Impacts of visual impairment on pragmatic impairment: A systematic review and meta-analysis.

ABSTRACT:

Background

Consideration for patients with visual impairment, from low vision to blindness, is an important part of building a barrier-free society. Some authors have elaborated that visual impairment can indeed lead to delayed development in theory of mind, thereby causing pragmatic knowledge deficiency. Verifying whether those with eye conditions have pragmatic impairment is an essential way for their clinical evaluation, intervention and rehabilitation.

Objective

We primarily carry out a meta-analysis of visual impairment from low vision to blindness and pragmatic impairment in people with low vision or blindness to verify visual impairment may cause pragmatic impairment.

Data sources

Electronic databases Pubmed, Medline, MesH, Psychinfo, Ovid, EBSCO and CNKI and the reference sections of previous reviews.

Study eligibility criteria

Studies were included when they built on primary data from clinical questionnaire surveys or field trials anywhere in the world, and when they reported impacts of visual impairment on social cognition, communication, skills, behavior and intelligence. In total, 25 original studies were included, in which 25735 people were evaluated.

Results

Statistically, visual impairments and pragmatic impairment exist correlation due to the significant p value(p = 0.0005 < 0.05) in group and the subgroup sorted in the light of 18 years old (p < 0.0001 and p = 0.003 < 0.05). Psychologically, because people with visual impairment can not normally get non-verbal information, they can not get a complete pragmatic knowledge system. Pragmatic knowledge deficiency leads to abnormal in executive functions and development delay from the perspective of theory of mind, inducing pragmatic impairment. Therefore, visual impairment has an impact on pragmatic impairment.

Conclusion

The meta-analysis reveals robust evidence on the relationship of vision impairment and pragmatic impairment in children or adults. Such evidence may help to gradually improve the clinical evaluation, intervention and rehabilitation of these people.

Project description:BackgroundVisual impairment is a public health problem in both developing and developed countries, especially, in developing countries where most visually impaired communities are found. Knowledge of the pooled prevalence of visual impairment among adults is useful in planning, preventive programs and the provision of eye-care services for communities.MethodsOriginal observational studies published in English were included in this systematic review and meta-analysis. Eleven studies with a total sample size of 8,161 study participants were included. PubMed/Medline, HINARI, Google Scholar, Cochrane library, Web of Science, and African Journals Online databases were used to search for published articles. Data were extracted on a Microsoft Excel spreadsheet and analyzed using Revman 5.4.1 Software. Meta-analysis of further pooled prevalence estimates using the inverse variance heterogeneity model. The pooled estimate of visual impairment in the current review was estimated using a random-effects model. Forest plots were used to illustrate heterogeneity and to quantify the pooled prevalence of visual impairment. Publication bias was assessed using funnel plots. Visual impairment was defined as a presenting visual acuity of less than 6/18 in the betting-seen eye.ResultsA total of 538 studies were identified from several Databases and digital libraries, of which eleven articles were selected for the final meta-analysis. Significant heterogeneity was observed across studies, suggesting a random-effects model to estimate the pooled prevalence of visual impairment. The prevalence of visual impairment in Ethiopia ranges from 10.3% in Addis Ababa central Ethiopia to 37.58% in the Northern Ethiopia. The pooled prevalence of visual impairment in the current review was 22% (95% CI: 17%-27%; I2 = 97%) using a random-effects model. There was also evidence of symmetry in the funnel plots.ConclusionThis systematic review and meta-analysis demonstrated a greater burden of visual impairment among Ethiopians in various study populations. Further, intervention strategies are required to reduce the burden of visual impairment and improve quality of life.

Project description:BackgroundSmartphone overuse has been cited as a potentially modifiable risk factor that can result in visual impairment. However, reported associations between smartphone overuse and visual impairment have been inconsistent.ObjectiveThe aim of this systematic review was to determine the association between smartphone overuse and visual impairment, including myopia, blurred vision, and poor vision, in children and young adults.MethodsWe conducted a systematic search in the Cochrane Library, PubMed, EMBASE, Web of Science Core Collection, and ScienceDirect databases since the beginning of the databases up to June 2020. Fourteen eligible studies (10 cross-sectional studies and 4 controlled trials) were identified, which included a total of 27,110 subjects with a mean age ranging from 9.5 to 26.0 years. We used a random-effects model for meta-analysis of the 10 cross-sectional studies (26,962 subjects) and a fixed-effects model for meta-analysis of the 4 controlled trials (148 subjects) to combine odds ratios (ORs) and effect sizes (ES). The I2 statistic was used to assess heterogeneity.ResultsA pooled OR of 1.05 (95% CI 0.98-1.13, P=.16) was obtained from the cross-sectional studies, suggesting that smartphone overuse is not significantly associated with myopia, poor vision, or blurred vision; however, these visual impairments together were more apparent in children (OR 1.06, 95% CI 0.99-1.14, P=.09) than in young adults (OR 0.91, 95% CI 0.57-1.46,P=.71). For the 4 controlled trials, the smartphone overuse groups showed worse visual function scores compared with the reduced-use groups. The pooled ES was 0.76 (95% CI 0.53-0.99), which was statistically significant (P<.001).ConclusionsLonger smartphone use may increase the likelihood of ocular symptoms, including myopia, asthenopia, and ocular surface disease, especially in children. Thus, regulating use time and restricting the prolonged use of smartphones may prevent ocular and visual symptoms. Further research on the patterns of use, with longer follow up on the longitudinal associations, will help to inform detailed guidelines and recommendations for smartphone use in children and young adults.

