Prevalence of and Factors Associated With Extraintestinal Manifestations and Their Remission in Inflammatory Bowel Disease: The EXTRA-Intestinal Manifestation Prospective Study From the Groupe d'Etude Therapeutique des Affections Inflammatoires du Tube Digestif.

ABSTRACT:

Introduction

Extraintestinal manifestations (EIMs) of inflammatory bowel disease (IBD) are challenging clinical situation. No prospective study assessed remission risk factors of EIMs. The aim of this study was to prospectively investigate the epidemiology, risk factors of EIM occurrence, and EIM remission in a large IBD cohort.

Methods

We conducted a cross-sectional study in 30 French referral centers. Between May 2021 and June 2021, all consecutive patients attending to hospital appointment were systematically invited to fill out a questionnaire.

Results

A total of 1,971 consecutive patients with IBD were analyzed. There were 1,056 women (53.8%), and the median age of patients was 41 years (31-54). The median disease duration was 11 years (1-18). Overall, 544 (27.6%) had at least 1 EIM. In 20.2% of cases, patients had multiple EIMs. The most frequent EIMs were rheumatological (19%) and dermatological (10%) manifestations. Immunosuppressant treatment (odds ratio [OR] = 2.56; P < 0.001) was a risk factor of EIM, while the Montreal A3 classification (OR = 0.61, P = 0.023) and male gender (OR = 0.61, P < 0.001) were associated with a lower risk of EIM occurrence. IBD current clinical remission (OR = 2.42; P < 0.001) and smoking cessation (OR = 2.98; P < 0.001) were associated factors of EIM remission. Conversely, age at IBD diagnosis (OR = 0.98; P < 0.018) was associated with a lower risk of EIM remission.

Discussion

One quarter of patients had at least 1 EIM. Beyond factors associated with the presence of EIMs, patients with IBD current clinical remission and smoking cessation are more likely to achieve EIM remission, while increasing age at IBD diagnosis is associated with decreased chance of remission.

Project description:BackgroundIrritable bowel syndrome (IBS) is characterized by abdominal pain and altered bowel habits. Further, IBS patients experience a high degree of extraintestinal symptoms.ObjectivesThe aim of this review was to describe the relation between IBS and extraintestinal manifestations and mechanisms and treatments of these extraintestinal manifestations.DesignThe study was performed as a systematic review.Data sources and methodsSearch terms including extraintestinal manifestations or somatization and IBS were used to scrutinize for publications in Pubmed. In total, 630 publications were identified and 80 were finally included in this review.ResultsAbout 50% of all IBS patients have extraintestinal manifestations in addition to gastrointestinal symptoms. Somatic pain, fatigue, and sleeping disturbances are most common, and most often described in women. Both extraintestinal manifestations and psychological distress are associated with exaggerated gastrointestinal symptoms, impaired quality of life, and difficulties to treat IBS symptoms. The extraintestinal manifestations render an excess of healthcare costs. Varying etiology and pathophysiology to IBS are discussed, and many patients express a general hypersensitivity. Extraintestinal symptoms are seldom documented at clinical healthcare or included in the assessment of treatment outcomes. A good patient-physician relationship and strengthening of coping mechanisms have rendered less gastrointestinal symptoms, psychological distress, and somatization. Altered lifestyle habits may improve both gastrointestinal and extraintestinal symptoms. Pharmacological treatment, including antidepressant drugs, should be considered when lifestyle advice fails. Teamwork between different specialists and healthcare providers may be of importance in the wide range of symptoms and extraintestinal manifestations.ConclusionExtraintestinal manifestations are common in IBS patients and is associated with worse suffering and difficulties to treat symptoms. Evaluation and treatment of IBS patients should consider also extraintestinal manifestations and their treatment outcome. Establishment of good relationship, strengthening of coping mechanisms, and education in healthier lifestyle habits are crucial in the management of these patients.

