Research on user's highly sensitive privacy disclosure intention in home intelligent health service system: A perspective from trust enhancement mechanism.

ABSTRACT:

Objective

The aim is to investigate the determinants and mechanisms that influence user's highly sensitive privacy disclosure intention (HSPDI) in home intelligent health service system (HIHSS).

Methods

This study improves the privacy calculus theory by considering the influence of service providers' trust enhancement mechanism besides benefit and risk factors and investigates their impact on users' HSPDIs. This study takes perceived valence and perceived security as the trade-off result among perceived benefits, perceived risks, financial trust enhancement mechanism, and the technical trust enhancement mechanism and suggests that perceived valence and perceived security further affect users' HSPDI in HIHSS. Moreover, the common and differential effects of the perceived justice of privacy violation compensation (PJOPVC) and the perceived effectiveness of privacy protection technologies (PEOPPTs) are studied. The structural equation model is used to analyze 204 valid samples to test the proposed model.

Results

The results show that perceived benefits and perceived risks are important predictors of perceived valence and perceived security, and further affect users' HSPDI. We find PJOPVC has a greater impact on perceived valence while PEOPPT has a greater impact on perceived security.

Conclusions

We recommend that the HSPDI of users with low perceived valence can be improved by providing privacy violation compensation while the HSPDI of users with low perceived security can be enhanced by popularizing relevant knowledge of privacy protection technologies.

Project description:BackgroundHealth facilities' level of readiness to provide adolescent and youth-friendly health services (AYFHS) is crucial for preventing and reducing teenage pregnancies and maternal mortality. This study examined friendliness and satisfaction with AYFHS among users.MethodsA cross-sectional study was conducted among 419 adolescents and youths recruited from six health institutions in the East Shewa Zone, Oromia region, and Addis Ababa, Ethiopia from August 1 to October 28, 2022. Based on the WHO Quality Assessment Guidebook, the friendliness of AYFHS was evaluated as a proxy for the quality of care. Descriptive measures were computed to summarize the participants' socio-demographic characteristics. Bivariate and multivariable logistic regression analysis was used to evaluate the potential association between each independent variable and client satisfaction. The type, magnitude, and precision of association were presented using an adjusted odds ratio (AOR) with 95% Confidence Intervals (CI) and a p-value of < 0.05 ascertained statistical significance.ResultsThe overall friendliness was 69%: Specifically, 82% of the participants stated the service was accessible, 72% said it was acceptable, and 90% thought it was effective. However, only 33% and 34% of respondents reported the services were equitable and appropriate respectively. The overall satisfaction with AYFHS was 25.3%. Increased age (AOR = 0.70, 95% CI: 0.57-0.86), being female (AOR = 0.11, 95%CI: 0.04-0.33), no prior information about AYFHS (AOR = 0.20, 95%CI: 0.10-0.44), and higher number of sexual partners (AOR = 0.3, 95%CI: 0.21-0.65) were factors statistically associated with client satisfaction with AYFHS.ConclusionsThe overall quality of AYFHS was sub-optimal and did not meet the four components of WHO's good quality standards; equity, appropriateness, acceptability, and accessibility must be improved. Female and older adolescents and youths and those with a higher number of sexual partners should be targeted for intervention.

Project description:ImportanceTransparency of industry payments to physicians could engender greater public trust in physicians but might also lead to greater mistrust of physicians and the medical profession, adversely affecting the patient-physician relationship.ObjectiveTo examine the association between nationwide public disclosure of industry payments and Americans' trust in their physicians and trust in the medical profession.Design, setting, and participantsSurvey study using difference-in-difference analyses of a national longitudinal survey comparing changes in states where industry payments were newly disclosed by Open Payments with changes in states where payments information was already available because of state sunshine laws. The US population-based surveys were conducted in September 2014-shortly before the initial public disclosure of industry payments-and again in September 2016. Final analyses were conducted September through December 2018. Participants were adults 18 years and older (n = 1388).ExposuresNational public disclosure through Open Payments of payments made by pharmaceutical and medical device firms to physicians.Main outcomes and measuresWake Forest measure of trust in one's own physician and Wake Forest measure of trust in the medical profession.ResultsOf the 3542 original survey respondents, 2180 (61.5%) completed the second survey 2 years later, and 1388 named the same most frequently seen physician in both surveys. The mean age of respondents at the time of the first survey was 53 years, and 749 (54.0%) were women. Race/ethnicity was white in 76.6% (1063 of 1388) and non-Hispanic black in 8.0% (111 of 1388). Public disclosure of payments was associated with lower trust in one's own physician regardless of whether respondents knew their physicians had received payments (decrease in Wake Forest measure of trust in one's own physician of 0.56 point; 95% CI, -0.79 to -0.32 point; P < .001). Open Payments was also associated with lower trust in the medical profession (decrease in Wake Forest measure of trust in the medical profession of 0.35 point; 95% CI, -0.58 to -0.12 point; P = .004).Conclusions and relevanceNationwide public disclosure of industry payments may be associated with decreased trust in physicians and in the medical profession. More judicious presentation of payments information may counteract unintended negative trust and spillover consequences of public disclosure.