Project description:PurposeThe aim was to compare fatigue levels between patients with visual impairment and controls with normal sight and to examine the association between fatigue and vision loss severity.MethodsA systematic literature search was performed using databases of PubMed, Embase, PsycINFO and Cochrane to identify observational studies with outcomes related to fatigue (e.g. vitality subscale of the Short-Form 36, Fatigue Assessment Scale). A meta-analysis was performed using standardised mean differences (SMDs) and odds ratios (OR) to quantitatively summarise the association between visual impairment and fatigue. Sources of heterogeneity were explored by subgroup and sensitivity analyses. Study quality was assessed with the Newcastle-Ottawa scale.ResultsAfter reviewing 4477 studies, 22 studies with a total of 40 004 participants were included, of which 18 contributed to meta-analysis. Among these, eight were assessed as moderate quality studies and 10 as high quality studies. Pooled analysis involving 2500 patients and 8395 controls showed higher fatigue severity levels (S.M.D. = -0.36, 95% CI -0.50 to -0.22, 14 studies) among visually impaired patients compared to normally sighted controls. This effect size was small and persisted in sensitivity analyses that involved study quality, fatigue assessment tools and visual acuity data. Furthermore, pooled analysis of four studies including 2615 patients and 5438 controls showed a significant association between visual impairment and fatigue (OR = 2.61, 95% CI 1.69 to 4.04). Secondary meta-analysis of four studies showed no significant difference in fatigue severity (S.M.D. = 0.01, 95% CI -0.37 to 0.39) between patients with moderate visual impairment and patients with severe visual impairment or blindness.ConclusionsCurrent moderate to high quality evidence suggest that patients with visual impairment experience more severe fatigue symptoms than persons with normal sight. However, a limited number of available studies indicates that fatigue is not associated with severity of vision loss. Future studies are required to determine which factors and underlying mechanisms may explain the association between visual impairment and fatigue. Discussing fatigue at an early stage and developing intervention options for vision-related fatigue should be considered within the field of low vision rehabilitation.

Project description:IntroductionSuicide is an important public health problem. Well-established risk factors of suicide include depression, family history of mental disorders, substance problem, chronic physical illness, and others. Sensory impairment, especially visual impairment (VI), has a critical impact on both mental and physical health. However, the association between VI and risk of suicide has not been thoroughly investigated and remains controversial. Our aim is to systematically review and meta-analyze the current evidence on the association between VI and risk of suicide and to evaluate the direction and magnitude of the association.Methods and analysisWe aim to search PubMed, EMBASE and the Cochrane Library to identify all population-based studies on the association between VI and risk of suicide. Two reviewers will independently conduct study selection, data extraction and risk of bias (ROB) assessment. The Newcastle-Ottawa scale will be applied to evaluate the methodologic quality of the included studies for ROB assessment. The primary outcome measure will be the relative risk (RR) of suicide, and the secondary outcome measures will be the risks of suicidal ideation (SI) and suicide attempt (SA). Estimates of risk with 95% confidence intervals (CIs) for suicide, SI and SA, respectively, will be calculated and summarized. We will perform random-effects meta-analyses to combine the pooled effects. Meta-regression will be applied to investigate the effects of multiple factors across studies. Subgroup and sensitivity analyses will be conducted for screening of any potential sources of heterogeneity. Publication bias will be evaluated by funnel plot and Begg and Mazumdar correlation testing. The body of evidence will be assessed using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) approach.ConclusionThis article presents a study protocol for investigating the association between VI and risk of suicide. The findings of this study will contribute to our current knowledge of the impact of VI as a risk factor of suicide. In addition, meta-regression and subgroup analyses will provide further insights to factors affecting the association between VI and suicide risk.Trial registrationSystematic review registration: PROSPERO CRD 42022325106.

