Development and validation of a fertility information support scale for reproductive-aged patients with breast cancer.

ABSTRACT:

Objective

This study aims to develop and validate a suitable scale for assessing the level of fertility information support in reproductive-aged breast cancer patients.

Methods

We utilized an exploratory mixed methods approach, dividing the study into two phases: scale development and validation. In Phase 1, initial items were generated through literature review and qualitative interviews. In Phase 2, a cross-sectional survey was conducted involving 468 reproductive-aged breast cancer patients to evaluate the scale's validity and reliability. Scale items underwent item analysis, and their validity and reliability were assessed using Cronbach's α coefficient, retest reliability, content validity, structural validity, and convergent validity.

Results

The final scale comprised 17 items, and exploratory factor analysis revealed three common dimensions with internal consistency coefficients of 0.894, 0.816, and 0.869. The overall internal consistency of the scale was 0.908, with a retest reliability Pearson correlation coefficient of 0.937, indicating strong stability and reliability. The construct validity of the scale was excellent (χ² = 198.606, df = 113, χ²/df = 1.758, root mean square of approximation error = 0.055, goodness-of-fit index = 0.917, comparative fit index = 0.979, null fitting index = 0.953, and incremental fitting index = 0.979).

Conclusions

Our fertility information support scale for reproductive-aged breast cancer patients demonstrates favorable psychometric properties. It serves as a valuable tool for evaluating fertility information support levels among these patients, assisting medical professionals in addressing fertility concerns and implementing personalized support programs. However, further research is needed to investigate the applicability of this scale in patients from diverse social and cultural backgrounds.

Project description:Social networks play an important role in smoking. Provision of social support during cessation is a cornerstone of treatment. Online social networks for cessation are ubiquitous and represent a promising modality for smokers to receive and provide the support necessary for cessation. There are no existing measures specific to online social support for smoking cessation.The objective was to develop a measure of social support to be used in online smoking cessation treatment research.Initial items for the Online Social Support for Smokers Scale (OS4) were based on existing theory and scales delineated in various taxonomies. Preliminary field analysis (N = 73) was conducted on 23 initial items to optimize the scale. Further development was conducted on a refined 15-item scale in the context of a large randomized trial of Internet and telephone cessation treatment with follow-ups at 3, 6, 12, and 18 months. In all, 1326 participants were randomized to an enhanced Internet arm that included a large online social network; psychometric analyses employed 3-month follow-up data from those reporting use of the enhanced Internet intervention at least once (n = 873). Items were subjected to a factor analysis, and the internal consistency reliability of the scale was examined along with construct and criterion validity. Other measures used in the study included demographics, nicotine dependence, partner support for cessation, general social support, social integration, stress, depression, health status, online community use, Internet use behaviors, intervention satisfaction, and 30-day point prevalence abstinence.The final 12-item OS4 scale demonstrated high internal consistency reliability (Cronbach alphas .86-.89) across demographic and smoking strata of interest. The OS4 also demonstrated good construct and criterion validity, with the directionality of the observed associations providing support for most a priori hypotheses. Significant Pearson correlations were observed between the OS4 and the Partner Interaction Questionnaire (PIQ) Positive subscale (? = .24, P < .001). As hypothesized, participants with the highest OS4 scores were more likely to have actively participated in the enhanced Internet community and to have high levels of satisfaction with the enhanced Internet intervention. In logistic regression analyses, the OS4 was highly predictive of 30-day point-prevalence abstinence at 6, 12, and 18 months (all P values <.001). The odds of abstinence at 6 months rose by 48% for each standard unit increase in online social support (95% confidence interval [CI] 1.17-1.71), dropping only slightly to 37% at 12 and 18 months (95% CI 1.17-1.59).The OS4 is a brief, reliable, and valid instrument for measuring online social support for smoking cessation. Results should be replicated and extended, but this study suggests the OS4 can be used to advance theory, understand mechanisms, and potentially help to improve the tailoring of Internet-based smoking cessation treatments. Project description:BackgroundApproximately half of premenopausal women diagnosed with breast cancer desire to conceive after they finish treatment. Counseling about the risk of infertility prior to cancer treatment has been proven to improve quality of life after cancer treatment. As a result of this, guidelines focus on informing women on this topic prior to treatment. However, it is equally important to provide fertility related information after primary treatment has been completed, when the wish to conceive might become actual. Therefore, the aim of this study was to identify the fertility and early menopause related information needs of young breast cancer survivors and to design, develop and implement online information material with input of stakeholders.MethodsA phenomenological qualitative study consisting of four phases was performed: identification of information needs through semi-structured interviews from a professional perspective (1) and a patient perspective (2). Exploration of stakeholders perspective regarding development and implementation of online information material (3) and development and implementation of the information material (4).ResultsProfessionals indicated that there are no guidelines regarding the provision of fertility related information during cancer survivorship. Survivors reported unmet information needs. Women identified the following as most important information needs (a) fertility preservation options, (b) the risk of menopause or infertility, and (c) long term consequences of early menopause. A wide range of stakeholders involved in breast cancer care were interviewed. Based on their proposed design the information material was implemented on a nationwide website aiming at informing and supporting breast cancer patients.ConclusionsFertility and early menopause related information needs of young breast cancer survivors and their professionals were identified. Information material has been designed, developed and nationally implemented. This way, professionals in breast cancer care are provided with an information tool that helps them meet the information needs and preferences of their patients.
