developed for US EPA to a simple model where past market share predicts future market share using data from model years 2008, 2010, and 2016. The simple model outperforms the nested logit model for all goodness-of-prediction measures for both prediction years. Including changes in vehicle price and fuel economy increases bias in forecasted market shares. This bias suggests price increases are correlated with unobserved increases in vehicle quality, changes in preferences, or brand-specific changes in market size but not cost pass-through. For 2010, past shares predict better than a nested logit model despite a major shock, the economic disruption caused by the Great Recession. Observed share changes during this turbulent period may offer upper bounds for policy changes in other contexts: the largest observed change in market share across the two horizons is 6.6% for manufacturers in 2016 and 3.4% for an individual vehicle in 2010.

Project description:ObjectivesExamine correlates of initiation of e-cigarette use among smokers and determine the impact of e-cigarette use on cessation among smokers in a national U.S. consumer panel.MethodsThis study used the Nielsen Homescan Panel data from 2011 to 2013, augmented with state-specific measures of tobacco control activities, to examine 1) correlates of single and repeat e-cigarette purchasing among panelists currently purchasing cigarettes; and 2) correlates of "cessation". Participating panelists scanned all retail purchases, and Nielsen recorded over 3 million product types. The key explanatory variable for cessation was e-cigarette purchase. Parallel analysis was conducted for conventional nicotine replacement therapy (NRT) purchase. Cessation was defined as no purchases for at least 6 months and no subsequent purchases until the end of 2013. Analysis was conducted in 2015. E-cigarettes tracked by Nielsen during this period were cig-a-like products resembling tobacco cigarettes in appearance.ResultsSingle e-cigarette purchase was associated with whether the panelist resided in a single person male household and bought a higher volume of cigarettes. Repeat purchase was associated with higher state cigarette taxes, less stringent state public smoke-free policies, lower cigarette prices, and more frequent cigarette purchasing. Cessation was associated with repeat e-cigarette purchasing, repeat NRT purchasing, younger age, lower monthly cigarette volume, less frequent purchasing of cigarettes, less recent cigarette purchase at baseline, and single e-cigarette purchase before baseline.ConclusionsBoth individual and policy variables were associated with e-cigarette use. Repeat e-cigarette purchase was associated with cigarette purchase discontinuation, as were various smoking intensity measures.

Project description:IntroductionLittle is known about where households shop for packaged foods, what foods and beverages they purchase, and the nutrient content of these purchases. This study describes volume trends and nutrient content (nutrient profiles, food and beverage groups) of household packaged foods purchases (PFPs) by store type.MethodsCross-sectional analysis of U.S. households' PFPs (Nielsen Homescan) from 2000 to 2012 (N=652,023 household-year observations) with survey weights used for national representativeness. Household PFP trends (% volume), household purchases of key food and beverage groups based on caloric contribution, and mean caloric and nutrient densities (sugars, saturated fat, and sodium) of household PFPs were analyzed by store type. Data were collected from 2000 to 2012. Analyses were conducted in 2014-2015.ResultsThe proportion of total volume of household PFPs significantly increased from 2000 to 2012 for mass merchandisers (13.1% to 23.9%), convenience stores (3.6% to 5.9%), and warehouse clubs (6.2% to 9.8%), and significantly decreased for grocery chains (58.5% to 46.3%) and non-chain grocers (10.3% to 5.2%). Top common sources of calories (%) from household PFPs by food/beverage group included: savory snacks, grain-based desserts, and regular soft drinks. The energy, total sugar, sodium, and saturated fat densities of household PFPs from mass merchandisers, warehouse clubs, and convenience stores were higher compared with grocery stores.ConclusionsPFPs from stores with poorer nutrient density (more energy, total sugar, sodium, and saturated fat-dense), such as warehouse clubs, mass merchandisers, and convenience stores are growing, representing a potential U.S. public health concern.

Project description:Environmental changes resulting from human activity and the negative impact of civilisational megatrends are being noticed and criticised increasingly often, and their consequences are becoming extremely severe. If people do not change their habits, changes in our ecosystems will become irreversible and it will be impossible to live in such environment. Thus, the aim of the paper is to review the lifestyles of responsible consumers against the background of the sustainable development paradigm. To engage in the debate as to how a sustainable lifestyle can be operationalized, we conducted a traditional, narrative literature review. Apart from revising the theoretical framework of a sustainable lifestyle, we describe selected lifestyles (such as lifestyle of health and sustainability, wellness, hygge, lagom, slow living, smart living, low-carbon lifestyles) and consumer behaviour patterns (fair trade, values and lifestyle segmentation). Each of these lifestyles relates to a broader or narrower extent to sustainable development, but none of the lifestyles is universal. Conscious and responsible consumer behaviour requires a long-term process and to a large extent depends on individual, political and marketing factors. Finally, we made an evaluation of the research used, pointing out challenges to be implemented, which will contribute to the development, enhancement and prominence of a sustainable lifestyle.