Project description:IntroductionVisual impairment is a major public health problem in developing countries where there is no enough health-care service. It has a significant impact on the affected child's psychological, educational and socioeconomic experiences, during childhood and beyond. Therefore, the aim of this review was to estimate the pooled prevalence of visual impairment and its associated factors among children in Ethiopia.MethodThis systematic review and meta-analysis was designed based on the PRISMA guidelines. Relevant published articles in Ethiopia from 2011-2021 were searched in PubMed/Medline, HINARI, Google scholar, and conference paper and thesis or research final reports were accessed from Ethiopian Universities' repositories. Data was extracted in Microsoft excel by using JBI data extraction checklist. The pooled prevalence and odds ratio of associated factors with their 95% CI was computed by using STATA 14/SE software. A fixed effect meta-analysis model was employed for a Cochrane Q test statistic and I2 test showed there was no heterogeneity in the included studies.ResultA total of 7,647 children from nine studies were included in this study. The overall prevalence of visual impairment among children in Ethiopia was 7% (95% CI: 6, 7%). The pooled prevalence of visual impairment by region was almost similar in Ethiopia. However, there was no significant association between the identified factors and visual impairment among children. But the result showed that being males (AOR 0.642, 95% CI: 0.357-1.156), Children in the age of 10-13 years (AOR 0.224, 95% CI: 0.046-1.102) and 14-18 years (AOR 0.508, 95% CI: 0.102-2.534) were found to be less likely to have visual impairment. On the other hand, children of parents with visual impairment (AOR 1.820, 95% CI: 0.381-8.698) more likely to have visual impairment.ConclusionVisual impairment among children in Ethiopia is still a public health problem one year later to VISION 2020, a global initiative aimed to eliminate avoidable blindness. All most one out of fourteen children in Ethiopia had visual impairment. Therefore, the government of Ethiopia should focus on effective, efficient, comprehensive eye health care services by integrating with the national health system to prevent avoidable visual impairment among children.

Project description:BackgroundOcular toxoplasmosis (OT) is caused by the parasite Toxoplasma gondii. OT is the leading cause of posterior uveitis globally; it is a recurrent disease that may result in visual impairment and blindness. This systematic review and meta-analysis aim to summarize and evaluate the risk factors for recurrences, visual impairment, and blindness described in the literature worldwide.Methods and findingsWe performed a systematic literature search in PubMed, Embase, VHL, Cochrane Library, Scopus, and DANS EASY Archive. All studies reporting patients with clinically and serologically confirmed OT presenting any clinical or paraclinical factor influencing recurrences, visual impairment, and blindness were included. Studies presenting secondary data, case reports, and case series were excluded. An initial selection was made by title and abstract, and then the studies were reviewed by full text where the eligible studies were selected. Then, the risk of bias was assessed through validated tools. Data were extracted using a validated extraction format. Qualitative synthesis and quantitative analysis were done. This study was registered on PROSPERO (CRD42022327836).ResultsSeventy two studies met the inclusion criteria. Fifty-three were summarized in the qualitative synthesis in three sections: clinical and environmental factors, parasite and host factors, and treatment-related factors. Of the 72 articles, 39 were included in the meta-analysis, of which 14 were conducted in South America, 13 in Europe, four in Asia, three multinational, two in North America and Central America, respectively, and only one in Africa. A total of 4,200 patients with OT were analyzed, mean age ranged from 7.3 to 65.1 year of age, with similar distribution by sex. The frequency of recurrences in patients with OT was 49% (95% CI 40%-58%), being more frequent in the South American population than in Europeans. Additionally, visual impairment was presented in 35% (95% CI 25%-48%) and blindness in 20% (95% CI 13%-30%) of eyes, with a similar predominance in South Americans than in Europeans. On the other hand, having lesions near the macula or adjacent to the optic nerve had an OR of 4.83 (95% CI; 2.72-8.59) for blindness, similar to having more than one recurrence that had an OR of 3.18 (95% CI; 1.59-6.38). Finally, the prophylactic therapy with Trimethoprim/Sulfamethoxazole versus the placebo showed a protective factor of 83% during the first year and 87% in the second year after treatment.ConclusionOur Systematic Review showed that clinical factors such as being older than 40 years, patients with de novo OT lesions or with less than one year after the first episode, macular area involvement, lesions greater than 1 disc diameter, congenital toxoplasmosis, and bilateral compromise had more risk of recurrences. Also, environmental and parasite factors such as precipitations, geographical region where the infection is acquired, and more virulent strains confer greater risk of recurrences. Therefore, patients with the above mentioned clinical, environmental, and parasite factors could benefit from using prophylactic therapy.