Project description:Introduction Le recours dermatologique par télédermatologie (TD) vs. consultation présentielle a été encouragé pendant la pandémie COVID-19. L’objectif de ce travail est d’évaluer l’effet de la pandémie COVID-19 sur le nombre de télé-expertises (TLX) pour cancer cutané (CC) au sein d’un réseau de TD durant les 2 vagues COVID-19 en France. Matériel et méthodes L’étude a été menée dans un réseau de TD qui permet des TLX entre professionnels de santé et dermatologues. Afin d’évaluer l’impact du COVID sur le nombre de TLX pour CC, nous avons recueilli et comparé le nombre des TLX des 2 premiers mois des 1re (mars et avril 2020) et 2e (octobre et novembre 2020) vagues françaises du COVID-19 à la même période en 2019. Les variables extraites étaient la provenance [institutions (hôpitaux et EHPAD) vs. médecins généralistes (MG)] et les diagnostics [CC vs. autres] des patients. Résultats 1re vague : Le nombre total des TLX était de 583 en 2019 vs. 629 en 2020. La proportion des TLX pour CC a diminué significativement, quelle que soit la provenance des patients (Institutions ou MG), 7 % en 2019 (41/583) vs. 1,6 % en 2020 (10/629) (p < 10−3). La majorité des CC vus par TLX étaient adressées par des institutions 83 % (34/41) en 2019 vs. 80 % (8/10) en 2020 (p = 0,82). 2e vague : Le nombre total des TLX était de 547 en 2019 vs. 600 en 2020. Il n’y avait pas de diminution significative des TLX pour CC quelle que soit la provenance des patients, 5,5 % en 2019 (30/547) vs. 4 % en 2020 (23/600) (p = 0,18). La majorité des CC vus par TLX, étaient également adressées par des institutions 86,7 %(26/30) en 2019 vs. 69,5 %(16/23) en 2020 (p = 0,18). Discussion Alors que la TD aurait pu permettre de poursuivre la prise en charge des CC pendant la 1ère vague, la diminution du nombre de CC pris en charge en TD durant la 1re vague montre qu’en période de pandémie, le contrôle de cette dernière était prioritaire chez les professionnels de 1er recours et pour les patients. Le confinement a également influencé les consultations des MG avec une diminution significative des avis de TLX par les MG pendant la première vague. En revanche, la 2e vague n’a pas eu le même impact. Après l’éducation sur le recours à la TD, l’activité des MG dans le contrôle de la pandémie ne se faisait plus au détriment de leur activité hors COVID. Les patients ont aussi à nouveau eu recours aux médecins pour des lésions cutanés d’apparition non-aiguë. Le CC est l’un des modèles d’application le plus abouti de la TD aussi bien pour le diagnostic que pour la prise en charge avec l’amélioration des délais/parcours de soin tout en limitant les déplacements inutiles des patients fragiles. Le retard de prise en charge des CC pouvant être à l’origine d’une augmentation de la morbidité, mortalité et coûts des soins, l’éducation sur le recours à la TD y compris direct des patients sont les options pour optimiser la prise en charge des CC.

Project description:Introduction: Cannabis use is common in the setting of inflammatory bowel disease (IBD). Patients frequently use cannabis to treat IBD-associated symptoms, and there is evidence that cannabis and its derivatives are helpful for this purpose. However, it is unclear how the symptom profiles of active IBD cannabis users and nonusers compare and how these symptoms may relate to their underlying disease state and/or complications. Materials and Methods: We performed a retrospective cohort study using a consented IBD natural history registry from a single tertiary care referral center between January 1, 2015 and August 31, 2020. We asked patients about current cannabis use and frequency. We also abstracted demographic and clinical characteristic information, including endoscopic severity, and totals and subscores of surveys assessing IBD characteristics, presence of anxiety/depression, and IBD-associated symptoms. We compared clinical and demographic factors of cannabis users and nonusers and developed a logistic regression model to evaluate for independent associations with cannabis use. Results: Three hundred eighty-three IBD patients met the inclusion criteria (206 females, 177 males; 258 Crohn's disease [CD], 118 ulcerative colitis, and 7 indeterminate colitis). Thirty patients (7.8%) were active cannabis users, consuming it for an average of 2.7 times per week. Cannabis users were more likely to report abdominal pain (83.3% vs. 61.7%), gas (66.7% vs. 45.6%), tenesmus (70.0% vs. 47.6%), and arthralgias (53.3% vs. 20.3%) compared to those that did not use cannabis (p<0.05 for each). Incidence of moderate-severe endoscopic inflammation was similar between cannabis users and nonusers, while CD-associated complications were more common in nonusers (39.1% vs. 69.7%, p<0.05). The only factor that demonstrated a significant association with cannabis use on multivariable analysis was arthralgia (p<0.01). Discussion: Active IBD cannabis users were more likely to report a variety of symptoms, including abdominal pain, gas, tenesmus, and arthralgias. However, they did not demonstrate more frequent active disease or IBD-associated complications, suggesting that other nonluminal factors influence their symptoms and/or decision to use cannabis. These findings demonstrate the importance of evaluating for extraintestinal contributors to symptom burden in IBD cannabis users, as well as the ongoing need to develop safer and more effective methods for recognizing and managing abdominal pain and other symptoms in this setting.