Project description:Fragranced consumer products, such as cleaning supplies, air fresheners, and personal care products, are a primary source of indoor air pollutants and personal exposure. Previous research indicates that fragranced products can trigger adverse health effects, with implications for workplaces and public places. This is the first study to examine the multiple dimensions of exposures related to fragranced products and effects in the US population. The study investigated the prevalence and types of fragranced product exposures, associated health effects, awareness of product emissions, and preferences for fragrance-free policies and environments. Data were collected using an online survey with a nationally representative population (n = 1136) of adults in the USA. Overall, 34.7 % of the population reported health problems, such as migraine headaches and respiratory difficulties, when exposed to fragranced products. Further, 15.1 % have lost workdays or a job due to fragranced product exposure in the workplace. Also, 20.2 % would enter a business but then leave as quickly as possible if they smell air fresheners or some fragranced product. Over 50 % of the population would prefer that workplaces, health care facilities and professionals, hotels, and airplanes were fragrance-free. While prior research found that common fragranced products, even those called green and organic, emitted hazardous air pollutants, more than two thirds of the population were not aware of this, and over 60 % would not continue to use a fragranced product if they knew it emitted such pollutants. Results from this study provide strong evidence that fragranced products can trigger adverse health effects in the general population. The study also indicates that reducing exposure to fragranced products, such as through fragrance-free policies, can provide cost-effective and relatively simple ways to reduce risks and improve air quality and health.

Project description:Taxes on unhealthy foods and sweetened beverages, as well as subsidies to healthy foods, have become increasingly popular strategies to curb obesity and related non-communicable diseases. The existing evidence on the welfare effects of such fiscal policies is mixed and almost uniquely focused on tax schemes. Using the 2016-2017 Chilean Household Budget Survey, we estimate a censored Exact Affine Stone Index (EASI) incomplete demand system and simulate changes in purchases, tax incidence, and consumer welfare of three different policy scenarios: (1) a 5 percentage point additional tax on sweetened beverages (currently taxed at 18%) and a new 18% tax on sweets and snacks, (2) a healthy subsidy by zero-rating fruits and vegetables from the current 19% value-added tax, and (3) a combined (tax plus subsidy) policy. Under full pass-through of these policies, the combined scheme captures the incentives to switch purchases from both single-policy alternatives, resulting in a net welfare gain and subsidy transfer for the average Chilean household. In terms of welfare, low-income households strictly benefit from a combined policy, while high-income households experience a small consumer welfare loss, resulting in re-distributional effects.

Project description:ObjectiveFood insecurity is positively associated with asthma, the most common chronic childhood disease, yet directionality is unclear. The objective was to determine the association between exposure to food insecurity in early childhood and the odds of asthma later in childhood.DesignData from four waves of the Early Childhood Longitudinal Study-Kindergarten (ECLS-K) cohort, a prospective, dual-frame, multistage probability cluster sampling study of school-aged US children were entered in multivariate logistic regression models, adjusted for covariates. Exposures to food insecurity were based on parental responses to the validated USDA 18-item module at each wave.SettingPublic and private primary and secondary schools between 1998 and 2007.ParticipantsAt its inception (1999), the ECLS-K had 20 578 kindergarteners; by the spring of eighth grade (2007), the cohort dropped to 9725 due to attrition. Children missing an exposure, outcome or confounding variable were excluded, final n=6731.Primary outcome measureChild's diagnosis of asthma by a healthcare professional as reported by the parent.ResultsHousehold food insecurity (vs food security) in the year before kindergarten and in second grade had a higher odds of asthma by 18% (95% CI 1.17 to 1.20) and 55% (95% CI 1.51 to 1.55). After removing asthmatics before third grade from the model, food insecurity in second grade was associated with higher odds of asthma at fifth or eighth grades (OR 1.55; 95% CI 1.53 to 1.58), whereas food insecurity in the year before kindergarten had a lower odds at fifth or eighth grades.ConclusionsFood insecurity in the year before kindergarten and in second grade were associated with a higher odds of asthma in third grade. Food insecurity in second grade retained the signal for increased odds of asthma after third and through eighth grades. Additional research is needed to explore childhood windows of vulnerability to asthma.
